When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency... the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage." MSD overboard discharge was not locked. Lock overboard discharge.

Decks between various spaces must meet the requirements of table 116.415 (c). Vessel is missing areas of structural fire protection in the overhead of the engine room. Install structural fire protection to as-built standards. 46 CFR 116.415 (c)

The marine inspector shall examine all accessible parts of the vessel’s machinery and equipment to be assured that they are in satisfactory condition. MI’s found the generator break-in interlock missing. Replace. 46 CFR 176.10-5 (b) (OLD T)

The marine inspector shall examine all accessible parts of the vessel’s hull to be assured that it is in satisfactory condition. MI’s found holes near the shore tie connection leading to the engine room, in the crawl space beneath the bridge, and in the overhead of the Bridge. Patch. 46 CFR 176.10-5 (b) (OLD T)

All hand portable fire extinguishers shall be checked as noted in Table 176.25-25(a)(2). MI’s found one fire extinguisher unserviced. Provide a checked extinguisher.

The marine inspector shall examine all accessible parts of the vessel’s machinery and equipment to be assured that they are in satisfactory condition. MIs observed multiple deck plates in the engine room were loose and unsecured. Secure all loose deck plates. 46 CFR 176.10-5 (b) (OLD T)

Examine and test by operation all watertight closures in the bulkheads. MI’s found the engine room watertight door inoperable due to misadjusted dogs. Restore to operation and perform a chalk test. 46 CFR 176.25-5 (2) (OLD T)

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. MIs observed wires connecting batteries to the battery charger were in the bilge wells of the engine room. Ensure all wires are kept above the bilge and secured properly. 46 CFR 183.01–15 (a) (OLD T)

Waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. MIs found combustible plastic waste receptacles on the vessel. Provide noncombustible receptacles. 46 CFR 116.405 (i)

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. MIs observed exposed wiring in the crawl space below the Bridge. Secure the exposed wiring and ensure the wire is not dangling. 46 CFR 183.01–15 (a) (OLD T)

… the marine inspector shall examine and test the following items to determine that they are in proper operating condition and fit for the service for which they are intended: Engine Control Mechanisms. Such mechanisms shall be tested by operation and examined, visually... MIs observed the STBD Hydraulic Engine Control system was below the minimum reservoir level and the system was not reaching the minimum pressure of 80 PSI. The crew was able to fill the reservoir and achieved minimum pressure required to operate. Corrected on the spot. 46 CFR 176.25-10(a)(3) (OLD T)

… the marine inspector shall examine and test the following items to determine that they are in proper operating condition and fit for the service for which they are intended: Main Propulsion Machinery. The marine inspector may require that the main propulsion machinery be operated both ahead and astern. MIs observed transmission fluid leak from the stern of the STBD Engine. The crew was able to secure the leak. 46 CFR 176.25-10(a)(3) (OLD T)

Vessel is required to display navigation lights in accordance with Inland & International Navigational Rules, as applicable. Anchor light was found to be inoperable. Repair / replace prior to operations during night-time and restricted visibility. 33 CFR 84 NO NIGHT-TIME OR RESTRICTED VIZ OPS 7/11/24 - Update: Anchor Light was still inop / missing. An all-around (anchor) light had been modified / re-purposed to act as a masthead light by taping the interior to mimic the correct arc of visibility, 225 deg.

