Each vessel that operates of the high seas must have on board a FCC Type Accepted Cat 1, 406 MHz EPIRB, installed to automatically float free and activate. MI identified the EPIRB hydrostatic release did not have expiration date marked on unit.

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirement of one of the standards listed below for the hull material of the vessel. MI identified several areas of the aft deck with delamination that exposed the glass laminate to seawater intrusion

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. MI Identified inoperable anchor light and stern light.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens of flame arrestors. the flame screen must consist of a single screen of corrosion resistant wire of at least 30x30 mesh. MI identified non-metallic screens fitted over the discharge end of both fuel tank vents.

When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. The Marine Inspector (MI) identified that the fuel tank fill pipes were not electrically bonded to the tank. Conduct repairs to the satisfaction of the attending Marine Inspector. 46 CFR 182.445(g)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. The Marine Inspector identified cracked welds along the engine room door hatches; 4 on the port side and 1 on the starboard side. Conduct repairs to the satisfaction of the attending Marine Inspector. 46 CFR 177.100

A vessel must comply with the applicable requirements for any radio…installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years…If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. The Marine Inspector identified that the Radiotelephony Certficate was expired. Complete a satisfactory FCC Inspection and obtain a valid Radiotelephony Certificate. Each…ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The Marine Inspector identified that the ring life buoy's markings were weathered and illegible. 46 CFR 185.604(b)

Each ring life buoy with a floating waterlight must..secur[e] the waterlight around the body of the ring life. The Marine Inspector identified that the waterlight was not properly attached to the ring life buoy. 46 CFR 180.70(d)(2)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Due to extended dry docking period, vessel did not complete annual inspection within +/-90 day grace period; conduct annual inspection prior to the carriage of passengers. 46 CFR 176.500 (b)(1)

If during the inspection of a vessel, the vessel or its equipment is found not to conform to the requirements of law or the regulations in this subchapter, the marine inspector will point out deficiencies observed and discuss all requirements with the owner. Wood rot present in the following areas and require repair: main passenger cabin outboard of longitudinal frames on both sides; aft weatherdeck corner and outboard frames as well as framing near engine room ventilation trunks; port head overhead. Starboard head paneling requires removal for inspection. Submit repair plan prior to commencing repairs and coordinate satisfactory repairs with Sector SF Marine Inspector.

If during the inspection of a vessel, the vessel or its equipment is found not to conform to the requirements of law or the regulations in this subchapter, the marine inspector will point out deficiencies observed and discuss all requirements with the owner. Forward hydraulic line under helm console is leaking and saturating wood deck that it passes through. Replace hydraulic line and affect repairs to deck.

The intent of this part is to insure that the scantlings, material, fastenings, workmanship and arrangement of each vessel are such as to provide for safe operation in accordance with the terms of its certificate of inspection. The report provided by the third party indicated excessive rot on the salon roof. Remove all paneling to allow for further inspection of the material condition of the salon roof. 46 CFR 177.01-5 (a) "OLD T"

No major repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge and approval of the Officer in Charge, Marine Inspection. On 28DEC2023, temporary repairs were made in multiple locations on the vessel after rotting wood was identified. Submit repair proposal for the following to the satisfaction of the marine inspector: 1. Replaced forward post on the port side windshield on the side of the wheelhouse. 2. Replaced inner structure of the upper part of the windshield. 3. Replaced door opening, structure, and lower section of the door and wall on the starboard side of the wheelhouse. 4. Replaced plywood above the forward windows on the port and starboard side of the salon. 46 CFR 176.20-1 (a) "OLD T"

Each marine employer shall ensure that random drug tests conducted under this part are unannounced and that the dates for administering random tests are spread reasonably throughout the calendar year. Company has designated a vessel Master to serve as the Company Designated Employer Representative (DER). This arrangement does not allow for a random selection of crewmembers as intended by this regulation. Company to assign responsibilities of DER to an employee that is not one of the members that shall be randomly drug tested as required by this regulation.

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free. Vessel's IBA painter line was not attached to the weak-link that would allow the IBA to detach when the vessel sinks. Vessel attached painter line to weak-link to the satisfaction of the attending marine inspector.

The master of the vessel shall ensure that each EPIRB must be tested monthly, using the integrated test circuit and output indicator, to deterfmine that it is operative. The EPIRB test required shall be logged or otherwise documented. Vessel did not have monthly EPIRB tests logged. Vessel to show that they are testing EPIRB monthly via vessel log to the satisfaction of the marine inspector.

