The construction and arrangements of a vessel must allow the safe operation of the vessel IAW the terms of its COI giving consideration to provisions for a seaworthy hull. During inspection, the port rudder post was observed to be leaking and compromises watertightness in a critical hull penetration. Identify the source of the leakage and conduct appropriate repairs to restore watertight integrity. All work must be verified by the attending Coast Guard Marine Inspector. 46 CFR 177.100 701c

The owner/operator shall have the vessel ready for inspections of the hull structure and all accessible parts of the exterior and interior. In the lazarette, identified cracks in welds, structural members, transom, and bottom plate around the struts, rudder posts, and shaft tubes. These cracks resulted in active water intake. Drydock and survey vessel to identify all affects areas; conduct approved repairs.

Repairs to the hull that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. In the lazarette, identified numerous existing unauthorized repairs which failed, resulting in active water intake. Crop and renew affected areas.

A vessel must be provided with a fixed self-priming bilge pump with a minimum capacity of 10 gallon per minute. Vessel operators were unable to operate bilge system due to unfamiliarity and possible pump malfunction. Conduct training and prove pump operation.

The vessel must be provided with individual bilge lines and bilge suctions for each watertight compartment. The bilge suction in the lazarette wasted and sheared at threads approximately 24" above suction and check valve. Repair and prove proper operation of the bilge suction.

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. The bilge suction in the engine room was clogged with debris. Clean and prove proper operation of the bilge suction.

The owner/operator shall have the vessel ready for inspections of the hull structure and all accessible parts of the exterior and interior. In the forepeak, an inset on the port bow resulted in side shell cracking in numerous areas; unauthorized repairs used to mitigate the cracks have failed. Crop and renew affected areas.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Fwd ER bulkhead has bolt holes from an abandoned fire extinguisher bracket. Repair bulkhead.

A hatch on a vessel that operates only on protected waters must be either watertight or weathertight. Gaskets on engine room hatches have failed; preventing weathertightness. Renew gaskets.

Fire fighting and man overboard drills and training shall be logged; including date and general description of the scenario and training topics. Vessel failed to conducted monthly drills and document as required. Conduct drills and document correctly.

A vessel of at least 7.9 meters (26 feet) in length must be provided with individual bilge lines and bilge suctions for each watertight compartment, Upon examination the bilge pumping system was not able to pump accumulated water from the space aft of the collision bulkhead. Vessel representative attempted repair of the system by removing the strainer and restarting pump with no success. Owner/Operator to repair system and notify attending inspector for visual confirmation of proper operation. 46CFR182.510(a) 60c.

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 (incorporated by reference; see 46CFR175.600) or be specifically approved by the Commandant. MI observed severely deteriorated flexible nonmetallic fuel hose at the fuel tank in the aft steering space. Owner/Operator to replace all unsatisfactory fuel hoses with approved material. 46CFR182.720(e) 60c.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. MI observed fuel leaking at the supply fitting before the port main engine primary fuel filter. Owner/Operator to repair leak and notify attending inspector of repairs. 46CFR182.100 60c.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. MI observed both aft engine room ventilation intakes unable to be closed off with provided plates due to lost and improper hardware. Owner/Operator to make repairs to all engine room supply intakes ensuring proper closing devices operate as designed. 46CFR182.465(h) 60c.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. MI observed starboard sea chest piping leaking at the pipe nipple before the valve. Owner/Operator to make repairs and notify attending marine inspector of repairs. 46CFR182.100 16c.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. MI observed the VHF antenna broken off at the base and attached to the mast with a plastic electrical tie wrap. Owner/Operator to replace the antenna with acceptable replacement and provide notification to attending marine inspector of repairs. 46CFR176.800(a) 16c.

Repairs or alterations to the hull that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel must submit a repair proposal for the cracking and clad weld in the steering compartment around the port strut, to the satisfaction of the attending marine inspector prior to starting repairs.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Inspector discovered cracking in the hull at the port strut post in the steering compartment leaking water through the hull into the space. Crop and renew hull plating around the port strut post to the satisfaction of the attending marine inspector.

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck. MI observed the valve was stripped and inoperable. 46 CFR 182.510(d) 60c

Starboard aft gunwale in the steering gear compartment was not watertight and water was leaking into the space. 2. Cracked welds on the port rudder post 3. Port engine onboard exhaust lagging was loose and the last 1’ of piping before water inlet was damaged. Repair or replace to satisfaction of attending marine inspector. 46 CFR 182.425(b)(3) 10c

Port engine exhaust couplings were loose and hose clamps were missing. Repair exhaust couplings and replace hose clamps to satisfaction of attending marine inspector. 46 CFR 182.410(d)(1) 10c

Port and starboard engine mounting bolts were loose. Tighten or replace to satisfaction of attending marine inspector. 46 CFR 176.800(a) 10c

Second void space has cracked and missing welds on the starboard side upper aft corner. Reweld and make repairs to satisfaction of attending marine inspector. 46 CFR 176.800(a) 10c

Bilge pick up tube located in steering space has severe chaffing from to mounting clamp. Replace affected piping to satisfaction of attending marine inspector. 46 CFR 182.500(a) 10c

Port battery has loose terminals. Tighten terminals and repair to satisfaction of attending marine inspector. 46 CFR 183.350(d) 10c

Port and starboard battery boxes have metal covers that are chaffing on the cables. Repair to the satisfaction of the attending marine inspector. 46 CFR 183.350(e) 10c

Replace flexible tubing from cooling system to exhaust. Hoses are kinked. Fix leak on flexible tube for exhaust supply cooling.

Vsl has installed CO2 system but does not have required fire detection system or auto actuation of CO2 as required by 46 CFR 181.400 c.

Reposition mast head light and anchor light to ensure visibility as per Navrules. Do not conduct night time operations until proven to the satisfaction of attending marine inspector.

INSTALL APPROPRIATE SIZED BILGE STRAINERS IN ALL COMPARTMENTS.

VESSEL TO MAKE PERMANENT REPAIRS TO BOTH STRUT WINGS AND RUDDER TUBES WHERETHEY CONNECT TO HULL AND ARE CRACKING. NO DOUBLERS WERE INSTALLED DURING NEW CONSTRUCTION, CAUSING THIS PROBLEM. VESSEL TO PROCEED TO GRAN BOIS TO EFFECT REPAIRS.

PROVIDE PROOF OF FIXED FIRE EXTINGUISHING SYSTEM PLAN APPROVAL.

Conduct simplified stability test as reference vsl for remaining sister vessels to be constructed out of Intracoastal Iron Works.

INSTALL HIGH LEVEL BILGE ALARMS IN ALL SPACES WITH THROUGH HULL FITTING BELOW THE WATERLINE IAW 46 CFR 182.530.