VESSEL IS REQUIRED HI-WATER ALARMS IN ALL SPACES SUBJECT TO FLOODING. BILGE / HI-WATER ALARM IN LAZARETTE WAS INOPERABLE, NOT WIRED. REPAIR / REPLACE TO M.I. SATISFACTION PRIOR TO CARRIAGE OF PASSENGERS (PAX). 46 CFR 119.530

Previously conducted passenger count IAW 46 CFR 115.113 using the fixed seating criterion and deck area criterion. While operating on Protected Waters a maximum of 330 passengers may be carried, with a maximum of 148 passengers carried on the upper deck. While operating on Partially Protected Waters, a maximum of 300 passengers may be carried, with a maximum of 110 passengers carried on the upper deck. The updated passenger count will be issued via amended COI. 46 CFR 115.113 REMAIN OPEN UNTIL NEW COI ISSUED - Amended COI routed for signature on 7/8/24.

arine Sanitation Device electrical components, motors, & controls must be installed, operated, & maintained in accordance with manufacturer instructions and good marine practice. Sewage discharge pump motor had exposed wiring, improperly twist spliced outside housing. Motor controller wiring was via improper switch with cracked / exposed housing and/or missing cover plate. Repair / replace wiring and sewage pump motor controller, prior to operation of sewage pump. 33 CFR 159.57 33 CFR 159.97 PRIOR TO USE OF SEWAGE PUMP

Gauges to indicate engine cooling water temp. and lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. Both port-side Main Diesel Engines (MDE) gauges in the pilot-house were inoperable. Repair / replace within 07 days. 46 CFR 182.20-5(b) Old-T WITHIN 07 DAYS. Extension granted, addtnl 07 days from 6/4/24.

A voltmeter, ammeter, means for measuring frequency (AC), control equipment and measuring instruments shall be provided to insure satisfactory operation of each generator(s). Starboard generator power available indicator was inop & frequency (hertz) meter was inconsistent / unreliable. Repair / replace to provide required generator monitoring, within 14 days. 46 CFR 183.10-5 Old-T

For portable fire extinguishers, the owner, managing operator, or a qualified servicing facility shall conduct the inspections & maintenance procedures required by Ch. 4 of NFPA 10 (IBR). The concession stand portable extinguisher had no record of being inspected / maintained in past year. Conduct and provide documentation of required inspection / maintenance within 07 days. 46 CFR 115.810

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Numerous locations, including the area below the pilot-house helm station had extension cords and power splitters connected in series ("daisy-chained" or "piggy-backed"). Remove & correct unsafe electrical practices within 07 days. 46 CFR 120.200

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters: "EMERGENCY EXIT, KEEP CLEAR". Vertical escape hatch from storage / work space forward of the engine room, into the concession area was unmarked & easily obstructed. Provide required markings. 46 CFR 185.30-15 Old-T 7/11/24 - UPDATE: All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”. All vsls shall be provided with not less than two avenues of escape from all general areas where the crew may be normally employed. 1) Required markings were not present at bottom of escape ladder from crew Work-Storage Compt (fwd of E/R). 2) Scuttle from fwd void. was seized and missing interior handle to activate dogs and open. Provide required markings and repair / replace fwd. scuttle to fully operational condition within 14 days. 46 CFR 122.606 46 CFR 177.15-1 (Old-TL)

Each vessel owner or operator must ensure that the approved VSP / ASP and any approved revisions or amendments are carried onboard the vessel. Most recent PVA-ASP amendment was not onboard or available. Provide current, approved ASP onboard within 07 days. 33 CFR 104.120

Vessel (vsl) has received waiver to employ Rescue Platform in lieu of Rescue Boat for Man Overboard (MOB) recovery. Current location of MOB recovery system (approx. amidship stbd side) was insufficient for Master / Operator visibility. Submit proposal & procedures for MOB recovery system on port bow, to OCMI within 14 days. Complete re-arrangement & crew training within 30 days of OCMI approved proposal. 46 CFR 117.210 46 CFR 122.420

For each watertight door which is in a required subdivision bulkhead, an indicator light must be installed in the pilothouse and at each other vessel operating station from which the door is not visible. The indicator must show whether the door is open or closed. Below deck Water-Tight Doors (WTD) between engine room, work-shop, and forward void were not equipped with indicators of door open / close condition. Provide WTD indicators in pilot-house. 46 CFR 170.270(e)

