Fixed fire-extinguishing systems shall be checked as noted in table 71.25-20(a)(2). Carbon dioxide: cylinders must be tested and marked, and all flexible connections on fixed carbon dioxide systems must be tested or renewed, as required by 147.60 and 147.65 of this chapter. Carbon dioxide cylinders forming part of fixed fire extinguishing system must be retested at least every 12 years. Flexible connections between cylinders and distribution piping of fixed carbon dioxide fire extinguishing systems must be renewed or retested at a pressure of 1000 psi. The test must be performed when the cylinders are retested. Fixed CO2 cylinders last hydro date is 12/2013 and are overdue for the 12 year hydro. Flex hoses marked as installed or tested on 12/2013 and are also overdue. All bottles and hoses due for hydro must be satisfactorily retested prior to to commercial operations.

An abandon ship and a fire drill, as described in 199.180, must be conducted on each passenger vessel at least weekly. The entire crew does not have to be involved in every drill, but each crewmember must participate in an abandon-ship and a fire drill each month.

At least three reinspections shall be made on each vessel within one year. These reinspections will be made at approximately equal intervals between annual inspections. In the case of vessels with a seasonal schedule, reinspections will be made during the operating season if practicable.

Hot water heating boiler control system does not provide a pre-purge period. Small oil fired heating unit control systems are required to provide a pre-purge of sufficient length to provide four changes of air in the combustion chamber and stack but in no case less than 15 seconds. MFG date on boiler is 10/87. 46 CFR 63.10-20(c) (1970).

Fuel oil pump on the #1 generator is leaking fuel. Repair or renew leaking pump, make oil tight.

Fuel soaked absorbent pads present under #1 generator and oil soaked pads under reduction gears. These absorbent pads must be removed. Pads shall be renewed regularly to prevent the unacceptable accumulation of combustible liquids (fuel/oil).

Fuel filter housing (front of engine) on #2 main engine is leaking fuel in way of drain plug. Make fuel filter housing oil tight.

Emergency generator fuel injector pump leaking fuel. Make pump housing oil tight.

05 emergency lights on car deck are extinguished. Both E-lights on port side, 01 on starboard side, and 02 on center car deck. 46 CFR 78.17-45

Alcohol test swabs onboard are all expired. 46 CFR 4.06-15(a)(1)

Plastic on ring buoy located on the bridge wing above rescue boat is cracked and needs to be replaced. 46 CFR 71.25-15

46 CFR 111.79-1(c) Each receptacle outlet must be compatible with the voltage and current of the circuit in which it is installed. Identified 15-amp receptacle on a 40 amp circuit (panel 207) in the engine room. Complete review/update to one-line drawing and submit to Marine Safety Center for review within 30-days.

The position of the rudder, if power operated, must be shown at the principal steering station. #2 rudder angle indicator on the #2 end not installed correctly. 46CFR113.40-5

For propulsion machinery spaces intended for centralized operations, alarms for abnormal conditions (pressure and temperature) of the cooling media and automatic safety system functions are to be provided. Unable to test High Temp Alarm on Main Diesel Engines during inspection. 46 CFR 58.01-5 ABS Rules 4-6-5/7.7.5

Repair and demonstrate proper operation of currently inoperable heat activated detector for emergency generator space CO2 fixed extinguishing system.

Demonstrate proper operation of both main propulsion engine overspeed trips or provide an authorized third party report indicating successful testing was performed.

Demonstrate proper operation of or provide an authorized third party report attesting to the proper operation of the auxiliary boiler pressure relief valve. 46 CFR 61.05-10

Repair and demonstrate proper operation of main steering sump low hydraulic oil low level alarms on both #1 and #2 end steering systems. No alarm was actuated upon attempted test. 46 CFR 58.25-25 (d)(3)

Repair and demonstrate proper operation of general alarm light in emergency generator space.

Provide updated Fuel Oil Transfer procedures with revised procedures for reporting discharges. Current procedures contain outdated reporting contact information as Washington State Ferry offices. 33 CFR 155.750

Each survival craft must be serviced at an approved facility within 12 months of each subsequent servicing. One of the IBAs was due to be serviced in February. Required to get the IBA serviced prior to the carriage of passengers. 46 CFR 199.190 (g)

Every crewmember must participate in at least one abandon-ship drill and one fire drill every month. Vessel has not been operating due to COVID. Required to conduct drills prior to the carriage of passengers. 46 CFR 199.180 (c)

As far as is reasonable and practicable, rescue boats, other than lifeboats which are also rescue boats, must be launched with their assigned crew aboard and maneuvered in the water each month. Such launching and maneuvering must occur at least once every 3 months. Required to lower and maneuver rescue boat in the water prior to the carriage of passengers. 46 CFR 199.180 (d) (7)

Noticed several emergency lights burned out. Required to ensure proper operation of all emergency lights prior to the carriage of passengers. 46 CFR 112.43

The minimum sound-pressure levels for the emergency-alarm tone in interior and exterior spaces must be a sound level of not less than 80 dB(A). The GA in the Salish Room did not produce an adequate sound level. Required to fix prior to the carriage of passengers. 46 CFR 113.25-12

The public address system was not audible in the Salish room. Required to fixed prior to the carriage of passengers. 46 CFR 113.50-5

A vessel that is certified to carry more than 150 passengers is required to have a properly installed USCG approved AIS Class A. The display is damaged and not legible. Required to repair or replace. 33 CFR 164.46 (b)

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. The time on the instructions does not match the time delay bottle. Redo instructions for the installed 30 second time delay. 46 CFR 76.15-10 (h)

One of the fire hose nozzles' quick release was malfunctioning. Required to repair or replace. 46 CFR 76.10-10

Submit plans to the USCG's Marine Safety Center to prove the structural adequacy of the tent installations is suitable for its intended service. The vessel's operation is limited such that the tent arrangement shall not be subject to apparent wind in excess of 30 MPH.

Remedy and/or address all items denoted on the HIYU's regulatory review (attached). Dated July 2, 2019.

The vessel owner or operator must ensure that they maintain one English language copy of the VRP, at a minimum the contents listed in paragraphs...of this section and a copy of the Coast Guard approval letter, onboard the vessel. In lieu of paper format, the vessel owner or operator may keep an electric copy of the VRP and approval letter onboard the vessel. Approval letter was onboard the vessel and avaible for inspection. Obtain approval letter within 30 days.

All subdivision bulkheads must be watertight. Provide temporary repair in way of caulking at the cable penetration located frame 18 prior to carrying passengers. Thereafter make permanent repairs with manufactures approved blocking system.

All stowage locations for lifesaving equipment must be labeled with the number of devices in that location. Label the amount of lifejackets in each stowage location.

Modify lifesaving plan to reflect changes in equipment. Resubmit to OCMI for approval.

Prove the electrical modifications in way of the receptacles were part of approved electrical plans. Submit the original drawing with list of materials to the OCMI for renewal.

Cables must be installed in accordance with IEEE 45-2002. Install metallic bonds for cable installations.

At least one lifebuoy on each side of the vessel be fitted with a life line. Secure life lines to the body of the lifebuoys IAW 46 CFR 199.70.

Replace all batteries in the floating water-lights onboard as they are expired IAW 46 CFR 199.190(f)(4)(5).

Liquid Propane Gas (LPG) installation for the food truck onboard the vessel must be approved, installed and tested in accordance with approved engineering requirements in sub-chapter "F" Marine Engineering IAW 46 CFR 58-16.