As appropriate for the intended voyagew, a vessel must carry adequate and up-to-date U.S. Coast Pilot or similar publication. Vessel's Coast Pilot was out of date. Vessel to procure latest edition to the satisfaction of the marine inspector.

No flammable or combustible liquids may be stowed in any accommodation, control, or service space, other than a paint locker. Vessel has over 20 gallons of oil and paint stored in forward compartment. Vessel to remove all oil and paint to the satisfaction of the marine inspector.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical outlet in forward compartment missing face plate. Vessel to replace face plate to the satisfaction of the marine inspector.

Each hatch exposed to the weather must be watertight. Engine compartment overhead hatches gasket material does not provide a watertight tight barrier. Vessel to replace gasket material to the satisfaction of the attending marine inspector.

Machinery and fuel tank spaces shall be separated from accommodation spaces by watertight and/or vapor tight bulkheads. Bulkhead between fuel tank compartment and engine compartment has multiple bulkhead penetrations. Vessel to fill penetrations to the satisfaction of the marine inspector. 46 CFR 182.35-1(a)

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Vessel does not have records of EAP training. Vessel to provide evidence of training of the supervisor as well as training to the crewmembers to the satisfaction of the marine inspector.

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Vessel does not have employer's policy displayed. Vessel procured and displayed company's policy to the satisfaction of the attending marine inspector. 46 CFR 46.401(a)

An adult life jacket must be provided for each person carried on board a vessel. 05 of the adult life jackets were of kapok design and were leaking air.

Vessels shall be inspected for compliance with the standards...acceptable to the Officer in Charge, Marine Inspection, as good marine practice. There were two sections of deck railing, one on the bow and one port side of the vessel, that were not secured in place. 46 CFR 176.25-1(a) "Old T"

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. There was no expiration date marked on the hydrostatic release for the EPIRP.

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. The painter line was not attached to the hydrostatic release of the IBA.

An adult life jacket must be provided for each person carried on board a vessel. Five lifejackets were found to have not passed inspection. Vessel limited to no more than total 45 persons on board.

Each rescue boat and life ring buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Retroreflective material was found to be faded beyond the ability to reflect light.

Each opening in an exposed weather deck must have a means for closing it weathertight. Main deck is leaking around engine compartment hatch. This is causing rot in the wood deck beams in that compartment. Repair proposal submitted. Deficiency due to be corrected by 28FEB2022.

A vessel must be operated in accordance with applicable laws and regulations in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Deck above steering compartment is soft and lacking support. Repair proposal submitted. Extended: Deficiency due to be corrected by 28FEB2022.

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces. Electrical cable penetrating bulkheads in and out of engine room needs to be secured for vapor barrier protection.

A vessel must be operated in accordance with applicable laws and regulations in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Bilge pumping arrangements was found to have multiple issues: 1. Steering compartment bilge pump wiring is lacking support. Remove wiring from bilge, clear and dead end wires, and properly support wiring. 2. Bilge pump by fuel tanks has excessive hose length that is causing excess strain on the pump. Shorten hose length to appropriate length. Extended: Deficiency due to be corrected by 28FEB2022.

Strong and effective rudder stops must be provided to prevent jamming and damage to the rudder and its fittings. These stops must be structural or internal to the main steering gear. Vessel is not fitted with rudder stops. Submit repair proposal within 30 days. Extended: Deficiency due to be corrected by 28FEB2022.

A vessel is inspected for compliance with the standards required by this chapter. Machinery equipment materials and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptance to the cognizant OCMI as good marine practice. Valve for bilge pump prime has wasted from the pump side of the line and is unable to be capped. New valve and piping required to be installed at through hull penetration. 46CFR176.800

At each initial and subsequent inspection for certification of a vessel the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of all accessible parts of the exterior and interior of the hull. Vessel was found to have damage on the stem just above the waterline creating a thin spot in the fiberglass. Repair damage to original construction. 46CFR176.802(a)(1)

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. IBA painter was found to be attached to vessel instead of weak link on hydrostatic release. 46CFR180.130(a)(2)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Drills were not conducted because the boat did not get underway due to a no sail item. Drills are required to be conducted prior to carriage of passengers. 46CFR176.808(g)

The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. Wire run on engine room forward bulkhead was not vapor tight. 46CFR177.405(a)

Found 02 life jackets with retro that was wasted.

Life jacket placard was unreadable due to fading.

Engine room bulckhead had several penetrations through cable run through holes and had wasted gasket on engine room hatches IAW 46CFR 177.405(e)

Electrical bilge pump discharge hose in fuel tank room had puncture leak.

Alcohol Strips were expired.
