Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. MI observed active leak on upper bearing of rudder post post vessel was maneuvering. Repair bearing on rudder post to the satisfaction of OCMI. 46 CFR 182.100

Vessel must carry adequate and up-to-date: charts of large enough scale to make safe navigation possible; U. S. Coast Pilot or similar publication, Coast Guard Light List, Tide Tables; and Current Tables. Operator could not provide charts or publications. Provide charts and publications to the satisfaction of MI.46 CFR 184.420

A vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. If posting is impracticable, the stability letter must be kept on board in a weathertight container readily available for use by the crew and display to passengers and others on request. Operator posted outdated stability letter on vessel. Provide updated stability letter and post as practicable. 46 CFR 176.306

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Operator could not provide alcohol testing devices. Provide alcohol testing devices onboard or in office.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Operator could not provide servicing for pre-engineered fixed gas fire extinguishing system. Provide servicing report for pre-engineered fixed gas fire extinguishing system. 46 CFR 176.810(b)(2)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. MI observed degraded placards for donning life jackets. Replace placards for donning life jackets. 46 CFR 185.516(a)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. MIs observed multiple items in the first-aid kit were expired. Replace all expired first-aid items within one week. 46 CFR 184.710

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. MIs observed the vessel only had 2 buckets on board and they were only 2 gallons. Replace all buckets with the right size and number of buckets within one week.

A pre-engineered fixed gas fire extinguishing system must: (1) Be approved by the Commandant; (2) Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector; (3) Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space; and (4) Be installed in accordance with the manufacturer's instructions. (b) A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (1) A light to indicate discharge; (2) An audible alarm that sounds upon discharge; and (3) A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. Vessel's pre engineered system has a independent toggle switch spliced in to overide function and does not have an audible alarm when it is actviated.

Vessel completed a repower prior to inspection, fill out the attached word document and return to the Coast Guard.

Ring life buoy grablines were wasted, purchase and install a new ring life buoy approved in accordance with 46 CFR 160.050.

Bilge pump indicator light was inoperable, complete repair.

Forward masthead light housing was cracked, install new housing.

Electrical wiring in forepeak obstructed lifejacket access, mount electrical wiring to forepeak overhead.

A pre-engineered fixed gas fire extinguishing system must automatically shut down all engines that draw intake air from within the protected space. Engine did not shut down during simulated activation of system.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Vessel does not have emergency instructions.

Provide means to demonstrate emergency steering.

SH03: Post copy of COI on boat, visible to passengers.

SH04: Provide Coast Pilot [name withheld], and up to date chart.

SH05: Organize forward life jacket storage area.
