Fire pump is not operational. Enact permanent repairs to the satisfaction of USCG Marine Inspector. CODE 705: Vessel is restricted to carrying no more than 49 passengers.

46 CFR 183.420 All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. AMI observed that masthead light was working intermittently. Enact permanent repairs to the satisfaction of USCG Marine Inspector. CODE 705: Vessel restricted to daytime operations and visibility greater that 2.0 nautical miles.

46 CFR 182.530 (b) On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. MI observed that visual indicator was working intermittently in in STBD tank space, STBD head space, and port forward space, Enact permanent repairs to satisfaction of USCG Marine Inspector.

46 CFR 177.960 An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Machinery guard was not installed on STBD engine. Install guard.

46 CFR 185.510 (a) The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Vessel did not have emergency instructions.

46 CFR 183.200 (b) Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including electrical shock. MI observed battery fuse in STBD engine room missing cover. Install cover.

46 CFR 181.520 Portable fire extinguishers location must be to the satisfaction of the Officer in Charge, Marine Inspection. 10 B.C extinguisher bracket in pilothouse was broken. Replace bracket.

46 CFR 183.200 Electrical equipment on a vessel must be installed and maintained to Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Owner told MI that generator is going to be replaced in kind due to a failed head gasket. CODE 705: Operational test of new generator will need to be witnessed by USCG Marine Inspector before it can be operated with passengers on board vessel.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Inspector observed the battery for the waterlight on the ring buoy was expired by nearly 5 years. Restricted operations to daytime and visibility >01nm. 46 CFR 180.70(c) (705c)

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Inspector observed batteries for lights for all seven life floats were expired. Restricted operations to daytime and visibility >01nm. 46 CFR 180.175(d) (705c)

The intent of this part is to insure that all machinery in such a manner as to afford adequate protection against fire, explosion, machinery failure and personal injury. Inspector observed missing insulation/lagging from the port side main engine turbo. Repair to satisfaction of CG inspectors within 03 days. 46CFR182.01-5(c) (705c)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. Inspector observed the bilge high level alarm in the port side engine room did not give a visual alarm at operating station. 46CFR182.530(a) (16c)

The name of a vessel must be marked or painted in clearly legible letters and numbers On each life float and buoyant apparatus. Inspector observed 02 life floats lettering was illegible. 46CFR185.604 (16c)

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. Inspector observed 03 life jackets needing replacement of retroreflective tape. 46CFR185.604(h) (16c)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to crew and passengers. Inspector observed multiple types of lifejackets but only had placard for one type of lifejackets. 46CFR185.516 (16c)

A vessel must comply with the applicable requirements for any radio including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Inspector observed vessel had installed INMARSAT telephone, but did not have an updated station license and installation certificate for the new equipment. 46CFR184.502

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Inspector observed vessel had no procedures in place for conducting and recording monthly inspections and test of portable fire extinguishers. 46CFR176.80(b)(1)(ii) (16c)

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. MI observed that generator didn't shut down when fire extinguishing system was tested. Vessel master identified that the two signal wires were hooked up backwards. Test sat. 46CFR181.420(a)(4) (10c)

Stbd engine room, main engine, raw water cooling, through hull ball valve is not closing all the way. Make repairs or replace valve and ensure it closes all the way.

Except as otherwise provided in this section, vessels in ocean service shall carry sufficient life floats for all persons on board. 46 CFR 180.10-5(a) MI observed deteriorated hand holds on (01) life float. Vessel passenger total is reduced by 12 until deficiency is rectified.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. 46 CFR 182.530(a) MI observed the vessel's port and starboard bilge high water level alarms at the operator's station incorrectly displayed corresponding alarms.

A vessel not required to have a power driven fire pump by 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words "FIRE BUCKET" must be stenciled in a contrasting color on each bucket. 46 CFR 181.610 MI observed vessel only had one, unlabeled fire bucket for emergency use. Vessel must add two more fire buckets with labeling and drilled 1" holes on each side.

As appropriate for the intended voyage, all vessels must carry adequate and up-to-date- Light Lists. 46 CFR 185.20-35 MI observed vessel's Light List was dated 2021, not 2024.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. 46 CFR 177.960 MI observed the starboard diesel engine did not have a machinery guard in place.

A vessel must comply with the applicable requirements for any radio installation IAW 47 CFR part 80. INMARSAT was found to be inoperable. Vessel operations has been limited to 20 nautical miles from land until repairs have been made. 46 CFR 184.502

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel shall install system to the satisfaction of attending USCG Marine Inspector. 46 CFR 181.405(c)

Flammable items not otherwise covered by the regulations of this subchapter, such as rechargeable batteries, including lithium ion batteries utilized for commercial purposes, must be handled, stored, and operated in a way that mitigates the risk of hazardous conditions. Vessel shall develop plan to the satisfaction of attending USCG Marine Inspector. 46 CFR 185.364

MI was notified that following a lightning strike, the vessel lost propulsion due to both main engine and generator failure. 46 CFR 182.100

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines. Vessel's fuel shutoff valve reach rod, for the port fuel tank, broke during operation for inspection. Repair fuel shutoff valve reach rod. 46 CFR 182.455(a)(4)

Each ring life buoy with floating water light must have a lanyard of at least 3 feet. Replace frayed lanyard for ring life buoy. 46 CFR 180.70(d)(2)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts and navigational publications. Obtain current paper charts and required publications for vessel. 46 CFR 184.420(a)

