Every tank vessel subject to the regulations in this subchapter shall be inspected every 5 years or more often, if necessary, by the Coast Guard to see that the hull, boilers, machinery, equipment, apparatus for storage, and appliances of the vessel comply with marine inspection laws, and the regulations in this subchapter, and when applicable, subchapters E, F, J, O, Q, S, and W of this chapter and 33 CFR parts 155 and 157. Vessel COI has expired. Conduct COI Renewal Inspection with local OCMI prior to any cargo operations or trades. Vessel is currently located at IMT Shipyard, Lemont, IL and is restricted from movement until COI Renewal Inspection is completed.

Vessels draft marks are missing or hand painted in the wrong locations by freehand. Bottom of each mark must indicate draft on each corner of barge.

Make permanent repairs to barge hull due to the following damage discovered: 1" x 1/8" fracture and a 1/2" x 1/16" fracture located in the #1 WT approx 6ft from the port forward bulked and 3ft inboard from the port side.

The tank top and associated external longitudinal stiffeners are badly wasted. Barge may load cargo one more time and transit to offload and clean tanks. Once clean the barge is to return to Paducah, KY for further evaluation and permanent repairs.

Repair oil leaks on the Cargo Pump prime mover. Oil leaking from block on both sides of engine and going onto the deck. Proper operation shall be conducted to the Satisfaction of an attending Marine Inspector.

Repair or renew hatch cover where wasted around the inner gasket lip in way of #8 port wing void.

PRIOR TO LOADING NEXT CARGO, MAKE REPAIRS TO STARBOARD SIDE SHELL IN WAY OF STERN VOID. LEGACY COMPONENT DESC: Side Shell

REMARK VESSEL NAME AND HAILING PORT ON STERN.

Renew all flame screens prior to loading cargo.

FRACTURED LAP WELD IN THE PORT STERN VOID WRAPPER PLATE AT THE 3' DRAFT MARK. DISCHARGE CARGO AND MAKE PERMANENT REPAIRS TO THE SATISFACTION OF THE COGNIZANT OCMI PRIOR TO LOADING THE NEXT CARGO.

MAKE PERMANENT REPAIRS TO 12" FRACTURE IN WAY OF STARBOARD SIDE OF STERN LOG DECK KNUCKLE NO LATER THAN COMPLIANCE DATE.

PROVE PROPER OPERATION OF EMERGENCY SHUTDOWN FOR CARGO PUMP ENGINE, PRIOR TO CARGO TRANSFER OPERATIONS, NO LATER THAN THE COMPLIANCE DATE.

PROVIDE PROPER CARGO TRANSFER PROCEDURES AS DESCRIBED IN 33 CFR 155.750, PRIOR TO THE COMPLIANCE DATE.

CONDUCT HYDROSTATIC TEST OF ON DECK CARGO PIPING AND PROVIDE PROOF OF SUCH TEST TO THE COGNIZANT OCMI PRIOR TO THE COMPLIANCE DATE.

MAKE #3 CARGO TANK AND AFT PORT CARGO TANK SAFE FOR ENTRY AND AVAILABLE TO A USCG MARINE INSPECTOR TO ENSURE BULKHEAD INTEGRITY.

MAKE PERMANENT REPAIRS TO THE FOLLOWING AREAS: (A) 8" FRACTURE ON #3 CARGO TANK TOP 8' FROM PORT SIDE IN WAY OF 2ND TANK TOP WELD SEAM. (B) 8" WASTED AREA AT AFT PORT CORNER OF #3 CARGO TANK TOP-MAIN DECK SEAM. (C) PIN HOLE IN AFT STBD CORNER OF #3 CARGO TANK TOP @ STERN VOID BULKHEAD.

Repair leak in vicinity of shaft packing to cargo pump prior to next cargo transfer and not later than compliance date.

PERFORM ANNUAL HYDROSTATIC TEST OF CARGO PIPING (LAST TESTED 22JAN90).

TEST CARGO TRANSFER PIPING, SYSTEM RELIEF VALVE AND PRESSURE GAUGES IN ACCORDANCE WITH 33CFR156.170. PROVIDE WRITTEN DOCUMENTATION TO OCMI LOUISVILLE PRIOR TO 15SEP86.

10" CRACK IN PORT CORNER WRAPPER, 4" CRACK AT 5' DRAFT IN SIDE SHELL OF #7 STBD WING VOID, AND 2" CRACK AT 6' DRAFT IN STBD SIDE SHELL OF BOX STERNCAUSED BY IMPACTS WITH UNKNOWN OBJECTS.

CRACKS IN SIDE SHELL AND BULKHEADS IN #6 AND #7 STBD WING VOIDS. MAKE PERMANENT REPAIRS TO THE SATISFACTION OF THE ATTENDING USCG MARINE INSPECTOR.
