Each nozzle must be of corrosion-resistant material and be capable of being changed between a solid stream and a spray pattern. A nozzle on a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, must:(1) Be of a type approved in accordance with approval series 162.027; or(2) Be of a type recognized by the Commandant as being equivalent in performance. Fire nozzles on the vessel were made of plastic material. Replace with approved brass or bronze type.

Each survival craft required on a vessel by this part must meet one of the following: (2) For a life float - Approved under approval series 160.027 or other standard specified by the Commandant. Life float was found needing repair to exposed top surface. Replace or repair to manufacturer's specifications.

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Suction for space designated as a ballast tank but operated through the bilge system was found to use a flexible hose without a strainer. Provide a suitable suction fitted with a strainer.

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck, or, if it is readily accessible under service conditions, a screw-down valve without a reach rod may be fitted to the bilge line on the after side of the collision bulkhead. The reach rod was found partially disconnected from the valve handle. Re-attach reach rod.

A hatch cover must: (2) Be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Frame attachment was missing from the forepeak hatch. Provide means to to secure and prevent loss.

Each pressure type wire connector and lug must meet UL 486A (incorporated by reference; see 46 CFR 175.600) or other standard specified by the Commandant. The use of twist-on type wire nuts is permitted under the following conditions: (1) The connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration; and (2) Twist-on type connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. A twist-on connector was found outside of an enclosure on the power cable to the electric submersible pump in the forepeak. Provide an acceptable connection.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] room escape hatch markings were worn off. Re-mark emergency escape.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel identified wastage in way of hydraulic hose penetrations of the forepeak deck, wearing of the center hinge tab for the swim step, a wasted holed area in the bulkhead between the forward bunkroom and the crew bunk room in way of the starbaord bilge, and cracks in the port generator exhaust hull penetration. Repair to the satisfaction of the attending USGC marine inspector.

Where rigid nonmetallic material is permitted for use in piping systems by this subpart, the following restrictions shall be adhered to: (1)Penetrations of required watertight bulkheads by any rigid plastic pipe systems are prohibited, except when: (i)Each such penetration is accomplished using an acceptable metallic fitting, welded or otherwise attached to the bulkhead or deck by an acceptable method; and (ii)An acceptable metallic shutoff valve is installed adjacent to the through deck or through bulkhead fitting. The valve shall be operable from above the bulkhead deck. If two valves are installed, one on either side of the bulkhead, the valves need not be operable from above the bulkhead deck provided immediate access to both is possible.(3) Through hull fittings and shutoff valves shall be metal. A PVC pipe was found penetrating the required watertight bulkhead between the engine room and the aft bunk room on the starboard side. Provide metallic bulkhead penetration and shutoff valves.

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. A multiple cable penetration of the required watertight bulkhead between the engine room and the aft bunk room was observed on the port side. Make penetration water tight.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. The fuel shutoff valve for the port fuel tank in the lazarette was not easily operable. Repair and demonstrate proper operation of fuel shutoff valve.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: (h) Operational test of the means provided for pumping bilges. Vessel was unable to test the means of pumping bilges. Demonstrate means of pumping all watertight compartments.

Protection from mechanical damage must be specifically considered and all protective covering or shields must be installed to the satisfaction of the cognizant [name withheld] hoses were found not properly supported in way of the fore-peak, crew bunk room and forward passenger bunk rooms. Properly support hoses and provide protection from chafing/damage.

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant OCMI. The outlet from the port fuel tank was found leaking in way of the nipple between the tank the shut off valve. Repair leak.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. The forward crew berthing and pilot house were found to have ungrounded electrical outlets.

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free. Life-floats were inhibited from a float free condition as they were used to store dive gear, mooring fenders, garbage and the painter line was entangled around the water-light mounting bracket.

Provide operable fire pump pressure gauge.

Provide portable fire extinguisher for the forward crew berthing of at least 2-A size.

Fire extinguisher bracket in engine room broken. Provide extinguisher mounted on approved bracket.

Properly support anchor winch clutch wire.

Emergency escape from forward crew berthing missing adequate ladder or other means to aid escape and missing latch to hold hatch open. - CG029

One child sized life jacket found with closed cell foam degraded.

Provide bridge to bridge safety certificate.

Provide current alcohol testing devices on board.

Power carrying conductors in engine room not adequately supported. Provide metallic wire hangers at least every 24".

Fire extinguisher in forward bunk room not immediately accessible. Move extinguisher or remove items impeding it.

Engine room emergency escape hatch not able to properly dog closed.

The required EPIRB tests were not being logged. Provide record and maintain onboard the vessel of EPIRB tests.

No record of monthly maintenance being performed on portable fire extinguishers. Provide a log that contains a record of all monthly maintenance being performed on portable fire extinguishers.

The life jacket donning instruction placards did not include all types of life jackets aboard the vessel. Provide life jacket donning instruction placards that include all types of life jackets kept aboard the vessel.

The 120 VAC electrical outlets in the galley were wired without the ground wire connected. Ensure outlets are wired with ground connected.

The 120 VAC electrical outlet boxes in the galley area were loose. Properly secure The 120 VAC electrical outlet boxes in the galley area.

Combustible material was located within 3’ of a cooktop that operated over 250 degrees Fahrenheit. Insulate or remove all combustible material within 3' of the cooktop.

There was no emergency lighting installed in the forward part of the below deck passenger accommodation area. Provide emergency lighting installed in the forward part of the below deck passenger accommodation area.

16 AWG wire was used to wire power and lighting circuit in crew bunk room. Replace wire with 14 AWG minimum.

The wire penetration for the watertight door alarm was not arranged to be watertight. Provide means to make the wire penetration watertight.

The 120 VAC electrical outlet GFCI in the bunk room was not functioning as designed. Repair or replace The 120 VAC electrical outlet GFCI in the bunk room.

The electrical wiring in the engine room was not supported as required. Provide metallic wire supports at least every 24" on all wire runs.

Non-approved extension cords were installed as a permanent wiring arrangements in engine room and Lazerette. Remove extension cords, or replace with approved UL marine wiring.

An ammeter was not installed in the battery charger circuit. Provide an ammeter in the battery charger circuit.

The existing mattresses used in the passenger sleeping bunks were not flame retardant. Provide flame retardant treatment to the mattresses or provide new mattress's that meet the flame retardant requirements of new subchapter "T".

The watertight door located in the mid-ships water tight bulkhead, was held open with a permanently installed hold back device. Remove the holdback device and ensure watertight door remains closed at all times except when persons are transiting through the doorway.

The required spanner wrench was missing from the two fire hose stations. Provide spanner wrenches at each fire hose stations.

Make aft E/R bulkhead penetrations watertight.

Replace gauge on the fire pump.

Remove dead end wires in E/R or place in junction box.

Make proper splice on wires in E/R and fwd peak.

Install blanks on electrical panel in E/R and passenger accommodations.

Install cover on circuit board in head.

Provide service report for portable fire extinguishers.

Update nautical charts, coast pilots, light lists, and tide tables.

Renew retro tape on 07 life jackets.

Repair leak on stbd fire main.

Repair mast head light prior to operating at night.
