DESTINY

Except for continuously manned operating stations, each accommodation space, control space, and service space must be fitted with a smoke actuated fire detection system of a type approved by the Commandant. Fire detection system displays trouble, prove proper operation of the fire detection system.

A pre-engineered fixed gas fire extinguishing system must have a means to reset devices used to automatically shut down engines. The overide switch for the port main engine is not functioning properly. Repair to the satisfaction of the attending USCG MI.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts. Vessel did not have current charts, provide current charts on board.

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the location of emergency exits, survival craft embarkation areas, and ring life buoys, the stowage location(s) of life jackets, the location of the instruction placards for life jackets and other lifesaving devices, that all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master. Provide a passenger safety orientation announcement.

A hold back mechanism for a door must allow the door to be released locally, upon a signal from a control space, and upon disruption of the power system. Holdback mechanism for the 03 deck fire screen door is not functioning. Prove proper operation of hold back mechanism.

On a vessel of more than 65 feet in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. 5 speakers on the 02 deck were not operational. Prove proper operation of the speakers.

"A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years." 46 CFR 176.600(c)(1)

"...must be so insulated w/ approved structural insulation..." Ceiling located in the aft of the engine room is missing insulation. Address the insulation. 46 CFR 116.415

"All unfired pressure vessels must be installed to the satisfaction on the cognizant OCMI." Air compressor located in pilot house does not have a pressure relief valve. 46 CFR 119.330

"Each life jacket must be marked with Type I retroreflective material." Several life jackets have worn retroflective material. 46 CFR 122.604

"Battery chargers must have an ammeter connected in the charging circuit." Ammeter not located on battery charger. 46 CFR 120.350

"...construction and arrangements... must allow... protection against fire." Port and starboard gaskets are worn for both ventilation closures. 46 CFR 116.100

"Comlete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place..." Instructions and diagram not located for the system in engine room. 46 CFR 122.612

The owner or managing operator shall notify the cognizant OCMI as far in advance as possible whenever a vessel is to undergo repairs or alterations affecting the safety of the vessel, together with the nature of any repairs or alterations contemplated. Vessel had alternation conducted on steering gear, installing 02 motor controllers without approval by MSC or OCMI. Vessel is prior to pax until plan review is conducted and approved by MSC.

All microprocessor or computer based systems must meet the requirements of part 62 in subchapter F of this chapter. Both Port & Stbd MDE ZF control microprocessors are showing red fault lights (2) and code 9110 is flashing. Must investigate & clear code.

Fire pump must be capable of both remote operation from the operating station & at the pump; vessel does not have either. Must fix to regulation.

Each fire hydrant must have a valve installed to allow the firehose to be removed while the fire main is under pressure. Vessel fire station #4 valve leaks by so the fire hose cant be removed while fire pump is on.

Vessel has holes on main deck in passenger area above both MDE, creating a fire spread hazard and vapor hazard. Must be fixed to like new condictions.

Fire control boundaries; missing structural fire protection insulation in main deck forward media speaker room bulkhead. (46 CFR 116.415(a)(2))

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. Missing fire stop in cable penetrations in the main deck forward media speaker room. (46 CFR 116.415(a)(2)(ii))

Cable and wiring requirements; All cable and wire must: Be installed with metal suppports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps mustm be limited to bundling or retention of multiple cable installations, and not used as a means of support. Support missing in main deck forward media speaker room. (46 CFR 120.340(b)(4))

Carbon dioxide warning signs. Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: Spaces storing carbon dioxide - "CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION." (46 CFR 122.612(i)(1))

Carbon dioxide warning signs. Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: Spaces protected by carbon dioxide - "CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING." The reference to wintergreen scent may be omitted for carbon dioxide systems not requird to have odorizing units and not equipped with such units.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: "EMERGENCY EXIT, KEEP CLEAR". All emergency escapes require proper marking. (46 CFR 122.606)

Complete but simple instructions for the operation of a fixed gas fire extinguishing systems must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schmatic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Each control valve to a distribution line must be marked to indicate the space served. Bow thruster fixed fire system is not marked properly. (46 CFR 122.612)