All electrical equipment and circuits must be clearly marked & identified. Various distribution panels / breaker boxes throughout vsl had unlabeled breakers. Accurately re-label electrical circuits & breakers as required. 46 CFR 120.220

Battery chargers must have an ammeter connected in the charging circuit. If the batteries are not adjacent to a distribution panel or switchboard, the battery lead must have a fuse in series, located as close as practicable to the battery. Battery charger was not installed with required ammeter. Both 12V and 24V battery systems were distant from switchboard and did not have installed fuses. Repair / replace as required. 46 CFR 120.350

All cable and wire must be installed with metal supports in such a manner as to avoid chafing and other damage, be protected by metal coverings or other suitable means if in areas subject to mechanical abuse. Wiring in concession stand area was not supported or protected from contact damage or sources of water / moisture in close proximity. Provide required support & protection. 46 CFR 120.340

A vessel must be operated in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Glass and other glazing material used in windows must be of material that will not break into dangerous fragments if fractured. Each window must be capable of withstanding the maximum load from wave and wind conditions expected due to its location and the authorized route of the vessel. Glass or other glazing material used in windows at the operating station must have a light transmission of not less than 70 percent according to Test 2 of American National Standards Institute (ANSI) Z 26.1 and must comply with Test 15 of ANSI Z 26.1 for Class I Optical Deviation. Pilot-house was observed to allow significant rain-water intrusion in close proximity to radios, AIS, and other electrical communication & navigational equipment. Additionally, stbd aft pilot-house window was broken / missing. Properly weather-proof pilot-house and properly replace pilot-house window as required. 46 CFR 122.100 46 CFR 116.1010-1030

At each inspection the owner / operator shall make the steering systems available as necessary to determine that they are in suitable condition and fit for the service intended. Surface rust / corrosion made inspection of the stbd rudder post problematic. Clean stbd rudder post and provide evidence of suitability. 46 CFR 115.814

A vessel must be fitted with ground tackle necessary for the vessel to be safely anchored. The ground tackle provided must be satisfactory for the size of the vessel, the waters on which the vessel operates, subject to the approval of the cognizant [name withheld] anchor did not include any chain rode between anchor and polypropylene line. Provide suitable (length, weight / material, & strength) anchor chain section between anchor & line, for vessel size & construction, per manufacturer recommendations. 46 CFR 121.300

At each inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Bilges and other spaces shall be free of excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Improper storage of spare hydraulic oil containers & significant accumulations of debris was observed in and around bilge areas. Remove and/or secure all loose items creating potential hazards, as required. 46 CFR 115.830 7/11/24 - UPDATE All observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Following conditions are required to be corrected to MI satisfaction within 14 days. 1) access to remote fuel shut-off valves was blocked by supplies & equipment in aft locker. 2) excessive oily water mixture in bilge of Engine Room (E/R) and water crew Work-Storage compt (fwd of E/R) 3) excessive debris / trash (water bottles, rags, etc.) in bilges and areas around and under deck-plates 4) excessive storage of oils, lubricants, and fluids throughout E/R and fwd crew Work-Storage compt 5) excessive oil accumulation on Starboard Generator 46 CFR 115.830 46 CFR 176.25-50 (Old-TL)

VESSEL ENGINE ROOM DOCUMENTED AS REQUIRED TWO (02_ 40-B:C PORTABLE FIRE EXTINGUISHERS. ONLY ONE (01) WAS PRESENT. REPLACE / PROVIDE MISSING USCG APPRVD. 40-B:C FIRE EXT., PRIOR TO PAX. (IAW COI Equipment Listin & MISLE tab) 46 CFR 118.500

PORT DIESEL GENRATOR (SSDG) OBSERVED TO HAVE SIGNIFICANT COOLING WATER LEAK, GAUGES INDICATED HI-TEMP + HI-LUBE OIL PRESS. CONDITION. ENGINE SECURING NOT IAW ENGINE PROCEDURES, LOCAL STOP CONTROL WAS INOP. REPAIR SYSTEMS PRIOR TO PAX.

