No repairs or alterations affecting the safety of the vessel with regard to the hull shall be made without the knowledge of the OCMI. The number 3(s) void, has a leak that appears to be located on the cargo deck above. Standing water approximately 2-4 inches was observed in the void and heavy condensation was observed. A drip rate of approximately 90-100 drops per minute was observed. The vessel shall submit repair proposal and complete repairs IAW applicable standards. Repair after completion of next voyage.

Forward starboard rake has a sharp inset on each side of the stiffener on frame number 4. The vessel shall submit repair proposal and complete repairs IAW applicable standards. Repair required at next dry dock February 2028.

Each U.S. oil tanker of 150 gross tons and above and each other U.S. ship of 400 gross tons and above: that engages in voyages to ports or off-shore terminals under the jurisdiction of other parties to MARPOL 73/78 must have on board a valid International Oil Pollution Prevention (IOPP) Certificate. The vessel did not have a valid IOPP certificate on board at the time of the inspection. Place a valid IOPP certificate on board the vessel to the satisfaction of the attending Marine Inspector prior to embarking on an international voyage.

An International Air Pollution Prevention Certificate(IAPP) shall be issued after an initial or renewal survey in accordance with the provisions of regulation 5 of this annex, to: any ship of 400 gross tonnage and above engaged in voyages to ports or offshore terminals under the jurisdiction of other parties. No documentation was available at the time of the inspection attesting to the issuance of a valid IAPP certificate to the vessel. Provide a valid IAPP certificate to the satisfaction of the attending Marine Inspector prior to embarking on an international voyage. MARPOL VI/6.1.1

Vessel's COI expired 16FEB2023. Vessel to complete satisfactory COI renewal.

Transverse frames 7 and 8 are buckled at port bilge keel. Longitudinal frame inboard of port bilge keel is fractured at the weld to frame 8. Frame 8 is fractured at the bilge keel Outboard vertical and diagonal stanchions at frame 8 are buckled. 2 strakes between frames 8 and bulkhead 9 are fractured. Inboard diagonal stanchion between frame 8 and bulkhead 9 is buckled Submit repair proposal and complete repairs IAW applicable standards prior to carriage of cargo.

ALLAPOOYA FAILED TO COMPLETE SUCCESSFUL DRYDOCK AND INTERNAL STRUCTURAL EXAMS IN FEBRUARY, AS REQUIRED AND NOTED ON HER COI. ACCORDINGLY, VESSEL COI IS INVALID AND VESSEL MUST COMPLETE A SUCCESSFUL COI INSPECTION, DRYDOCK INSPECTION, AND INTERNAL STRUCTURAL EXAM.

Original Certificate of Inspection was not onboard and not found in office files. Find or request new COI prior to sailing.

Renew stbd forward rake aft bulkhead stiffening frames 3 to 5, 6' in length under ladder.

Replace stbd aft rake bulkhead stiffeners 4 to 7, 4' length under manhole cover.

Removed Loadline marking on port side and repaint as per load line certificate.

SUBMIT COPY OF VESSEL RESPONSE PLAN COVER LETTER TO OCMI PUGET SOUND NO _x001C_ LATER THAN 18FEB93 IAW NVIC 8-92, NO OIL MAY BE HANDLED STORED OR TRANSPORTED AFTER THIS DATE IF NOT IN COMPLIANCE WITH THIS REQUIREMENT AFTER THAT DATE.

PROVIDE COPY OF SHOP TEST REPORT FOR CARGO RELIEF VALVES.

PROVE SAT OPERATION OF PUMP ENGINES AND GENERATOR ENGINE CONTROLS AND SHUTDOWNS.

TEST CARGO PUMP RELIEF VALVES NEXT CARGO DISCHARGE AND PROVIDE SATISFACTORYEVIDENCE OF THE SAME TO THE SATISFACTION OF THE OCMI. THIS REQUIREMENT IS TO BE COMPLETED NO LATER THAN 13 JULY 1989.

REMOVE TEMP REPAIRS AND MAKE PERMANENT REPAIRS TO FRACTURED BOTTOM PLATING IN WAY OF #1 PORT CARGO TANK 1' AFT OF FWD BLKHD AND 15' INBOARD OF SIDE- SHELL. FRACTURE IS APPX 8" LONG.

HYDRO CARGO DISCHARGE PIPING AND HOSES PRIOR TO NEXT LOADING/DISCHARGE OF CARGO.

TEST CARGO RELIEF VALVE PRIOR TO NEXT LOADING/DISCHARGE OF CARGO.

REPLACE ALUMINUM HATCH COVERS ON GRADE "A" CARGO TANKS 1, 2, 3 AND 4 PORT AND STARBOARD WITH STEEL COVERS IN ACCORDANCE WITH 46 CFR 56.60-20 AND 32.57.
