If, during an internal structural exam (ISE), damage or deterioration to the hull plating, structural members, or cargo tanks is discovered, the Officer in Charge, Marine Inspection may require the vessel to be dry docked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. During the ISE, multiple areas of damage were discovered, including damaged structural supports and a leaking weld on the #2 cargo tank, port side in the #3 void. Barge shall discharge all cargo at the John W. Stone facility Intracoastal City, LA and proceed to the FMT shipyard in Harvey, LA, in accordance with the approved transit plan, to effect permanent repairs prior to carrying cargo. Vessel rep shall contact local OCMI upon arrival and prior to commencing repairs. 46CFR31.10-21(c)

Transfer hose, coming off loading arm, has kinks and needs to be replaced. Replace to the satisfaction of the attending USCG Marine Inspector prior to loading cargo.

Upper radiator hose on prime mover has a kink and needs to be replaced.

Replace fuel hose on port side due to outer layer wasted.

Several brackets throughout the barge need to be reattached. (Fuel line, fuel oil, hydraulic oil on loading arm) to the satisfaction of the attending USCG Marine Inspector.

Personnel engine platform is wasted and needs to be replaced or removed. STBD side.

Gauge on main cargo header needs to be replaced.

Replace hoses on fuel tank and port prime mover with new SAE J1942 hoses.

All three PV Valves inadequately labeled. Each valve shall be legibly marked with the style or other designation of the manufacturer, the size, pressure and vacuum settings.

Non Intrinsically safe batteries / electrical equipment located within 10 feet of discharge manifold/valve.

Observed multiple insets to the side shell plate in way of starboard #3 wing void. Insets appeared to have caused damage to internal frames: (side longs, diagonal stanchions, side transverse truss & intermediate floor frames). Internal framing was repaired during ISE. Side shell and intermediate floor frames to be cropped out and renewed during next dry dock in 2017.

Remove cargo hose obstructing the operation of OPA 90 rising stick gauge at the #2 cargo tank and ensure proper operation of that rising stick gauge.

INSTALL OVERFILL DEVICES IAW 33 CFR 155.480. VESSEL IS TO CARRY NO OIL AS DEFINED IN 33 CFR 151.05 UNTIL PROPER INSTALLATION OF OVERFILL DEVICES IS COMPLETE.
