Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. MI identified minor leaking fuel line at bulk head penetration connection located in the aft bulkhead of fuel tank space. MI identified minor leaking aft fuel tank isolation valve. 46 CFR 182.100

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. MI identified electric cable runs located in the following spaces requiring metal supports: port/stbd generator space and port/stbd fuel tank space. 46 CFR 183.340(b)(4)

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. MI identified loose generator control box in generator room. 46 CFR 183.200(a)

Each vessel must have a detailed inspection of the radio installation by an FCC-licensed technician once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Vessel does not have certificate available for inspection. 47 CFR 80.903

Replace the wet and degraded insulation hanging from the overhead bulkhead in the compartment below the accommodation space.

Treat the wood brackets/supports with dry rot for the port holding tank in the compartment below the accomdation space.

Replace wire nuts for the light fixture in the accommodation space.

The master of the vessel shall ensure that: The EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. EPIRP battery found to be expired. Vessel's route is restricted to within 3 nm. 46 CFR 185.728(b)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. FCC Radiotelephony Certficate is expired. 47 CFR 80.903

An adult life jacket must be provided for each person carried on board a vessel. 02 life jackets failed inspection and require replacement.

the painter for life boats must be stowed in such a way that it runs out freely when the life float or buoyant apparatus floats away from a sinking vessel. The painter attached to the inflatable liferaft was not attached to the cradle or deck.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First aid kit expired March 2022.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. The vessel did not possess alcohol testing kits.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guard and protective devices must be in satisfactory condition. Dead end wires are present throughout the vessel.

Vapors barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. Starboard side, aft engine room as wiring/cabling penetrating the bulkhead to the aft void with no vapor barrier present.

Vessel did not have adequate means of blocking ventilation into the engine room in the event of an engine room fire.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel has not received had a hull exam since 28SEP2017. Vessel must receive credit drydock before being issued a COI. 46 CFR 176.600(c)(1), 705c - Prior to being issued a COI.

An inflatable buoyant apparatus must be serviced at a facility no later than the month and year on its servicing sticker. The IBA was 3 months overdue for maintenance. Please provide proof of IBA being serviced by an approved facility. 46 CFR 185.730(a)(1), 705c - Prior to being issued a COI.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Hydrostatic release unit on IBA was expired. Provide a new hydrostatic release unit for the IBA. 46 CFR 185.740(b), 705c - Prior to being issued a COI.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. EPIRB’s hydrostatic release unit was expired. Provide a new EPIRB hydrostatic release unit. 46 CFR 185.740(b), 705c - Prior to being issued a COI.

To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. Vessel could not produce EPIRB NOAA registration. Provide proof that EPIRB is registered with NOAA. 47 CFR 80.1061(f), 705c - Prior to being issued a COI.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Ventilation for the engine room had no means of closure. Provide ventilation closures for all supply/exhaust ducts for the engine room, ensuring they are readily available and mounted in the vicinity of each opening. 46 CFR 182.465(h), 705c - Prior to being issued a COI.

A vessel not required to have a power driven fire pump by §181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel did not have fire buckets onboard. Provide 3 fire buckets IAW regulations. 46 CFR 181.610, 705c - Prior to being issued a COI.

The emergency lighting required by paragraph (a) of this section must automatically actuate upon failure of the main lighting system. Current emergency lighting in crew accommodation below deck must be individually turned on by hand. Provide adequate emergency lighting that are automatically actuated upon loss of normal power. 46 CFR 183.432(b), 705c - Prior to being issued a COI.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. STBD E/R batteries displayed significant wastage on the terminals. Replace STBD E/R batteries. 46 CFR 183.200(b), 705c - Prior to being issued a COI.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Master did not have onboard a marine radio operator permit. Provide proof of master’s marine radio operator permit. 47 CFR 80.159(e)(1), 705c - Prior to being issued a COI.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. There were multiple types of life jackets on the vessel, but only one donning placard. Provide and mount a donning placard in a conspicuous location for each type of life jacket carried onboard. 46 CFR 185.516(a), 705c - Prior to being issued a COI.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Vessel did not have an emergency instruction placard at the operating station. Provide an emergency instruction placard mounted at the operating station. 46 CFR 185.510(a), 705c - Prior to being issued a COI.

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. The anchor control valve mount was ziptied to the vessel. Provide permanent connection of the anchor control valve to the deck. 46 CFR 184.300, 705c - Prior to being issued a COI.

The owner, or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by §185.510 of this part and, when applicable, the duties listed in the station bill required by §185.514 of this part. Vessel did not have a logbook available for documenting drills or training of crew, and the operator was unaware of this requirement. Provide a means of logging training and drills required by the emergency instruction placard. 46 CFR 185.420 / 185.520 / 185.525, 705c - Prior to being issued a COI.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. STBD engine raw water intake piping showed significant corrosion in the section of piping between the overboard valve and the strainer, resulting in a small but consistent flow of saltwater into the bilge. Repair or replace stbd engine raw water piping. 46 CFR 182.100, 16c

Vessel is required to complete a full DAPI Audit prior to receiving a COI. 16 CFR 16.113 / 49 CFR 40, 705c - Prior to being issued a COI.

Fuel shutoff valves for aft tank found to be inoperable. Shutoff valve at the tank must be operable outside the compartment in which the valve is located. Prove proper operation IAW cite.

Visual bilge alarm for spaces found to be inoperable. A visual and audible alarm must be provided at the operating station to indicate a high water level.

Vessel found to have no means for alcohol testing on board. Marine employer must have a sufficient number of alcohol testing devices readily accessible. Provide a means to test for alcohol IAW cite.

Provide adequate means to serve recover a helpless person from the water (man overboard).

Found 05 adult life jackets and 01 child life jackets to be unserviceable. Owner destroyed in the presence of the MI. Provide sufficient number of serviceable life jackets as listed on the COI.

Found vessel is missing EPIRB monthly testing log and Crew training log. Provide satisfactory log book showing monthly EPIRB tests to MI.

Conduct a satisfactory inspection for renewal/issue of the Certificate of Inspection.

1) Prior to carriage of passengers, contact USCG Sector Los Angeles - Long Beach to request an amended COI as per 46 CFR 176.120(b)

PRIOR TO CARRYING PASSENGERS FOR HIRE, PROVIDE AN APPROVED VALID HYDROSTATIC RELEASE FOR THE EPIRB.

SHOW PROOF OF PURCHASE OF BATTERIES FOR WATERLIGHTS ON PFD'S, AND_x001C_ REPLACE SAME IN ACCORDANCE WITH 46 CFR 180.25-20. VESSEL WILL NOT CARRY PASSENGERS FOR HIRE UNTIL REQUIREMENT IS MET.
