It shall be the duty of the master to see that all watertight doors in subdivision bulkheads that may be opened at sea, and all mechanisms, remote controls, and indicators therewith, shall be periodically inspected at least once in each week that the vessel is navigated to be assured that they are in proper operating condition. Low Pressure alarm for WTD 3 is active in both pilot houses.

TACOMA reported they have a leak coming from the deck scupper on the car deck #2 end STBD between frames 33 and 34. They found a small puddle on the deck below in the #2 end storeroom. They cannot identify exactly where the pinhole is, but it appears to be coming from the scupper that runs from the center tunnel to the wing tunnel at the curb. These have been problem areas for us.

Doors, other than those which are normally locked, such as from staterooms, fan rooms, lockers, etc., shall be of the self-closing type capable of closing against a 3 1/2 degree list, and such doors shall be numbered in accordance with subpart78.47-35 of this sub-chapter. MI observed fire screen door #38 failed to fully close and latch when released.

Where pipes are carried through watertight or oiltight bulkheads, the integrity of the structure must be maintained. Stern tube seal self reported as leaking approx. 1 gal per minute into the bilge.

An inspection, either general or partial depending upon the circumstances, shall be made whenever any important repairs or alterations are undertaken. Vessel to repair damage to Texas deck at next dockside availability. Vessel crew to prohibit passengers from damaged deck area.

Issued CG-835V for multiple ventilation closures found seized/ unable to close.

IAW ASTM F1197-19 (11.4) Power control systems shall close doors at an approximately uniform rate of closure, but in no case shall the closure time, from the time a door begins to move to the time it reaches the fully closed position, be less than 20 s or more than 40 s. Shaft alley door on the number one (1) end closed in 13 seconds.

Fire on the number 2 drive motor, alpha 1 cabinet.

Stern tube is leaking oil due to a faulty stern tube seal. Replace seal prior to movement. 33 CFR 155.5021

Repair the NFU steering control system. 2A and 2B pumps surge/hunt in NFU mode from the pilothouse. 46 CFR 58.25-10

Prove the proper operation of the inport generator. The governor control system could not maintain 1800 RPM. Complete PSTP: IPG-1, 4, SPM, 1, 2 prior to using the inport generator. 46 CFR 62.25-15

The visual indicator for the No.3 MDE did not indicate on a low lube oil alarm. Repair/provide a visual indicator. 46 CFR 62.25-25

The CO2 system shall auto shutdown the power ventilation in the engineering paint locker. The supply fan remained online. 46 CFR 76.15-35

Prove the proper operation of the No.1 pilothouse shaft RPM meter. The meter displayed 8-9 RPMs at 5 turns. 46 CFR 62.35-5

Leak on saltwater flange on cooler piping in #1 engine room. Crew must maintain rounds to observe if leak rate increases past 1-2 drips/seconds. Vessel must replace piping to ensure permanent fix of flange.

Fire doors 21 and 33 did not fully close. Show proper operation of fire doors.

Vessel must have fully functioning public address system for transmission of orders and information throughout vessel/ Must fix sound powered phone circuit to not automatically feed into 1MC.

All fire screen doors must be capable of release from control station and from a position at the door. Ensure 02 galley doors and FSD 09 and 12 are capable of this function.

Hazardous Material Training record must reflect training documentation for new employees within 90 days and recurring training once every 3 years.

Passenger accomodation spaces must be made ready for caqrriage of passengwers under safety general provisions.

JHOC rcvd call from WSF Opcen reporting the #2 end whistle is stuck open and inoperable.

Make the vessel's fuel oil tanks ready for inspection by the vessels next drydock.

Implement WSF's periodic safety test procedure (PSTP) corrective action plan as indicated on the SIP submittal dated JUL2020 within 90 days.

Doors installed in class A fire boundaries, except those which are normally locked shall be of a self-closing type. Remove all hold-back devices and prove doors are self-closing.

Provide operating instructions with a block diagram of the steering gear control system and power units permanently displayed on the bridge and steering room.

Replace the painted and deteriorated No.1 end, P/S void space vent gaskets.

Mark general alarm bells throughout the vessel.

Complete outstanding PSTP items not performed.