t time of inspection, both port main engine propeller shaft seals, and inboard stbd engine prop shaft seal, were observed to have steady streams of water leakage / intrusion. Shaft seal packing glands appeared to be nearly fully compressed. Incoming water was mixing with residual oil in bilge and from running machinery, creating potential hazardous conditions. Within 14 days provide written proposal to address excessive shaft seal leakage. Followed by report detailing effectiveness of adjustments, or submit repair proposal for OCMI review & approval, prior to commencing any repairs beyond in-place adjustments. 46 CFR 182.01-5 46 CFR 176.25-50 (Old-TL)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. The sewage / toilet flushing pump was observed to be only secured to deck plate by one (01) sheet-metal screw. Pump motor designed with four (04) or more mounting brackets. Properly secure pump / motor within 14 days. 46 CFR 119.100

Electrical equipment must be protected from wet and corrosive environments. Pilot-house battery charger, batteries, and CCTV equipment (w/ AC outlets), beneath the helm were observed to be in close proximity to multiple unsealed penetrations in the super-structure, potentially allowing water ingress and electrical damage, shorting and potential fire hazard. Properly seal deck-house / super-structure openings to prevent water entry within 14 days. 46 CFR 120.210

Each Ring life buoy on a vessel must:... Be readily accessible. Marine Inspector noticed a lifebuoy bracket without a lifebuoy. 46 CFR 117.70

Electrical equipment on a vessel must be installed and maintained to:... minimize accidental personnel contact with energized parts... Marine Inspector noticed GFCI electrical receptacle cover damaged. 46 CFR 120.200

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits.... Marine Inspector noticed conflicting signage on aft life jacket storage regarding number of life jackets. 46 CFR 122.604

An inflatable liferaft or inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant for the particular brand, and in accordance with servicing procedures no later than the month and year on its servicing sticker. The vessels route will be restricted to rivers until all three liferafts are serviced. 46CFR122.730

Electrical equipment used in a location normally exposed to splashing, water washdown, or other wet conditions must be dripproof. Receptacle on top deck aft port side is not weather-tight. 46CFR120.210

Cooking surfaces that may exceed 250*F must be kept in a galley. Pizza oven must to be removed immediately from concession stand. 114.400

A voltmeter and an ammeter, which can be used for measuring voltage and current of a generator that is in operation, must be provided for a generator rated at 50 volts or more. Both voltmeter and ammeter are inop. 46CFR120.320(c)

At each initial and subsequent inspection for certification, re-inspection and at every other vessel inspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations are corrected. During an in-service inspection attending marine inspectors witnessed a temporary soft patch on the raw sea water intake into port side out board engine in between the sea chest and the shut off valve which is an unauthorized repair and must be returned to its proper construction. Provide a repair proposal for the repair of the soft patch. 46 CFR "OLD T" 176.25-50

Marine Inspector shall require that all observed unsafe practices and hazardous situations be corrected. STBD outboard Main Diesel Engine is missing a guard around PTO pump belt and pulley system. Install machinery guard on MDE. Repair within 14 days. 46CFR176.25-50

WATERTIGHT DOORS SHALL BE WATERTIGHT. ALL OF THE VESSEL'S WATERTIGHT DOORS FAILED A CHALK TEST, FAILING TO PROPERLY SEAT THE GASKET UPON THE KNIFE EDGE. 46CFR170.270

MACHINERY SPACE BULKHEADS/DECKS SHALL BE OF SUITABLE FIRE PROTECTION TO MAINTAIN BOUNDARIES LISTED IN 46CFR72.05. THE ENGINE ROOM VENT BETWEEN THE E/R AND ACCOMODATION SPACE HAD INSUFFICIENT/FAILING INSULATION & THE SUPPORT STRUTS ON BOTH OUTER SHELL BULKHEADS WERE NOT INSULATED. 46CFR177.10-5(a) "OLD T"

