A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The FCC Safety Certificate and FCC Radio Authorization is expired. 46 CFR § 184.502

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. The port rudder packing gland is leaking water into the port lazarette space. The leak is approximately one liter every 5 days. The packing gland shall be replaced and made watertight. 46 CFR § 176.700

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (1) A light to indicate discharge; and (2) An audible alarm that sounds upon discharge. The audible and visual alarm is not operational on the port side FireBoy system protecting the machinery space. The audible alarm is not operational on the starboard side FireBoy system protecting the machinery space. 46 CFR 181.420(b)

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J–1942 (incorporated by reference; see 46 CFR 175.600) or be specifically approved by the Commandant. The following restrictions apply: For the purpose of flexibility only, flexible hose installed in lengths of not more than 760 millimeters (30 inches) and subject to pressures of not more than 35 kPa (5 psig), may meet the following requirements: USCG Type A1, A2, B1, or B2 flexible hose may be accepted in accordance with 33 CFR part 183, subpart J. Flexible hose was found installed on the port and starboard fuel tank delievery lines and next to the main engine fuel pump. The flexible hose is less than 30" but does not meet USCG A1 fuel hose standards. 46 CFR 182.720(b)

All equipment, including switches, fuses, lampholders, etc., must be suitable for the voltage and current utilized. Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. Conductors must be protected in accordance with their current carrying capacities. Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Each circuit breaker must meet UL 489 (incorporated by reference; see 46 CFR 175.600) or other standard specified by the Commandant. The primary branch overcurrent protection on the starboard side is a 100 amp 220V AC double pole breaker which does not protect the 12V DC 8 AWG conductor based on its current carrying capacity or DC current. The port side primary overcurrent protection could not be identified by its current rating or breaker size. Both overcurrent protection devices will be replaced with a UL listed fuse or UL 489 breaker rated at the conductor's current carrying capacity. 46 CFR 183.380

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. The port and starboard starting batteries are not secured to prevent shifting or movement. 46 CFR 183.350(b)

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. The terminal strip for the 12V DC circuit located under the operating console was found with heavy corrosion creating resistance and heat when energized. 46 CFR 183.200(b)

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The battery conductors to the main engine are not supported and installed in the bilge. 46 CFR 183.340(b)(4)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. The port and starboard machinery space have a rectangular cover over the access to each space that is not vapor tight which permits the passage of the FireBoy agent outside of the space if discharged. 46 CFR 182.465(h)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The port and starboard main engines do not have guards for the rotating belts which endangers personnel entering the engine room while the machinery is energized. 46 CFR 176.830

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies. Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The master was unable to provide a record of crew drills. 46 CFR 185.520

The lights prescribed by these Rules shall, if carried, also be exhibited from sunrise to sunset in restricted visibility and may be exhibited in all other circumstances when it is deemed necessary. The stern light was not operational when tested and the arc of visibility is larger than what is permitted by the navigation rules. 33 CFR 83.20

VSL must comply with FCC requirements for station license. On board FCC station license expired on 2021.

Master shall ensure that monthly test of EPIRB is performed and logged. No records of conducted EPIRB monthly test.

VSL operates on high seas must have on board FCC Type, Cat I, 406 MHz EPIRB. Found on board EPIRB with expired NOAA registration and battery.

Fixed gas fire extinguisher system must be service and maintained annual. Last services conducted on board fire protection equipment was APR 2021.

Wire must be protected from environment. Found several bilge pumps with improper wire connections, conductors are exposed to bilge water.

Company must has drug and alcohol program. No records of current drug and alcohol program o f board.

Vessel inspected under subcahpter T may not operate without having on board a valid USCG COI. Vessel's hull exam and is overdue.

VSL must have valid FCC Safety Radio Certificate. Provide proof of valid FCC Certificates.

Vessel must conduct crew training once every three months. Provide proof of conducted training.

Provide up-to-date light list and tide tables.

EPIRB must have current NOAA registration. Provide proof of EPIRB registering with NOAA.

FGFE must be service and test annually by qualified individual. Proved proof of completed services.

First aid kit content must not have expired items. Replace expired items in first aid kit.

Vessel must have valid FCC Safety Certificate as required by 47CFR80. Provide proof of valid FCC Safety Certificate.

A space with non-watertight closure on main deck must have bilge high level alarms as required by 46 CFR 182.530. Install bilge high level alarm in port and starboard space with non- water tight hatches.

Vessel must have up-to-date light list, coastal pilot, local notice to mariners and charts as required by 46 CFR 184.420

Replace non-metallic hoses on reduction gear heat exchanger.

Replace wasted through-hull fittings on port & starboard exhaust discharge pipes.

Remove dead end wires in port engine compartment.

Repair inoperable operating indicator for forward bilge pumps.

Repair inoperable bilge high level alarm for starboard steering compartment.

Provide two portable lights.

Replace wasted hatch frames on forward water tight hatch.

Vessel must be in compliance with standard for machinery required in 46 CFR 176.800. Conduct repairs and demonstrate proper operations of the port propulsion engine.

All wire and cable must be protected from the weather as required by 46 CFR 183.340. wire conductor to mast all round light are exposed. Make proper wire protection

Portable CO2 must be in good general condition as required by 46 CFR 176.810. Replace (02) portable CO2 with excessive corrosion.

Vessel must have visual and audible alarms at the operator station as required by 46 CFR 182.530. Repair inoperable bilge high level alarm in port steering compartment.

Deck rails must be designed and construct to withstand a point of load of 200 lbs as required by 46 CFR 177.900. Repair upper deck hand rail with cracked welds.

Propulsion engine must have jacket water discharge temperature gauge as required by 46 CFR 182.410. Repair inoperable engine temperature gauge on port engine.

Vessel 26 and 65 feet long must have 1 fixed power pump and 1 portable hand pump or fixed hand pump as required by 46 CFR 182.520. Install missing fixed hand pump.

Waste receptacles must be constructed of non combustible material withy no opening in the side or bottom as required by 46 CFR 177.405. Replace existing plastic waste receptacle.

Submersible electric bilge pump must meet requirements in UL 1113 as required by 46 CFR 182.520. Demonstrate proper operation of the bilge pump in port forward space.

Battery component must be in operational status as required by 46 CFR 186.806. Repair inoperable ammeter for STBD battery.

Program MMSIM on VHF's DSC and conduct training to crew as required by 47 CFR 80.905(a)(1).
