Repairs to the hill that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] was notified of a repair to the vessel's deck and had not approved written specs describing details of the proposed repair. 46 CFR 176.700(a)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferable from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Electric solenoid valves must not be used, unless used in addition to the manual valve. MI observed fuel shutoff at the tank connection does not have means to be manually operated from outside the space. Vessel must submit plans for modification.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. MI observed both forward bench stowage areas for life jackets missing identification of items stowed inside. Mark locations of all PFDS.

A hatch cover must have securing devices. MI observed engine room maintenance hatch missing means of securing hatch. Provide securing device to hatch cover.

Where a flexible fill pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. Such hose must overlap metallic pipe ends at the least 11/2 times the pipe diameter and must be secured at each end by clamps. The flexible section must be accessible and as near the upper end of the fill pipe as practicable. When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. MI observed grounding wire missing from fuel tank fill pipe. Provide grounding wire for fill pipe.

Conductors in power and lighting circuits must be No. 14 American Wire Gauge (AWG) or larger. MI observed undersized power wiring for deck lights. Replace wiring with proper 14 AWG boat wire or remove installation.

A vessel must have at least one ring life buoy fitted with a lifeline. Inspector found that the lifeline and grab line on the aft ring life buoy were deteriorated and not fit for service.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date Chart, U.S. Coast Pilot [name withheld] List, Tide Tables and Current Tables. Inspector found that the paper navigation charts and navigation publications were not up-to-date.

Electrical equipment on a vessel must be installed to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Inspector found wire connections within the steering console to be made with electrical tape and twist cap connectors. Also, bare electrical wires were found exposed at the connections.

A vessel must comply with the applicable requirements for any radio installation including the requirements for a station license and installation certificate issued by the Federal Communications Commission. Inspector found that the Radiotelephony Certificate was expired.

46 CFR 180.70(d)(3) 10c Each floating water light must be attached to the lanyard with a corrosion-resistant clip. Water light clip was corroded and could not open. Vessel must replace corroded clip with new, water-resistant clip.

A visual and audible alarm must be provided at the operating station for bilge high level. Forepeak visual alarm is inop.

Repair gasket for engine room hatch to make weathertight.

Replace PVC piping used in bilge system in the engine room.

Replace PVC plugs in unused sea valve hull connections with metal plugs.

Provide strainer on the engineroom bilge suction foot.

Make vessel available for an Inspection for Certification. Vessel is overdue for a COI renewal inspection as required by 46 CFR 176.404.

Port MDE exhaust pipe coupling fell off the turbo as a result of the 4 bolts holding it to the turbo shearing, due to a design/installation flaw. Result was that the exhaust sooted up the engine air intake, causing loss of propulsion.

Provide bowls under fuel filters in the engine room IAW 46CFR182.455

Resolve loose cable in port side of engine room IAW 46CFR183.200

Provide non-conductive cover over terminal block in engine room IAW 46CFR183.200

Make vessel available for annual inspection in accordance with 46 CFR 176.500 (b)(1).

Provide repair proposal for port/starboard stern tubes.

Make permanent repair to port/starboard stern tube (shaft log).

Prove Proper operation of propulsion system to include a check ride.

Determine source of oil leak from starboard MDE and make necessary repairs.

Provide FCC Safety Certificate, route reduced to LBS, no more than 1,000 feet from shore.

REmove excessive oil from bilge prior to carrying passengers.

Provide proof of pre-employment testing, and conduct Drug and Alcohol audit prior to carrying passengers.

Company unable to provide proof of enrollment in a random drug testing program in accordance with 46 CFR 16.

Watertight bulkhead penetrations in engineroom forward and aft bulkheads need to be water tight in accordance with 46 CFR 179.320(c).

Renew/replace flame scree in port side fuel vent on cargo deck in accordance with 46 CFR 182.450(e).

Provide medium frequency capabilities in accordance with oceans route and 47 CFR 80.905(a)(2). Vessels route limited to Limited Coastwise.

Provide lifesaving equipment for 100% of passenger capacity in accordance with 46 CFR 180.202. Vessel's passenger carrying capacity reduced to 22, one lifefloat removed temporarily for repairs.

Plug valve openings in engine room FWD watertight bulkhead.

Install machinery guard on fan belt for STBD engine.

Weld crack on FWD portside frame in compartment FWD of the engine room.

REPLACE LIFE LINES ON RING BUOYS AND BUOYANT APPARATUSES.
