Cargo ports and other similar openings in the side of ships below the freeboard deck shall be fitted with doors so designed as to ensure watertightness and structural integrity commensurate with the surrounding shell plating. PSCO observed the upper edge of the cargo port sealing surface not seating properly due to gasket failure and a wasted steel plate. In addition, PSCO observed a pinhole-sized hole approximately 2 inches above the deck in the center of the door, surrounded by wastage in the steel.

Oil filtering equipment shall be of a design approved by the Administration and shall be such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15ppm. Vessel's installed separating/filtering equipment is type A.393(X). The system has been taken down by the vessel's master due to a repair required filter second stage cartridge.
