Each vessel must be marked as required by part 67, subpart I, of this chapter: The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel in clearly legible letters not less than four inches in height. Name and Hailing Port markings were removed for for hull painting. 46 CFR 185.602(a)

If a vessel operates on exposed or partially protected waters, an opening port light is not permitted below the weather deck unless it has an inside, hinged dead cover. Hinged dead covers for port and starboard-side port lights located in forward crew accommodation below main deck did not close properly. Provide means to close that is sufficient enough to prevent water intrusion if glass windows fail. 46 CFR 171.119(a)(2)

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. Light fixture in damage prone location of forecastle missing a guard. 46 CFR 183.410(a)

A lighting fixture may not be used as a connection box for a circuit other than the branch circuit supplying the fixture. The light fixture in forecastle was used as a junction box and outlet. 46 CFR 183.410(b)

Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Approval of bowls of other than steel construction will specify if a flame shield is required. A pair of fuel filters forward of starboard engine are missing flame shields. 46 CFR 182.455(b)(6)

All cable and wire must be installed with metal supports spaced not more than 24 inches apart, and in such a manner as to avoid chafing. Various wire runs in the forecastle and black water void were found without required metallic supports and run through holes in aluminum deck stiffeners without chafing protection. 46 CFR 183.340(b)(4)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. E/R breaker box on starboard side had an opening/missing breaker creating a potential shock hazard. Remove hazard to satisfaction of MI. 46 CFR 183.200 (c)

Drills required by § 185.524 (fired drills and training), including fire location and fire type; and Emergency egress training for each member of the crew, to occur for all members of the crew—(i) At least monthly while such members are employed on board the vessels; and (ii) Each time a crew member joins the crew of such vessel. Vessel drill log revealed there were several-month gaps between entries. Drills conducted and logged during inspection. 46 CFR 185.420

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including ... Operation of the fire main system... During test of fire pump, Marine inspector observed discharge piping leaking at the pipe fitting. Vessel not to carry more than 49 passengers until satisfactory permanent repair is made. 46 CFR 180.175(d)

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems... must be conspicuously marked as follows: (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.”. (2) Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted...Signs for space storing CO2 and CO2 protected space are incorrectly marked. 46 CFR 185.612(g)

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Marine inspector observed life float light inoperable. 46 CFR 180.175(d)

Special attention should be paid to: Magnetic variation and deviation errors of the compass. Vessel was not provided a magnetic compass deviation table. 46 CFR 185.304(a)(8)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Drill log does not include description of drill scenario or training topics. 46 CFR 185.520(f)

Each vessel must meet the applicable requirements in Subchapter S of this chapter. Each port light installed below the bulkhead deck must...be constructed so that it can be secured watertight. MI observed numerous port lights with seized latches, additioanlly portlight in starboard galley prep area was missing a latch. 46 CFR 177.13-1 (Old T)46 CFR 171.116(f)(3)

Attention shall be paid to insure that... protective devices are provided in all hazardous places. MI observed broken guard on port generator. 46 CFR 176.25-10(b) (Old T)

Wiring joints and splices shall be mechanically secure and made in junction boxes. MI observed junction box in forward storage space overhead without a cover. 46 CFR 183.05-50(b)

Paint and flammable liquid lockers must be constructed of steel or equivalent. MI observed flammable liquids stowed in forward storage space. 46 CFR 177.405(d)

Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. Starboard pilot house window glass is cracked. 46 CFR 177.1020

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Piping for raw water system had substantial leak due hot water heater breaking pipe. Repair pipe and secure heater to the satisfaction of the marine inspector. 46 CFR 182.100

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Hydraulic leak found on steering system. Repair system to the satisfaction of the marine inspector. 46 CFR 176.814

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in for the service in which they are to be employed. Excess fuel found in the engine room, due to possible fuel leak. Clean fuel, determine source of leak, and repair to the satisfaction of the marine inspector. 46 CFR 176.840

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. During the inspection of the pump room area, there was built up debris in the space preventing marine inspector from inspecting the space. Clean bilge of excess debris. 46 CFR 176.830(b)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Raw water leak found on generator. Repair to the satisfaction of the marine inspector. 46 CFR 182.100

Batteries shall be located as high above the bilge as practicable and secured against shifting with motion of the boat. Battery was not secured and was moving while vessel is underway. Secure the battery. 46 CFR 183.05-20(b) “OLD T”

Except as otherwise provided in this section, machinery and fuel tank spaces shall be separated from accommodation spaces by watertight and/or vapor tight bulkheads of double diagonal wood, marine plywood, steel plate, or equivalent construction. Door to engine room had gasket pieces missing and was allowing air flow to accommodation spaces. Repair to the satisfaction of the marine inspector. 46 CFR 182.35-1(a) “OLD T”

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. No date was listed on hydrostatic release. Provide date of install and properly mark or replace hydrostatic release unit. 46 CFR 185.740(b)

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Operator did not have fire servicing report. Provide report to the marine inspector. 46 CFR 176.810(b)(1)(iv)

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.”.(2) Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. (3) Spaces into which carbon dioxide might migrate—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. DISCHARGE INTO NEARBY SPACE CAN COLLECT HERE. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED VACATE IMMEDIATELY.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. No signs were posted. Post signs to the satisfaction of the marine inspector. 46 CFR 185.612(g)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems. Due to a birds nest in one of the Life Floats, the Marine Inspectors were unable to successfully inspect the life floats. Clear out nest and make available for Marine Inspectors to inspect. 46 CFR 176.808(a)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Starboard shaft seal is leaking excessive water during operation. Repair to the satisfaction of the Marine Inspector. 46 CFR 182.100

