Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light. Operator to repair 01 non-functioning life jacket lights to the satisfaction of the Marine Inspector. Maximum passenger count to be reduced by one (01) until repaired. 46 CFR 180.75(a)

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. The flame screens or flame arresters must be of such size and design as to prevent reduction in the net cross-sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements. Vessel’s port fuel tank missing flame screen. Operator to replace to the satisfaction of the marine inspector. 46 CFR 182.450 (e)

Vessel did not have a log as required by 46 CFR 185.420 - crew training, 46 CFR 185.520 – abandon ship and man overboard drills, 46 CFR 185.524 – firefighting drills, 46 CFR 185.702 – maintenance of survival craft, 46 CFR 185.728 – EPIRB testing. Operator to provide evidence of existing log, or start new log, to the satisfaction of the marine inspector. 46 CFR 185.420

An adult life jacket must be provided for each person carried on board a vessel. In addition, a number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. Vessel is missing one adult and two child life-jackets according to count on COI. Vessel is limited to 29 adult and 01 child passengers until replaced to the satisfaction of the marine inspector. 46 CFR 180.71

An interconnected detection and alarm system must be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) to be protected by the interconnected detection and alarm system. Vessel does not have an interconnected alarm system installed. Vessel’s COI is for an Ocean route, however, vessel is now restricted to a Limited Coastwise Route until interconnected detection and alarm system is installed to the satisfaction of the marine inspector. 46 CFR 181.450

Each officer employed on any vessel subject to this subchapter must have his or her license or merchant mariner credential onboard and available for examination at all times when the vessel is operating. A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Vessel operator has expired medical certificate. Vessel to provide documentation of valid/unexpired medical certificate, to the satisfaction of the Marine Inspector, prior to the carriage of passengers. 46 CFR 185.402 46 CFR 15.401 (c)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Vessel has deck support beam in forepeak area no longer attached. Vessel to repair to satisfaction of the marine inspector. Vessel to repair or replace in accordance with approved plan prior to credit dry dock. 46 CFR 177.100

A vessel of at least 7.9 meters (26 feet) in length must be provided with individual bilge lines and bilge suctions for each watertight compartment. Vessel has broken bilge line connection. Vessel to repair to satisfaction of the marine inspector. 46 CFR 182.510(a)

For the purpose of flexibility only, flexible hose installed in lengths of not more than 30 inches and subject to pressures of not more than 5 psi, may meet the following requirements: Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. Vessel has non-metallic pipe not double clamped. Vessle to install two clamps, at both ends of the hose to the satisfaction of the marine inspector. 46 CFR 182.72 (e)(3)(v)(B)

A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Fuel filter and strainer bowls must be highly resistant to shattering due to mechanical impact and resistant to failure due to thermal shock. Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Vessel using fuel filter with no bowl attached. Vessel to install metal bowl on fuel filter assembly to satisfaction of the marine inspector. 46 CFR 182.455(b)(6)

A pre-engineered fixed gas fire extinguishing system must: Be approved by the Commandant; Be installed in accordance with the manufacturer's instructions. Vessel has temporarily unused ducting in engine room creating ventilation opening into space. Vessel to seal opening to satisfaction of the marine inspector. 46 CFR 141.420(a)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel to repair forward mast head light and starboard running light to satisfaction of the marine inspector prior to operations at night. 46 CFR 183.420

Each life jacket carried on a vessel on oceans or coastwise route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter. Vessel has 06 life jackets with nonworking lights. Vessel's max passenger count to be reduced by 06 persons from that listed on COI until repair completed to satisfaction of the marine inspector. 46 CFR 180.75(a)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel has expired first-aid kit contents (ammonia inhalant, aspirin, burn gel, eye wash). Vessel to replace with unexpired items to satisfaction of the marine inspector. 46 CFR 184.710

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03–2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel to replace expired alcohol testing devices to satisfaction of the marine inspector. 46 CFR 185.212 46 CFR 4.06-15(a)(1)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Vessel has forward end of main engine, with exposed rotating belts and pulleys, not protected by a guard. Vessel to install guard or rail to satisfaction of the marine inspector. 46 CFR 177.960

Each small battery installation must be located in a well ventilated space and protected from falling objects. Vessel has engine room batteries not protected. Vessel to install covers for batteries to satisfaction of the marine inspector. 46 CFR 186.354

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel does not have installed detectors in required spaces by 27Dec2022. Vessel has been limited to a Limited Coastwise route until detector installation has been verified by a Marine Inspector. 46 CFR 181.405(c)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. The disposable hydrostatic release that allows the EPIRB to float free, is expired. Install a new un-expired hydrostatic release that allows float free operation of the EPIRB.

Vessels operated beyond the 20 nautical miles, but not more than 100 nautical miles from the nearest land, must be equipped with a MF-DSC frequency transmitter meeting the requirements of §80.1101(c)(3) and capable of transmitting J3E emission and a receiver capable of reception of J3E emission within the band 1710 to 2850 kHz,or a functioning satellite phone. The DSC function was not operable on the vessels MF Radio.Provide MF radio installation with functioning DSC or a functioning satellite phone.

The following spaces must be equipped with a fixed gas fire extinguishing system, in compliance with §181.410, or other fixed fire extinguishing system specifically approved by the Commandant, except as otherwise allowed by paragraph (b) of this section: (1) A space containing propulsion machinery; (2) A space containing an internal combustion engine of more than 37.3 kW (50 hp) The generator space containing the ships service diesel generator was not equipped with any fixed gas fire extinguishing system. Operation of the ships service diesel generator is prohibited until an approved fixed gas fire extinguishing system is installed, or proof that the Diesel engine is not more than 50 horse power.

The master of the vessel shall ensure that: Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable the required EPIRB tests were not being logged. Provide record and maintain onboard the vessel of EPIRB tests.

The master of the vessel shall ensure that the EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. The vessels EPIRB battery is expired. Replace vessels EPIRB battery or install a new EPIRB with an unexpired battery.

Each survival craft must be: Stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. The hydrostatic release that allows the liferaft to automatically float free was expired. Install new unexpired hydrostatic release.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry, Six hand red flare distress signals approved in accordance with §160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and, Six hand orange smoke distress signals approved in accordance with §160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. There were only three orange smoke distress signals onboard the vessel. Three additional unexpired orange smoke distress signals shall be placed in service onboard the vessel.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. The first aid kit contained several items that were expired. Provide new first aid kit, or replace expired items with new non expired items.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The FCC Safety Radio Certificate was expired. A new unexpired FCC Safety Radio Certificate must be obtained and placed aboard the vessel.

Bus bars located in the forward accommodation space must be covered to protect passengers and crew from electrical shock.

All broken windows in or around passenger accessible spaces need to be replaced.

Documentation of inspection/servicing of the fixed fire-fighting system is not available at the time of inspection.