Prove the proper operation of the harbor generator's L/O shutdown. The manufacturer's manual specifies min L/O of 15 PSI. The generator shutdown at 4 PSI.

Provide an updated CO2 service report. The service report provided was missing the annual cylinder weights.

Reference CG-ENG-4 letter dated 11/15/2020 (attached)Case-by-Case approval of the Elkhart 241-95 Inline Foam Eductor. The use of this eductor is contingent upon using an appropriate nozzle that provides enough water and pressure in accordance with the manufacturers requirements to appropriately discharge the foam. The operating and maintenance instructions (attached) state that the "Elkhart eductors are calibrated to operate at 200 psig inlet pressure... The eductors can educt foam concentrate at inlet pressures as low as 100 psig when used with a recommended constant flow nozzle but the foam concentration will be richer". Provide objective evidence that the Elkhart 241-95 Inline Foam Eductor is able to discharge firefighting agent with the correct mixing rate of foam concentrate according to the guidelines from the foam manufacturer for the particular foam concentrate carried onboard. This must be completed prior to replacing the existing foam carts.

#1 drive motor over-current relay light went off. Determine cause or switch out faulty relay.

In ventilation systems, fire dampers or other suitable means shall be provided for shutting off the passage of air in the event of a fire. The following fire dampers are non-operational: Fan room #2, sundeck lounge, #1 end, manual pull. Fan room #3 above panel intake handler, 5-5-9-1, manual pull. Fan room #3, passenger deck, #2 end starboard, manual pull. Fire damper label 5-09-2(1) on ceiling above door to men's head, no damper behind ceiling. Fire damper E1-19-1 in stairwell to engine room is stuck in the open position. Vessel has 7 days (5Apr19) to correct deficiencies.

Foam agent, its container, and all control valves for the operation of the system shall be of an approved type. The AFFF concentrate in the ready tanks located on the upper car deck needs to be laboratory tested to prove effectiveness, and manufactures specifications needs to be provided on the spare containers to prove effectiveness. Additionally, functional testing and crew proficiency with equipment is required.

Each life-saving appliance must be inspected monthly using the checklists required under 46 CFR 199.190 (b)(1)(i) to make sure the appliance and the equipment are complete and in good working order. A report of the inspection, including a statement as to the condition of the equipment, must be recorded in the vessel's official logbook. Maintenance records for the vessel lifesaving equipment were not available because the records are being maintained with the watch-specific crew documents. Maintenance logs must be kept on board the vessel in the official logbook.

ll doors, except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a postion at the door. Fire screen door (FSD), #1 end, upper car deck port side does not close and requires labelling. FSD 10 does not fully close. FSD 25 does not latch shut. Provide identification label for FSD, #1 end, upper car deck port side. All items were corrected on the spot and do not require further action, excluding the label for the referenced FSD.

Each section of fire hose must be lined commercial fire hose that conforms to UL 19. Fire station 43 hose was punctured and in unsatisfactory condition. The hose was condemned and replaced with a new hose. No action required.

Vessel auxiliary boilers past inspection due date. Tag out both boilers and may not use until required hydro test and inspection complete to USCG satisfaction.

Fire screen door #34 failed to self-close. Door self-closing device was repaired on spot.

Child life jackets were stored with adult jackets intermingled. Adult life jackets properly re-stored by crew.

Lock-out CP223, breaker No. 5 "Galley food prep area ltg". The breaker shall remain to be locked until the circuit wiring integrity (from panel breaker to outlet box) can be established via 500 volt megger test or above. Conduct fire and smoke rounds in the affected space at least every 15 minutes for the first 4 hours of operation. Thereafter, if no indications (smell or visual) are noted the additional rounds may be discontinued.

Found Main Propulsion switchboard cabinets in both engine rooms are not fitted with dripshield as required. Repair proposal must include engineering calcuations ensuring that the explosion lelif characteristics of the cabinet, designed by the original manufactuer, are maintained.

Prove proper oepration of #1 Boilwe and complete PSTP to the satisfaction of the attending marine inspector.

Vessel is restricted from operating from sunset to sunrise and during restricted visibility until proper operation of navigation lights.
