Electrical equipment on a vessel must be installed and maintained to:(a) Provide services necessary for safety under normal and emergency conditions;(b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock;(c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. Circuit leading from stbd generator to a ventilation closure solenoid, shorted and back-fed current to ground at stbd generator. Engine was isolated due to rubber gaskets between engine mounts preventing safe ground. it is believed that the current found the easiest path through the engines fuel line causing the fuel line to fail. Provide full analysis of this deficiency and your repair proposal. Ensure all materials used are as per manufacturer/regulatory requirement. Deficiency must be cleared in the presence of a USCG Marine Inspector.

The pull solenoid that releases the hatch/ventilation to the E/R was drawing too much current. The ground wire from the solenoid is connected to the battery that the starboard generator uses. O/O added a large ground wire after it was determined there was insufficient grounding on that specific engine.

A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Strainers must be leak free. Fuel filter is leaking. Replace/repair fuel filter to manufacturer specifications and to ensure leak free.

If a vision waiver is granted, a limitation will be placed on his or her MMC indicating the mariner may not serve under the authority of the endorsement unless corrective lenses are worn and spare lenses are carried onboard a vessel. Master was unaware of spare lenses being required. Master must abide by waiver on medical certificate and provide proof of spare lenses.

A vessel on a Great Lakes route must carry—(1) Six hand red flare distress signals approved in accordance with §160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and (2) Six hand orange smoke distress signals approved in accordance with §160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Additional hand red flares may replace hand orange flares. Missing correct number of flares. Provide correct number/type of flares.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. First aid kit contents expired/missing. Replace required contents of first aid kit with unexpired/missing items.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Starboard engine exhaust is leaking. Repair leaking exhaust as per regulations and manufacturer specifications.

Galley was found to have grease over the griddle (fire hazard) and needed thorough cleaning behind equipment and on the deck. Clean to the satisfaction of attending USCG Marine Inspector.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Decorative rope ligting on 01 deck, E/R bilge alarm wire and E/R hatch solinoid wire not well supported. Support wires to the satisfaction of attending USCG Marine Inspector.

Each floating waterlight must be approved in accordance with §161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Waterlight battery was not changed out as per manufacturer and was expired. Change waterlight battery and mark with date of install.

the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. Stability letter posted was not most recent. Post most current stability letter.

The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Emergency instructions posted did not follow the actual actions of the ship's crew in case of a Man Overboard (MOB). MOB instruction made note of crew member entering water to assist MOB which is unsat. Make posted emergency instructions as to the actual emergency practices of the vessel and drill procedures of the crew.

Door to ventilation in E/R was not closing when testing fixed FM-200 system.

COI was not posted such that all pages are visible

Adequate and up-to-date charts were not available. Operator stated and provided electronic charts but could not provide proof of the charts being "official electronic charts" as per NVIC 01-16.

Persons used as kitchen staff were being used in safety sensitive positions (firefighting) and were not enrolled in a random chemical drug testing program. see MSA 06-16

Fire pump intake hose from sea chest was rubbing on metallic knife edge.

Window was broken on bridge

Provide required contents of first aid kit.

Provide ring life buoy with serviceable grabline and life line.

Provide wiring/cable supports and protection within engine room.

Provide FCC Safety Certificate for VHF radio installation.

Provide Substance Abuse Professional Contact Information

Prove proper operation of bilge high water alarms, both audible and visual to correctly identify each individual space.

Complete DAPI requirements; Maintain records for each employee for entire employment period. Records must be maintained in a secure location.

Provide proof of each Master holding a Radio Operators Permit.

Post Stability Letter in Pilot House.

Replace battery floating water light (Replace annually or by expiration marked on battery). Vessel is limited to day time operation until corrected.

Provide on board up to date Rules of the Road (Ed D). Extended req 30 days.

Replace rotted / broken paddles at next haul out. (Identified 2 with a small amount of rot/broken/missing sections).

Properly ground Bilge alarms and Fire Hatch releasing mechanism. (Bilge alarm and fire hatch mechanism are grounded to hull).

Prove proper operation of engine room fire hatch. (Hatch releasing mechanism inoperative).

Remove combustibles within main galley refer. (Aft refer requires house keeping with excessive amount of grease and comestibles on deck below unit).

Properly wire water heater in galley. (Exposed wires at connections).

Replace portable bilge pump hose. (Cracked).

Remove electrical extension cords used throughout vessel.

Make proper electrical connections for the starboard battery barrel switch. (Exposed wires).

Replace missing fasteners for the starboard generator top access cover.

Provide cap for MSD discharge pipe.

Replace fuel hose and copper line for the starboard MDE. (Leaking)

Replace return line from injector to fuel pump. (Leaking)

Replace fuel supply line to port MDE. (Chaffed where line passes through longitudinal).

Properly label bilge discharge line. (Use and direction of operation).

Repair leak in starboard MDE and Generator exhaust lines between fiberglass elbow and flexible hose connection. Requirement extended as haul out required to correct deficiency.

Correctly label the emergency fuel shutoff valve.

Provide the proper amount of flares IAW 46 CFR 180.68

Provide loading marks as per the stability letter IAW 46 CFR 178.230

Provide hydro testing for fixed fire fighting container IAW 46 CFR 181.410

Provide proper operation for the lube oil gages at the operating station.

Renew ring buoy’s life line IAW 46 CFR 180.70 (c).

Provide training for supervisors for the DAP IAW 46 CFR 16.401.

Provide operating instruction for bilge system IAW 46 CFR 176.800.

Provide proposal for repairs or modification to the rudder. IAW 46 CFR 176.700.

Crop and renew damaged area of starboard side shell and bottom plating as necessary to complete insert of a 18"x30" minimum. Insert must be installed a minimum of 6" forward of bulkhead from lazeratte and the number 3 void.

Grind and weld keelson and plate within the number 3 void aft starboard.

Lazeratte portside; weld pipe penetration for waste line.

Submit electrical one line drawing for suggested repair to vessels steering circuit. Vessel shall not operate as a commercial passenger vessel until approved and completed to the satisfaction of the attending CGMI.

Vessel’s route is here by restricted to the following, as well both bow thrusters must be operational prior to 2008 operating season. Vessel shall not operate as a commercial passenger vessel with sustained winds of 20 knots or more.

Place (1) A-II or B-II Class fire extinguisher in galley space, and (2) Class A-II's in accomodation spaces (1 A-II per each accomodation space).

Vessel is limited to daytime operations until adequate water light on ring life buoy is provided.

Prior to operating next year, make modifications to bow ramp or provide alternative means to recover helpless person form the water.

Properly segregate child-size lifejacket from adult-size lifejacket stowed on the lower deck.

Reinstall covers on speaker system junction boxes.

Provide Portable Bilge pump of adaquate length to pump deepest bilge pocket.

Install check valves as required in fixed bilge pump system.

Install plug or cap or remove valve on fuel supply line at tank.

Weld pipe flange IWO aft bar sink leading to void.

Install chaffing gear around both sides of hydraulic lines for propulsion under the protection covers for the paddle wheel.

Finish welding vent pipe to hull connection for the #5 compartment vent pipe.

Renew the line on the bow ramp for hoisting and lowering.

Replace the waterlight battery.
