The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of of the relevant rules and regulations. In meeting these requirements the company should ensure the inspections are held at appropriate intervals, any non-conformity is reported, with its possible cause, if known, appropriate corrective action is taken; and records of these activities are maintained. Vessel is enrolled in ABS Planned Maintenance Program (PMP). A review of randomly select items from the annual vibration and oil analysis reports, identified that several critical components required immediate attention (Bow Thruster Lube Oil Pump, Air Operated Oily Waste Transfer Pump, #1 Main Propulsion Generator). Review of the vessels SAMMS maintenance program revealed no corrective actions had been taken for these items. Conduct external audit on ABS planned maintenance program. 74 SOLAS (24)IX/3.1, ISM Reg. 10.1 and 10.2

Vessel reported a scheduled AFFF agent lab test cameback as unstable. Vessel used back-up agent to replace the primary that was unstable. Vessel is required to have backup AFFF agent in accordance with their approved fire plan. 46CFR193.05-10(b)

A sufficient number of lifejackets shall be carried for persons on watch. Lifejackets shall be so placed as to be readily accessible and their position shall be plainly indicated. Where, due to the particular arrangements of the ship, the lifejackets provided may become inaccessible and alternative provisions shall be made to the satisfaction of the Administration which may include an increase in the number of lifejackets to be carried. Engine Control Room does not have adequate number of life jackets and immersion suits for watch standers when personal survival appliances may be inaccessible. Fire control plan and SOLAS placards to be updated to include personal survival appliances to be stowed in the ECR for both watchstanders. 74 SOLAS (1997) III/7.1.1.2 and 74 SOLAS (1997) III/7.2.2

The port and stbd engine room supply dampeners and natural supply dampeners do not fully close when activated.

The port and stbd life boat's aft quarter is damaged by the davit's inboard aft resting blocks. Make repairs in accordance with manufacture's approved procedures and provide approval letter.

The port and starboard life boat's davit has excessive metal wastage on the davit's harbor locking pins receptacle padeyes and accepting flat bar, cable pulley mounts, & davit arm heads.

The port life boat embarkation station flood lamp mount has broken off.

Every ship shall be provided with a main and auxiliary steering system to the satisfaction of the administration. Vessel's thruster system is inoperable and satisfactory operation was unable to be witnessed. 74 SOLAS(14) II-1/29.1

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire resisting divisions, and protection of openings and penetrations in these divisions. Machinery space fire dampers failed to fully close upon activation of remote ventilation shutdown. 74 SOLAS(14) II-2/14.2.1.1.1

Where penetrations of watertight bulkheads and internal decks are necessary for access, piping, ventilation, electrical cables etc. arrangements are to be made to maintain watertight integrity. Power cable for STBD underwater hull shall be routed through the fore and aft watertight bulkheads of the space with type approved penetrations. 74 SOLAS(04) II-1/25-9.1

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. 480 volt junction shall be replaced with appropriate type/size and dead end cable shall be removed or properly terminated. 74 SOLAS(04) II-1/40.1.3

Every person designated to take charge of medical care onboard vessels subject to the STCW Convention must hold an MMD or MMC endorsed as person in charge of medical care. Vessel has onboard ships 'Medic/Nurse' who does not currently hold endorsement for person in charge of medical care. 46CFR15.404(i).

In addition to complying with the specific provisions for fire integrity of bulkheads and decks of cargo ships, the minimum fire integrity of bulkheads and decks shall be as prescribed in tables 9.5 and 9.6. Opening in two class A galley pass through bulkheads are protected with steel roller shutters and dedicated fusible links. Fubislbe links are concealed within metal overhead and shielding them from heat/proper operation. Install links so they are exposed to properly operate in the event of a fire. 74 SOLAS(14) II-2/9.2.3.3.1

Ballast/SW Cooling pipe located in overhead in aft pump room found with temporary repair (JB Weld/rubber patch/C-clamped) Make permanent repairs to the damaged section of piping as designed.

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. Ventilation closure on port side 01 deck at FR76 found to be inoperable as handle moves but internal mechanics do not operate. Make permanent repairs as required.

46CFR199.70(a)(3)/199.45(b)(3): Stbd ring buoy lanyard with dry rot. Replace with new to as original. Ring buoy, smoke float, and buoyant light must be properly installed in a quick release buoy bracket to allow all three to easily deploy.

46CFR199.70(b)(2)(i): USCG observed crewmember on bridge having difficulty removing PFDs that are jammed inside stowage box attached to bulkhead. Install larger stowage box for bridge watch stander PFDs or provide alternative location on bridge to stow PFDs to be readily available.

Oily Water Seperator is inoperable as designed. When operating underway vessel must retain waste oil/slops onboard. In-port vessel will discharge to approved shore side facility or another vessel until repairs are completed. Make permanent repairs IAW manufacturers specifications.

Approximately 20% of onboard life-jackets have holes/tearing and are past service life. Replace holed/deteriorating jackets. The remaining 80% are nearing service life expiration. Recommend implementing replacement plan over next few years to replace remaining deteriorating life-jackets.
