Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. MI identified the hydrostatic release for the EPIRB is expired.

All vessels shall be provided with bilge pumps in accordance with table 182.25-10(a). MI observed the fixed power pump inoperable.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. MI identifies the screw down valve for suction from the forepeak tank was not labeled in any conspicuous manner.

At least one ring life buoy must be fitted with a lifeline. MI identified the ring life buoy lifeline excessively frayed.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. MI identified missing and/or worn gasket material on all hatches and weathertight openings.

Where the principal business of the vessel requires the discharge of personnel in a seaway, the peripheral rails may be wholly or partially omitted or reduced in height to not less than 30 inches. When such rails are omitted, center rails or other suitable hand holds shall be substituted therefor. MI observed missing handrails, center rails, or suitable hand holds missing on the back deck of the vessel.

Each hatch exposed to the weather must be watertight. Each hatch cover must have securing devices. MI observed no securing devices on lazerette and engine room hatches.

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel leaves port. MI observed life float severely cracked, lines degraded, and not in a float free configuration. 46CFR185.700(a)

All vessels must have navigation lights that are in compliance with the compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65feet) must also have navigation lights that meet the UL1104 standards. MI observed navigational lights damaged and not permanently affixed. 46CFR183.420

Each rescue boat and life ring buoy must be marked with Type II reflective material. Be stowed in a way that it can be rapidly cast loose. MI observed life ring buoy with worn reflective material and line zip-tied together. Make corrections to the satisfaction of the attending MI. 46CFR185.604(i) 46CFR180.70(b)(3)

All vessels except non-self propelled vessels shall be provided with suitable steering apparatus. MI observed hydraulic steering pump gauge broken and inoperable. 46CFR182.30-1

A hatch exposed to the weather must be watertight. Marine Inspectors witnessed no gasket in the lazarette hatch, making the hatch not watertight. Install gasket to the satisfaction of the attending Marine Inspector.

A vessel on a limited coastwise must carry six hand red flare distress signals and six hand orange smoke distress signals. Vessel was missing two hand red flare distress signals, provide two additional distress signals using 46 CFR 180.68(c) as a guide if needed.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who holds a marine radio operator permit or higher class license where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power. The radio operator did not have a marine radio operator permit or higher class license, vessel cannot operate commercially without a radio operator on board who holds the above permit or higher.

Each major generator and motor shall be fitted with a nameplate of corrosion-resistant material marked with the following information as applicable: Name of manufacture, Manufacturer's type and frame designation, Output in KW or watts or horse-power rating, kind of rating, revolutions per minute, Amperes at rated load, frequency, number of phases and an ambient temperature of 50 °C (122 °F). Generator on board did not have a placard or proof of generator specs including the ambient temperature that it was designed for. Provide proof that the generator is designed to operate at the ambient temperature of 50 °C or have the generator de-rated in accordance with 46 CFR 183.320(b)(2).

The first aid kit was missing medication and contained medication that had expired. Replace to the satisfaction of attending Marine Inspector.

The FCC Radiotelephony Certificate was expired.

Vessel's ship service generator has oily windings. Clean and identify source of the oil and provide findings to attending USCG Marine Inspector.

The battery in the light attached to the life buoy was inoperaable. Replace battery and provide proof of operation to the satisfaction of the attending CG Marine Inspector.

Light attached to the life float was inoperable. Replace and provide proof of operation to the satisfaction of the attending CG Marine Inspector.

Lifefloat found cracked with frayed webbing and worn away data plate.

Fire/bilge pump reach rod was unlabled and inoperable and firehose was not installed. Hand operated pump onboard was inadequate/unsuitable to the task.

Oil found on generator windings, needs to be cleaned.

Vessel needs to have a life-saving maintenance log onboard.

Vessel needs to have a waste management plan onboard.

Replace expired aspirin and burn gel from the onboard medical kit.

Flexible fuel and jacket water lines were found cracked and bulging which need to be replaced.

Vessel's ship service generator has oily windings clean and identify source of oil then present findings to attending Marine Inspector.

Vessel is missing FCC Radiotelephony certificate. Present certificate to attending Marine Inspector.

Engine room lights are missing protective glass and guard. Replace immediately. Portside light is missing guard and starboard side light is missing glass.

Multiple fractures found in bulwards on the bow and stern of the vessel. Make repairs at next scheduled drydock.

Provide Employee Assistance Program information onboard.

Provide proper dayshapes for vessel.
