Vessel shall enact permanent repairs to the satisfaction of class and provide documentation attesting to correction to [email withheld] .

After any survey of the ship under regulations 7, 8, 9 or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the administration. Due to multiple marine casualties occurring where the ship lost main propulsion, the vessel's crew installed a new refrigerated dryer system to replace the old desiccant air dryer system for the control air system on the main engines. The new air system is not currently reflected in the approved ship's drawings for the air system, preventative maintenance system including updating the spare parts list, or daily round check sheet for the engineers on watch. 74 SOLAS(2) Ch. I Reg. 11(b)

MI was notified by the Mobile Pilots and confirmed with the vessel's master that the vessel suffered multiple engine start failures when transitioning from stop to ahead. A damage survey shall be completed by the RO (ABS) and the results provided to the Coast Guard marine inspector for acceptance prior to departure. 74 SOLAS (2004 cons.) II-1/46.2

Received notification that on 05NOV23 at 1800, vessel conducted pre-arrival checks approximately 95 miles west of Egmont Key Sea Buoy. Engineers tested astern propulsion, then tested the forward propulsion. Upon going forward, fuel cam shaft abnormality alarms sounded for both the forward and astern directions. Engineers proceeded to redo the test, the forward abnormality alarms cleared however, the astern #2 cylinder alarms remains active. 74 SOLAS (04) II-1/46.2

Where the emergency source of electrical power is a generator, it must be started automatically upon failure of the main source of electrical power supply. The master of the T/V OREGON reported a failure of the emergency generator to start and operate when tested by the crew. The emergency generator is unable to provide an emergency source of power upon failure of the main source of electrical power supply. SOLAS 2004 cons. Ch II-1 Reg 43.1.1
