Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Life float grab lines are deteriorated. 46 CFR 176.808 (b)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible;(2) U.S. Coast Pilot or similar publication;(3) Coast Guard Light List;(4) Tide tables; and(5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. Vessel did not have up to date coast pilot or light list. 46 CFR 184.420(a)

Electrical equipment on a vessel must be installed and maintained to:(a) Provide services necessary for safety under normal and emergency conditions;(b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock;(c) Minimize accidental personnel contact with energized parts; and(d) Prevent electrical ignition of flammable vapors. Vessel had loose / missing grounding wires and several dead end wires in engine compartment. 46 CFR 183.200

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel has a loose nut on port main engine mount (inboard aft). 46 CFR 182.100

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel's life jacket placard does not match life jackets on board. 46 CFR 185.516(a)

All fuel tanks must be electrically bonded to a common ground. Vessel's fuel tank was not grounded. 46 CFR 182.440(10)(b)(4)

Fuel lines must be protected from mechanical injury and effectively secured against excessive movement and vibration. The return fuel line for the generator was not secured. 46 CFR 182.455(b)(3)

If the life float or buoyant apparatus does not have a painter attachment fitting, a means for attaching the painter must be provided by a wire or line that:(1) Encircles the body of the device. The painter line for life float was not attached around the body of the float. 46 CFR 180.137(d)(1)

A flexible hose may be used instead of fixed pipe for the discharge line of a submersible electric bilge pump. The bilge discharge hose in F/O tank compartment was dry rotted. 46 CFR 182.520(f)

46 CFR 176.500(b)(1): Vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel did not complete last annual inspection. Annual inspection is required prior to carrying passengers.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. A through hull fitting in steering compartment was replaced without an approved plan given to OCMI for approval. Haul out required to verify what was installed. 46CFR176.700(a)

Replace plastic zip ties used to secure cords and wires with metal clamps in main hold compartment. Cable and wiring must be installed with metal supports spaced not more than 24 in apart. 46 CFR 183.340(b)(4)

Port exhaust is missing a metal clamp. Nonmettalic hose may be used for wet exhaust systems provided it is ... double clamped where practicable. 46 CFR 182.430(e)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette. Forward and Aft hatch spaces visual alarms were inoperable. 49 CFR 182.530(a)(1)

In systems and applications where flexible hoses are permitted to be clamped: (1) Double hose clamping is required where practicable. Fuel fill tube connection on tank only has one clamp. 46 CFR 182.410(d)(1)

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. The flame screens or flame arresters must be of such size and design as to prevent reduction in the net cross sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements. Fuel vent is painted over. 46 CFR 182.450(e)

Lifeline and pendants. The lifeline and pendants must be as furnished by the manufacturer with the approved life float or buoyant apparatus. Replacement lifelines and pendants must meet the requirements in Subpart 160.010 of this chapter. Light. The light must be a floating waterlight approved under approval series 161.010 or other standard specified by the Commandant. The floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. The name of the vessel must be marked or painted in clearly legible letters on each life float paddle. Lifefloat missing pendants, lifeline, and vessel name on the paddles. Float light line was attached to the sea painter and not the body of the lifefloat. 46 CFR 185.604(g) 46 CFR 180.175(f)(1) 46 CFR 180.175(f)(4)

For vessels other than those covered in paragraphs (b) and (c) of this section, the name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel was not marked with the vessel name or hailing port on either bow or the stern. 46 CFR 67.123(a)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. 46 CFR 181.610 Vessel missing 01 fire bucket.

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Starboard steering gear mount is highly corroded. 46 CFR 176.814

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. Ring life buoy missing retro, and life float has DOT retro. 46 CFR 185.604(i) 46 CFR 160-010-4(p)

A hatch cover must: (1) Have securing devices; and (2) Be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Aft hatch missing attachment to coaming. 46 CFR 179.360(b)(2)

The color of the buoyant apparatus must be vivid reddish orange as defined by FED-STD-595C, sec 13 and 14. Life float is painted wrong color. 46 CFR 160.010-4(p)

Each battery must be secured to protect against shifting and free from exposure to water. Battery box was filled with water. 46CFR183.350(b)

Electrical equipment must be permanently bonded together to a common ground by a normally non-current carrying conductor. Ground wire from the Port engine starter was not attached. 46CFR183.372(a)

Valves for removing water or impurities from diesel fuel in strainers are permitted. Valves must be provided with caps or plugs to prevent fuel leakage. Two of the three racor filter drains fitted with petcocks valves did not have caps. 46CFR182.455(b)(9)

Replace/Renew retro-reflective “SOLAS”-type Tape on life float.

Replace/Renew deteriorated “grab lines” on life ring.