Verification of vessel compliance with applicable stability requirements. Max passengers for top deck listed on stability must be marked on th aft stairs. (46 CFR 122.315)

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and teh vessel from electrical hazards, including fire, caused by or originating in electrical e3uqipment, and lectrical shock. 3rd deck bar daisy cord electrical strip must be removed. (46 CFR 120.200(b))

Compressed gases; Stowage and care of cylinders; Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, wiht the valve protection cap in place. 2nd deck bar CO2 cylinder not secured. (46 CFR 147.60(b)(1))

Machinery and equipment installed on each vessel must be suitable for the evessel and its operation and for the purpose intended. Must rectify fuel line leak on bow thruster. (46 CFR 119.100)

Portable lights. Each essel must be equipped with at least two operable portable battery lights. One of these lights must be located at the oerating station and the other at the access to the propulsion machinery space. Portable light at operating station is inop.

The fire detection and alarm system must be so arranged and installed that the presence of a fire in any of the protected spaces will be automatically registered visibly and audibly in teh pilothouse or fire control station. The visible notice must indicate the zone in whcih the alarm originated. Pull station located outside of restrooms must be properly labeled on the fire panel. (46 CFR 76.27-5(f))

Except as otherwise allowed by this subpart, a vessel must comply witht he structural design requirements of one of the standards listed below for the hull material of the vessel...Steel hull vessels operating in protected waters - Rules for Building and Classing Steel Vessels for Service on Rivers and Intracoastal Watersways, ABS. 2nd deck aft port and starboard wasted bulwark at deck must be cropped and renewed. (46 CFR 116.300(c))

A vessel must have a sufficient number of fire hydrants to reach any part of the vessel using a single length of firehose. Due to the vessel modification (adding length), the firehoses do not reach the stern of the vessel. (46 CFR 118.310(a))

All microprocessor or computer based systems must meet the requirements of part 62 in subchapter F of this chapter. A qualitative failure analysis (QFA) of the vessel's propulsion engine control system (throttle) should be submitted. The QFA should specifically address the failure of the microprocessor based control system hardware and those inputs to the control system whose failure could affect the propulsion system's ability to default to the required conditions in 46 CFR 121.620(c). The QFA must indicate continued compliance with 46 CFR 121.620, so that power failuer to the processor or processor failure will not result in an increase of shaft speed or propeller pitch. The QFA should be based on single, non-concurrent system failures (including loss of power). A DVT Procedure (DVTP) document that incorporates all failure modes identified on the QFA should be submitted for review. Onboard DVTs shall be witnessed by the Coast Guard and should be performed immediately after installation, or before the issuance of the initial Certificate of Inspection of the vessel. The DVTP should enable the inspector to verify compliance with Ref. (a) based on failure of the processor control system. Onboard DVTPs should include verification of status of the propulsion plant upon loss of power to the processor. a)Recovering a person in the water directly from the Small Passenger Vessel itself: The intended recovery point should be visible from the control station. Exceptions may be permitted where the operator can move quickly between the controls and the observation area without losing control of the vessel or endangering the person in the water. b)Use of a rescue platform as an alternative to a rescue boat: Locations where platforms will be rigged should be visible directly from the control station. Exceptions may be permitted where the operator can move quickly between the controls and the observation area without losing control of the vessel or endangering the person in the water. Rescue platforms must be sturdy enough to support at least two 165 pound persons, and must be able to withstand the effects of wave action. If a portable platform is used: a)It must be versatile enough to be used in several locations on board the vessel, so as not to restrict its use under any deck loads or sea conditions. d)Means shall be provided to secure it to the vessel with a lanyard or lifeline. Vessels station bill and emergency instructions are not tailored to the vessels operation during emergency situations as demonstrated during the COI inspection. Must conduct training, adjust station bill to actual on-board situation, add crew members if necessary, and if wait staff/cooks/food handlers are intended to be used that request must be in writing to ensure members meet regulation and policies. 46 CFR 122.420 46 CFR 122.508 46 CFR 122.512 46 CFR 122.514
