All vessels shall be provided with bilge pumps in accordance with table 182.25-10(a) Inspector witnessed bilge pump would not take suction from the port forward void. 46 CFR 182.25-10(a) Action 60C

An interconnected detection and alarm system must be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system. MI's arrived and found one detector in the pilothouse missing and a new one ready for replacement but not connected. 46 CFR 181.450(a)(2) Action 701c

Each vessel certificated to operate on an oceans route in warm water must either:(1) Be provided with inflatable buoyant apparatus of an aggregate capacity that will accommodate at least 67% of the total number of persons permitted on board; or(2) Be provided with life floats of an aggregate capacity that will accommodate at least 100% of the total number of persons permitted on board. MI's observed one hydrostatic release on the 6 man liferaft and one hydrostatic release on the starboard inflatable bouyant apparatus expired. The 6 person liferaft was not properly secured to the cradle. The starboard 15 person buoyant apparatus was not serviceable due to wasted straps and a cracked floating light. Bring all lifesaving equipment into compliance with requirements of Subchapter T. Vessel is restricted to carriage of no more than 67 passengers until deficiency is rectified. 46 CFR 180.202(b) Action 701c

Suitable covers, guards or rails shall be installed in way of all exposed and hazardous places such as gears, machinery etc. MI observed missing guard on the generator pulleys. Install guards to the satisfaction of USCG MI. 46 CFR 177.35-15(a) 16c

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel could not produce a complete, unexpired, serviceable medical kit meeting requirements in 46 CFR 199.175(b)(10). Present to MI a medical kit containing all contents required to USCG MI. 46 CFR 184.710 Action 701c

Applicants seeking a national endorsement must either hold an unexpired medical certificate or submit a medical certificate application. Vessel master could not produce a medical certificate. Master provide proof of valid medical certificate prior to serving as Master. 46 CFR 10.227(d)(6) 50c

Materials used in piping system must meet the requirements of this subpart and otherwise be acceptable to the cognizant [name withheld] observed raw water leaking from the raw water discharge to the exhaust pipe in the port machinery space. Submit proposal to affect permanent repairs to the pipe to the satisfaction of USCG MI.

Each battery must be secured to protect against shifting with the roll and pitch of the vessel and free from water splash or spray. MI observed a battery in the port engine room sitting on the deck plates. Properly secure in approved container or remove battery from vessel.

Each life jacket must be approved under approval series in subparts 160.002, 160.005, 160.055, 160.115, 160.176, or 160.255 in part 160 of this chapter, or other standard specified by the Commandant. MI's observed approximately 21 life jackets not meeting approval due to either being unserviceable or not having adequate lights. Vessel is restricted to the carriage of 90 persons for this purpose until adequate, approved life jackets are provided for all 111 authorized.

Each portable fire extinguisher on a vessel must be of an approved type. MI observed the stbd engine room fire extinguisher missing the cone on the end of the hose. Replace extinguisher with approved type 40BC.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. MI observed no separation of the adult and child life jackets on the main deck. Separate Adult from child life jackets.

Equipment mounted on a hinged door of an enclosure must be constructed or shielded so that a person will not accidentally contact energized parts of the door mounted equipment when the door is open and the circuit energized. MI observed open front ends on the stbd engine room distribution panel. Ensure the front of the panel is shielded so no personnel can contact engergized equipment.

The strength of the float-free link under paragraph (b)(2) of this section and the strength of the painter under § 180.175(f)(3)(ii) must be determined by the combined capacity of the group of life floats and buoyant apparatus. MI observed the float free links on both lifefloats rated for a breaking strength exceeding the weight of the installed lifefloats. Vessel corrected on the spot.

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Maintenance logs for lifesaving appliances were not available for review by the attending marine inspector. Provide proof of lifesaving maintenance documentation to the satisfaction of the attending marine inspector. CITE: 46 CFR 185.702(d)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB registration is expired. Provide proof of valid registration to the satisfaction of OCMI. 46 CFR 180.64

Each buoyant apparatus must have Type II retro reflective material meeting subpart 164.018 of this chapter on each side and end. The starboard side life float requires new retro reflective tape. 46 CFR 160.010-4(p)

Each survival craft required on a vessel by this part must meet one of the following: For a life float—Approved under approval series 160.027 or other standard specified by the Commandant. Life float is missing the approval plate and could not be verified to be approved. Life float was replaced by an IBA. CITE: 46 CFR 180.200(a)(2)

Each buoyant apparatus must be fitted with a lifeline, pendants, a painter, and a light. Top Center life float straps securing center webbing are deteriorated and not serviceable. CITE: 46 CFR 180.175(e)

Each life jacket must be marked with Type I retro reflective material approved in 46 CFR 164.018. 06 life jackets require new retro reflective tape. CITE: 46 CFR 185.604(h)

Vessel had groups of life floats tethered to single weak link. Weak link installed was designed to break free with life raft capacity less than capacity of group.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. USCG Marine Inspectors found three (3) out of nine (9) of the life floats onboard had broken and missing straps, deteriorated lines, and missing data plates. Remove and replace the life floats or effect appropriate repairs to the satisfaction of the attending marine inspector.

Each ship transporting more than 6 passengers for hire equipped with a radiotelephone station must carry a radio operator. At time of inspection, vessel master did not have a valid FCC Marine Radio Operator Permit. Provide FCC Marine Radio Operator Permit.

If a garden hose is used, it shall be fitted with a commercial garden hose nozzle of good grade bronze or equivalent metal. Vessel did not have nozzles on either of its firehoses. Provide satisfactory nozzles for fire hoses.

Fire hoses onboard were not of a good commercial grade and lacked nozzles.

Portable hand fire pump does not have suction and discharge hoses of adequate length.
