Provisions shall be made for closing all supply duct cowl openings for spaces protected by a fixed gas fire extinguishing system. All closure devices must be readily available. Vessel has protection rails installed in front of all supply duct openings which were observed bent inward, thus preventing the closure devices from being able to be installed properly. Vessel shall make necessary repairs to ensure proper installation can be achieved prior to departure. 46 CFR 182.465(h)

Before getting underway on a voyage or as soon as practical thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of of emergency requirements listed in 46 CFR 185.506. Vessel has not been conducting passenger safety orientations. Vessel shall develop a passenger safety orientation and provide to the Marine Inspector the method of ensuring orientation is completed as noted above. 46 CFR 185.506

Rails shall be provided on the periphery of the aft deck of at least 36" with coarses no less than 12" spaced between. Rails along the aft deck are missing/not in place during inspection. Vessel reports rails are not on board and not used. Vessel shall install adequate railing along aft deck and have installed when aft deck is not in use during transfer of cargo. 46 CFR 177.35-1

The owner or master shall instruct each crew member at least once every three months as to the duties the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard. Crew training shall be logged or otherwise documented. The training entry shall include the date of the training and the general description of the training topics. The vessel could not provide logs or evidence of training covering all emergency instruction topics, specifically rough weather at sea/flooding. Vessel shall incorporate this aspect of training as part of vessels training procedures and ensure log entries are made. 46 CFR 185.420(a) & (c)

Vessel experienced turbo charger MDE fire on port inboard engine. Fire burned port outboard engine also. Moored EPS Dock Venice, LA. Provide Masters statement, tech report attesting to repiar and function of both affected MDE. Doc. that all onboard equip. has been tested and operates sat. Make vessel available for CG inspection once repairs conclude.

Vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored. Vessel is missing an adequate anchor to anchor line connection device such as a shackle.

Vessel must be arranged and equipped to allow the crew to recover a helpless person from the water. The rescue platform needs adequate gear such as chains, shackles and/or quick snapping clips to securely attach to the vessel.

FWD accommodation space (passenger seating area) emergency escape hatch must be capable of being opened by one person for rapid means of escape as required per 46 CFR 177.500(h). Escape hatch must be lubricated/adjusted accordingly.

Engine room escape flush-deck hatch must be attached to the frame or coaming by captive chains, or other devices of substantial strength to prevent its loss, which are missing and need to be installed.