A dual voltage generator installed on a vessel shall be of the grounded type, where: The neutral of a dual voltage system must be solidly connected at the switchboard's neutral bus and t he neutral bus shall be connected to ground. The neutral of a dual voltage system must be accessible for checking the insulation resistance of the generator to ground before the generator is connected to the bus. Ground detection must be provided that: for an alternating current system, meets § 111.05-27 in subchapter J of this chapter. The dual voltage generator onboard with a manufacturer date of 2012 was installed without Coast Guard oversight and plan review. The neutral to ground is not installed at the switchboard and is missing ground fault detection. Submit electrical plans to the MSC for approval. Per 46 CFR 183.330(j), switchboards and distribution panels must be sized in accordance with Section 7.10 of IEEE 45-1998 or IEC 60092-302 (clause 7) as required by 46 CFR 111.30-19(a). 46 CFR 183.324 46 CFR 183.376

A grounded alternating current system must be grounded to the hull of a metallic vessel or, on a nonmetallic vessel the neutral must be connected to a common ground. The operator removed the common ground from the starboard hull. The neutral for the dual voltage generator is not connected to a common ground. The common ground is missing in the starboard hull. 46 CFR 183.376(d)

Per 46 CFR 183.376(a), there must only be a single neutral connection to ground in a grounded distribution system, regardless of the number of power sources. When the power source is onboard the vessel, this ground connection must be at the switchboard. When connected to shore power the vessel neutral to ground bond must be severed because the shore power neutral to ground to neutral connection is made at the service entrance panel on shore. If this neutral to ground bond is not broken, the neutral to ground bond onboard the vessel will create a parallel circuit and neutral current will flow on both the shore power neutral and the shore power safety ground. This presents an electric shock hazard, posing a threat to passengers and crew, and also may create stray currents that endanger any swimmers or divers in close proximity to the SPV. The current shore power connection to the vessel was found with a dead end grounding conductor presenting a hazard to the vessel and personnel. The electrical installation onboard does not provide the ability for the permitted shore power connection to be installed in a manner to comply with 46 CFR 183.376(a). Submit electrical plans to MSC that illustrate the proper shore power installation in order to prevent an improper neutral to ground bond onboard the vessel. Contact USCG MI for verification of installation once plans are approved. 46 CFR 183.376(a)

Metallic enclosures and frames of electrical equipment must be permanently bonded to the common ground on a non metallic vessel. The common ground was removed in the starboard hull and all electrical metallic enclosures were found without a bond to the common ground. Size grounding conductors IAW 46 CFR 183.372(c). 46 CFR 183.372(a)

Except as otherwise provided in this section, rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Port forward railing found fractured at the port bow. 46 CFR 177.35-1

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Overcurrent protection for the conductor between the DC panel and the inverters is missing. 46 CFR 183.380

Inspectors identified diesel leak on the fuel return line after racor fuel filter housing for the port main diesel engine. Vessel operator repaired the pipe prior to getting underway removing pipe and resealing. Operational test SAT. 46 CFR 182.01-5 "Old T"

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. The float-free link required by 180.130(a)(1) must be of proper strength for the size of the life float as indicated on its identification tag. Inspectors were unable to determine the breaking strength of 04 float-free links due to missing identification tags. The links were also stretched and twisted. The vessel operator replaced all links except the one on the 8 person life float. Passenger count is reduced to 67 until float-free link is replaced. 46 CFR 180.137 (b)(2)

A life float must be approved under series 160.027. The color of the buoyant apparatus must be primarily vivid reddish orange. The number of persons capacity must be marked or painted in clearly legible letters and numbers on each life float. The 8 person life float was severely faded and the person capacity was not visible. Passenger count is reduced to 67 until life float is repaired. 46 CFR 180.200(a)(2) 46 CFR 160.010-4(n) 46 CFR 185.604(e)

A number of child size life jackets equal to at least 10% of the number persons permitted on board must be provided. 01 child life jacket was unserviceable. Child passenger count reduced to 07 until life jacket is replaced. 46 CFR 180.71(b)

Each flare must be stowed in a portable container marked as required by 185.614 of this chapter. Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: ‘‘DISTRESS SIGNALS’’. The portable container was severely corroded and the bottom was rusted out. 46 CFR 180.68 (e)(1) 46 CFR 185.614

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Coast Guard Light List. The Light List onboard was from 2019. Replace with up-to-date version. 46 CFR 184.420 (a)(3)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. The items contained in first-aid kit shall be as listed in Table 160.041-4(b). Aspirin supplied in the first-aid kit was expired. Operator provide new non-expired aspirin. 46 CFR 184.710 46 CFR 160.041-4(b)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel's hydrostatic release expired on January, 2019. Vessel owner replaced on the spot. - 46CFR180.64

Each life float must be secured to the vessel by a painter with a float-free link permanently attached to the vessel; certified to meet § 160.073 in subchapter Q of this chapter or of proper strength for the size of the life float or buoyant apparatus as indicated on its identification tag. Float-free link on starboard bottom life float was wasted and float-free link on port top life float was larger gauge and incorrectly sized for 12pax life float. Vessel owner replaced on the spot. - 46CFR180.137(b)

Windows and other openings at the operating station must be of sufficient size and properly located to provide an adequate view for safe navigation in all operating conditions. Aft-most port side bridge safety glass is cracked. - 46CFR177.1030(a)

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. MI observed 02 life float water lights extinguished. Replace. 46 CFR 180.175 (d)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. MI observed tear/hole on inside grab line of life float. Repair. 46 CFR 176.808 (b)

The number of persons capacity and name of a vessel must be marked or painted in clearly legible letters and numbers on each life float. Ensure all (07) life floats are marked as required. 46 CFR 185.604

A vessel must comply with the applicable requirements for any radio installation as set forth in 47 CFR part 80. Vessel's VHF radio was missing GPS input. Repair. 46 CFR 184.502

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. MI observed electrical wiring passing through holes centerline bulkhead in engine room compartment. Seal penetrations. 46 CFR 176.800 (a)
