fire extinguishing system must automatically shut down
all engines that draw intake air from within the
protected space. The pre-engineered fixed gas fire
extinguishing system protecting lazarette did not
automatically shut down crane/winch engine which draws
intake air from within the protected space. The engine
in lazarette may not be used until fixed fire fighting
shutdown failure is diagnosed, repaired, and tested to
the satisfaction of the attending Marine Inspector.

Vessel is restricted to maximum deck loading capacity by 75% and crane lifting capacity by 75%.

requirements. Port-side hull was found wasted (pinhole
leak) below the waterline under the ladder in forepeak.
Provide gauging report for area ivo wastage, along with
repair proposal to attending Marine Inspector. forepaeak
shall be inspected daily while underway & kept dry.
Daily inspection shall be logged until deficiency
cleared.
