Found portable bilge pump suction hose improperly connected.

Found minor fuel leak on return line valve.

Found packing on raw water valve leaking.

Main and aux. machinery were found oil covered. Engineroom bilges had excessive amount of oil accumulated creating a fire hazard. Must be cleaned and maintained.

Pilothouse port MDE RPM gauge is inoperative, must be replaced.

Main and aux machinery must be monitored for excessive oil leaks and maintained in good, clean and operable condition. Leaks should be minimized and any further degradation must be immediately reported to the Sector NGL vessel inspections office.

Multiple maindeck windows cracked, broken, missing. Make safe for pax, repair/replace.

Make collision blkhd bilge line closure valve function.

All bilges require cleaning due to excessive debris, welding materials, trash, etc.

Maindeck cleanliness unacceptable for safe passenger loading and carriage. Must be cleaned.

Pilothouse port MDE tachometer inop, requires repair/replacement.

Port MDE has oil leak. Determine source & effect permanent repairs. Demonstrate proper operation.

Genset fuel line chaffed, requires replacement.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel reported to [name withheld], of RPM fluctuations on the main diesel engines. Vessel secured the port generator which brought both main diesel engines offline. Vessel was unable to restart main diesel engines and required tow assist from nearby vessel to Mackinaw City, [name withheld] indicated that the vessel ran out of fuel during last transit into Mackinaw City. Vessel refueled and tested all machinery. Master intends to revise company procedures to ensure all vessel captains top off fuel tanks prior to end of shifts.

Maindeck forward portion of the vessel requires freeing ports; during the 2022 hull exam & subsequent repairs freeing ports were not calculated or installed. Submit calculations sheet and freeing port installation plan for approval.

Stbd side genset exhaust is missing lagging creating a hazard to personnel and potential fire hazard. Must be properly lagged. Stbd side MDE exhaust muffler sheet metal is deformed creating a snag hazard, must be corrected/repaired.

Shaft alley bilge alarm is not functioning; repair/replace & prove proper operation. ** Compartment must be checked regularly until corrections have been made.

2 windows cracked on maindeck. Must be replaced.

Multiple wiring issues sighted in the pilothouse. Remove deadend wiring. Properly terminate.

Port side genset has excessive oil leak and unapproved flexible fuel line installed. Port genset must be taken out of service and not used until both items are corrected.

Both MDEs and stbd genset have oil leaks. Determine the source of each leak and make repairs. **Leaks must be monitored dialy and any degradation must be immediately repoarted to the cognizant OCMI.

Steering cable improperly routed, must be corrected.

Old fuel fill piping must be removed and fill port properly capped.

Engineroom bilge has oil & debris, must be properly cleaned to avoid fire hazard, bilge plumbing clog hazard and pollution potential hazard. Insulation installation must be completed.

Sighted 3 broken windows, must be made safe for passengers & replaced. ** Extended due to vendor availability/scheduling.

Minimum flares not on board. Flare container not on board.

Show proof of original COD on board or apply for replacement.

Clean up wiring under helm; remove or properly deadend any wiring no longer in use.

Engineroom overhead lagging deteriorated, must be re-installed or replaced for sufficent fire/heat boundary.

Internally inspect all life jackets and replace retro tape on those that need it. During next 2 transits, ensure box of new life jackets is on board during voyage. 46 CFR 122.604

Current condition indicates only line 2 regardless of which generator is the on line generator. Line 1 is never identified as operational or loaded.

Stencil and mark life jacket storage and quantity as required by 46 CFR 185.30-10.
