INDICATOR

Each collision bulkhead required by § 179.210, must be constructed in accordance with § 179.320, except that a collision bulkhead:(1) Must extend to the weather deck or to one deck above the bulkhead deck, whichever is lower, for service on oceans or coastwise routes; and(2) Must not be fitted with any type of penetration or opening except penetrations may be made if they are located as high and as far inboard as practicable and they have a means to make them watertight. Collision bulkhead had several non-watertight penetrations near the top of the bulkhead. Make collision bulkhead penetration watertight to the satisfaction of the attending marine inspector.

n interconnected detection and alarm system must:(1) Consist of multiple-station smoke detectors listed by an NRTL, or independent laboratory accepted by the Commandant according to 46 CFR subpart 159.010, as meeting UL 217 (incorporated by reference, see 46 CFR 175.600);(2) Be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system;(3) Contain an independent power source; and(4) Alarm on low power. Vessel's generator space smoke detector did not function and system was in error state. Vessel replaced five interconnected detectors and provided satisfactory test. Deficiency rectified on site.

(b) At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: (b) Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. (3) Adult and (2) Child Kapok life jackets were deemed to be unserviceable condition. Vessel replaced (3) Adult and (2) Child life jackets on site. Deficiency rectified.

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel's was missing previous month's inspection logs. Vessel had April and conducted May while onboard. Deficiency rectified on site.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces:(1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette. Fish hold bilge alarm acuation did not produce visual alert at the operating station. Provide attending marine inspector proof of visual alarm at operating station when fish hold bilge level switch is activated.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16) (incorporated by reference; see 46 CFR 175.600). (1) adult and (1) child life jackets retroflective tape was faded. Replace both lifejackets retroflective tape to the satisfaction of the attending marine inspector. Vessel's passenger capacity is reduced by (2) passengers until the deficiency is rectified.

Electrical equipment exposed to the weather must be watertight. Both stbd floodlight electrical junction boxes were missing covers and wasted. Replace both floodlight electrical junction boxes.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel's (3) fire buckets on board at the time of inspection were only 2-gallon capacity. Replace fire buckets with 2.5 gallon capacity type.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel's dry dock examination date was overdue on 01FEB24. Vessel is not to carry passengers until a satisfactory dry dock examination is completed.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel incurred hull damage to stbd midship above the chine rail during dry docking. Submit repair proposal to marine inspector. Repair damaged area.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB hydrostatic release was found expired (2022 expiration). Provide current hydrostiactic release.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel did not install an interconnected fire detection system by 27DEC2022. Vessel is restricted to a LIMITED COASTWISE route and is prohibited from allowing passengers to utilize overnight accomodations until it's compliance with the interconnected fire detection requirements of 46 CFR 181.405 have been verified by a USCG marine inspector.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Both vessel electronic plotter and paper charts were found to be over three years old. Provide current electronic or paper charts.

The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Posted emergency instructions were not vessel specific and listed equipment not carried on vessel. Provide vessel specific emergency instructions.

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge Marine Inspection (OCMI) must comply with this part. A bait/galley refrigerator has been installed after the vessels annual COI inspection. The original (manufacturer) electric cord has been cut and spliced at two locations with 14AWG wire by twisting he wires together and covering with electrical tape. The cord was then plugged into an outlet protected by a 20AMP breaker. (14AWG incompatible with 20 AMP breaker).

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. An open ground was found in the forward passenger berthing electrical outlet.

A vessel must comply with applicable requirements for any radio and Electronic Position Indicating Radio Beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Title 47 CFR Part 80.148: Each compulsory vessel, while underway, must maintain a watch for radiotelephone distress calls on 156.800 MHz (Channel 16) whenever such station is not being used for exchanging communications. Vessel operator failed to respond to Channel 16 after forty minutes and activating blue L/E lights.

Fixed fire fighting system shall be able to shutdown machinery and prevent it from starting unless it is overridden. Engines were able to be started without overriding them.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB hydrostatic release expired in March 2020. Provide current hydrostatic release.

An adult life jacket must be provided for each person carried on board a vessel. (04) adult lifejackets were condemned due to degraded straps. Total persons on board reduced by (04) persons. Total persons allowed on board now (41).

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Aspirin was found expired. Provide complete and current first aid kit.

Exhaust gas must not leak from the piping or any connections. Exhaust piping on both port and starboard MDE exhausts found with pinhole leaks forward of water injection points. Repair and demonstrate proper operation to attending inspector.

Abandon ship drills, monthly EPIRB tests, and monthly portable fire extignuisher inspections wer not conducted or logged.

Mattress shall meet the standard of 16CFR1632 or prove mattress is original to vessel. O/O shall provide letter attesting to its age & proof of fire retardancy.

Vessel shall have 03 2.5 gallon fire buckets. Vessel is missing fire buckets but has a washdown pump. Provide 03 2.5gal fire buckets.

Provide proper means of escape. Vessel ladder was missing a couple of rungs. Install the missing ladder rungs.

Hatches shall have a captive chain. Captive chain was missing. Provide a captive chain.

Secondary means of escape was partially obstructed due to bulkhead.

Aisles alongside bunks where joined were obstructed by a freezer and was less than 42 inches.

Renew expired EPIRB battery; vessel restricted to 3nm from shore.

Install blank in electrical panel on the bridge.

Install 30x30 mesh on fuel tank vents.

Repair minor cooling water leak on port MDE.

Replace glass lens on magnetic compass or renew compass.

Renew retro-tape on lifering.

Perimeter grab line on the ring buoy showed excessive U/V damage. O/O procured and installed new R/B prior to the completion of the inspection.

Found broken wire to electical rule pump in fish hold. Wire repaired prior to the completion of the inspection.

Renew broken EPIRB antenna. Antenna insulation found wasted away down to metal. New EPIRB antenna purchased and installed.

Portable fire extinguisher did not have any restraint from swaying. Vessel operator installed a bungie cord belly band to keep extinguisher tight to the bulkhead.