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Acepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB on the vessel was not installed to float free, the hydrostatic release was expired, and the battery was expired. Vessel is restricted to operating no more than three nautical miles from shore until cleared. 46CFR180.64

All machinery installed and maintained in such a manner as to afford adequate protection against machinery failure. Starboard main diesel engine wet exhaust supply hose leaking at a steady drip. Replace supply hose to satisfaction of attending Marine Inspector. 46CFR182.01-5 'Old T'

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Starbord 22 person life float did no have a floating water light. Attach CG approved floating water light IAW 46CFR180.175(f)(4). 46CFR180.175(d)

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. The electrical cable for the electric Jabsco water pump located in the engine room is chafing on the fuel shut off valve cables. Reroute electrical cable to eliminate chafing. 46CFR183.220(a)

Each window must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. The port pilothouse window glass is cracked. Replace glass. 46CFR177.1020

At each initial and subsequent inspection for certification, the marine inspector shall examine all accessible parts of the exterior and interior of the hull. The vessel's through hull valves showed significant corrosion that could not be inspected. Clean and make available for reinspection. 46 CFR 176.25-5 (a)(1) (OLD T)

On passenger decks of vessels engaged in a ferry or excursion type operations, rails shall be at least 42 inches high. The space below the rail shall be fitted with bulwarks, chain link fencing, wire mesh, or the equivalent. The vessel did not have the appropriate courses for the vessel's railings. 46 CFR 177.35-1(c) (OLD T)

VESSEL'S SWITCHBOARD SHALL FOLLOW THE SWITCHBOARD REGULATIONS FOUND IN (OLD) 46 CFR 111.30-1 (C). SWITCHBOARDS SHALL HAVE NONCONDUCTING HANDRAILS AT THE FRONT. EACH SWITCHBOARD SHALL HAVE A DRIP-COVER OVER THE TOP. NONCONDUCTING MATS OR GRATINGS SHALL BE PROVIDED ON DECK IN THE FRONT OF THE SWITCHBOARD. DISCOVERED THE VESSEL'S SWITCHBOARD DID NOT HAVE NONCONDUCTING MATS, HANDRAILS AND A DRIP-COVER OVER THE TOP OF THE UNIT. 46 CFR 111.30-1 (c) (OLD)

PIPING, INCLUDING VALVES, PIPE FITTINGS AND FLANGES, CONVEYING VAPOR, GAS, OR LIQUID, THE TEMPERATURE OF WHICH EXCEEDS 65.5 °C (150 °F), MUST BE SUITABLY INSULATED WHERE NECESSARY TO PREVENT INJURIES. DISCOVERED MISSING PORTIONS OF INSULATION OF THE VESSEL'S ENGINE EXHAUST PIPING. 46 CFR 177.970

EACH LIFE JACKET MUST BE MARKED WITH TYPE I RETROREFLECTIVE MATERIAL APPROVED IN 46 CFR 164.018. AS PER IMO RESOLUTION A.658(16), IN THE CASE OF A REVERSIBLE LIFE JACKET, THE ARRANGEMENT SHOULD BE COMPLIED WITH NO MATTER WHICH WAY THE LIFE JACKET IS PUT ON. DISCOVERED THAT ONE OF THE SIDES OF EACH 05 LIFE JACKETS WERE MISSING THE APPROVED RETROREFLECTIVE MATERIAL.

At least one ring life buoy must be fitted with a lifeline. Each ring life buoy must be resistant to deterioration from ultraviolet light. MI found ring life buoy lifeline to be severely deteriorated from UV light.

Electrical equipment exposed to the weather must be watertight. MI found hanging on the canopy lights on the upper deck not watertight receptacle.

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The vessel’s port and starboard shaft packing glands are leaking at an excessive rate. The vessel’s port shaft packing gland is leaking at a rate of 3-5 drops per second, and the starboard shaft packing gland is leaking at a steady rate of one cup per minute. Repair shaft packing glands to the satisfaction of the attending Marine Inspector. 46 CFR 182.05-1 “Old T”

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The starboard main propulsion engine is leaking at the raw water pump housing at a rate of half cup per minute. Repair raw water pump housing to the satisfaction o the attending Marine Inspector. 46 CFR 182.05-1 “Old T”

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. 06 life jackets were not ready for immediate use. 02 life jackets required new retro-reflective tape and 04 had holes and need to be replaced. Vessel is restricted to the carriage of 74 passengers until the PFD’s are replaced to the satisfaction of the MI. 46 CFR 176.806 (b)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock or personnel. Vessel has several dead-end wires in the forward accommodation space and in the engine room. Remove dead end wires IAW 46 CFR 183. 46 CFR 183.01-5 “Old T”

Each connection to a conductor must be made within an enclosure. The vessel was using outlets to energize the permanently installed lighting fixtures. Rewire the lighting fixtures IAW 46 CFR 183.340.

46 CFR 182.530 - A visual and audible alarm must be provided at the operating station to indicate a high water level in a machinery space. Visual and audible alarm associated with E/R were both barely perceptable.

46 CFR 185.728 - Each EPIRB must be tested monthly. Owner could not provide proof of testing.

46 CFR 183.350 - Connections must be made to battery terminals with permanent type connectors. Several batteries were connected via wingnuts.

Tighten and Seal all deisel fuel overfill and fill pipes to ensure that overflow of liquid or vapor cannot escape to the inside of the vessel.

Provide proof that all employed deckhands are enrolled in company DAPI program.

Remove portable cables throughout the vessel: bar, wheelhouse.
