All piping, valves, and fittings shall be in accordance with good marine practice and suitable for the purpose intended. Fire main pipe corroded and leaking along span from starboard exhaust stack to top deck. Submit permanent repair proposal and affect approved repairs to satisfaction of MI. Old T 46CFR 181.15-5(c)

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. The ground tackle and mooring lines provided must be satisfactory for the size of the vessel, the waters on which the vessel operates, subject to the approval of the cognizant [name withheld] shows significant corrosion wastage in several areas.46 CFR 121.300

Second deck bow found with areas beyond 25% wastage that require permanent repair by 31OCT2024. Except on a vessel limited to daytime operations, the compass must be illuminated. The compass onboard the vessel did not illuminate.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF–DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). The vessel did not have its latitude and longitude coordinates programmed into the VHF-DSC radio.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessels Port/Starboard shaft seal was dripping excessively. Vessel to tighten shaft seal packing glands to the satisfaction of the marine inspector. 46 CFR 182.05-1 "OLD T"

AIS Class A device. The following vessels must have on board a properly installed, operational Coast Guard type-approved AIS Class A device: (iii) A self-propelled vessel that is certificated to carry more than 150 passengers. Vessel was found with an AIS that is not transmitting their location. Repair AIS to the satisfaction of the Marine Inspector. 33CFR164.46(b)(1)(iii)

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space, including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Operator reported wastage in the hull plating that is causing water intrusion into the lazarette. Identify extent of damage during drydock using NDT. Submit repair proposal for approval prior to starting work. Make permanent repairs to the satisfaction of the attending Marine Inspector prior to carriage of passengers.

Each entrance to a space storing carbon dioxide cylinders... must be conspicuously marked as follows: “CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.”. CO2 storage space does not have proper sign. Install sign to the satisfaction of the marine inspector. 46 CFR 122.612(i)(1)

...Each vessel inspected and certificated under the provisions of this subchapter must, when any passengers are aboard during the tenure of the certificate, be in full compliance with the terms of the certificate. COI requires vessel to designate senior deckhands in writing, in accordance with NVIC 1-91. Vessel did not have a document clearly indicating Senior Deckhands. Create document stating Senior deckhands, to the satisfaction of the marine inspector. 46 CFR 115.100(b)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Hole found in deck from the main deck to the CO2 void, however, the location of the water intrusion to the main deck could not be determined. Determine location of water intrusion. Submit repair proposal for approval. After proposal is approved, make permanent repairs to the satisfaction of the marine inspector. Rectify prior to receiving credit drydock. 46 CFR 115.840

Examine all weathertight closures above weather deck and inspect the provisions for drainage of sea water from the exposed decks. Weather tight door on second deck does not close properly and has extensive internal damage. Repair to the satisfaction of the marine inspector prior to receiving credit drydock. 46 CFR 176.25-5(a)(5) "OLD T"

All enclosed spaces within the vessel shall be properly vented or ventilated. Where such openings would endanger the vessel under adverse weather conditions, means shall be provided to close them. Ventilation closure on starboard side, second deck, had corrosion. Repair to the satisfaction of the marine inspector prior to receiving credit drydock. 46 CFR 177.20-1(a)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Engine was leaking hot coolant. Engine was repaired and bilge cleaned to the satisfaction of the marine inspector. 46 CFR 115.830(b)

A vessel must carry one ring buoy with a floating light. Ring buoy outside pilothouse has inoperable light. 46 CFR 180.70

Repairs or alteration to the hull machinery, or equipment that affect the safety of the vessel must not be made without approval of the cognizant OCMI, except during an emergency. Vessel struck antoher vessel causing exterior and interior damage while getting underway from pier. Vessel is not permitted to movement prior to an MI conducting a damage assessment and internal structural exam to ensure that no damage occured from mishap.

Make permanent repairs to holes in deck IWO main deck bar currently repaired with a doubler in no more than 60 days.