THE VESSEL THE MANHATTAN IS A SPV-TL, THEREFORE NOT REQUIRED A SAFETY MANAGEMENT SYSTEM (SMS) PER REGULATION. SECTOR NY HAS FOUND THE FOLLOWING DEFICIENCIES TO BE OBJECTIVE EVIDENCE THAT A SERIOUS SUBSTANDARD CONDITION HAS NOT BEEN PRO-ACTIVELY MANAGED BY THE VESSEL'S OWNER/OPERATOR, LEADING TO A LACK OF SAFETY CULTURE ONBOARD THE VESSEL.

STRUCTURAL AND WATERTIGHT INTEGRITY SHALL BE MAINTAINED ON THE VESSEL AT ALL TIMES. THE STBD QUARTER OF THE VESSEL, BELOW THE MAIN DECK LINE, IN THE LAZARRETTE HAS BEEN DAMAGED & THE WATERTIGHT ENVELOPE COMPROMISED. ADDITIONALLY, THE THIRD WINDOW FROM THE CABIN DOORS ON EACH SIDE IS COMING OUT OF ITS INSTALLATION. PROVIDE REPAIR PROPOSAL.

EACH SPACE ON A VESSEL (LISTED IN THIS PART) SHALL HAVE A HIGH BILGE WATER ALARM THAT HAS AUDIBLE/VISUAL DISPLAY AT THE OPERATING STATION. THE PANEL AT THE OPERATING STATION DID NOT GIVE AUDIBLE ALARMS.

BILGES SHALL BE KEPT FREE OF OILY WATER. THE ENGINE ROOM AND LAZARRETTE BILGES HAD OILY WATER IN THEM.

VESSELS MASTER SHALL ENSURE THAT THE CREW IS SUFFICIENTLY TRAINED IN PERFORMING A MAN OVERBOARD DRILL. VESSEL’S CREW PERFORMED M.O.B. DRILL IN AN UNSAFE MANNER, THAT WAS ALSO NOT IAW THE MASTER’S EXPLANATION OF HOW THE DRILL WAS TO BE PERFORMED.

ALL REPAIRS/MODIFICATIONS SHALL BE SUBMITTED AND APPROVED BY THE [name withheld]. VESSEL CREW INSTALLED SEVERAL LED LIGHTING SYSTEMS ON BOARD AS A REPLACEMENT FOR NON-OPERATIONAL LIGHTS. ELECTRICAL ONE-LINE DRAWING AND EQUIPMENT SPECIFICATION SHEET TO BE SUBMITTED TO THE OCMI.

MACHINERY SHALL BE INSTALLED IAW SUBCHAPTERS F AND J OF THIS CHAPTER. THERE WERE MULTIPLE PUMPS AND EXTENSION CORDS FOR ELECTRICAL EQUIPMENT INSTALLED IN THE FREEZER VOID, LAZARRETTE, AND MAIN CABIN NOT IAW SUB F/J.

RAILS SHALL BE CAPABLE OF WITHSTANDING 200 POUNDS OF FORCE. DECK RAILS IN STBD QUARTER OF THE VESSEL HAVE BEEN DAMAGED, TO INCLUDE CRACKED RAILS AND WELDS. PROVIDE REPAIR PROPOSAL. PASSENGERS NOT PERMITTED IN THE AREA OF DAMAGE UNTIL REPAIRS HAVE BEEN MADE.

ELECTRICAL INSTALLATIONS SHALL BE MADE TO MINIMIZE DANGERS TO PASSENGERS/CREW. THE VESSEL HAVE MUTIPLE CABLE/WIRE RUNS IN THE ENGINE ROOM, LAZ, FREEZER VOID, AND VOID 2 THAT WERE DIPPING DOWN INTO THE BILGE; MOST NOTABLY UNDER THE AFT PORTION OF THE GENERATOR.

A VESSEL MUST COMPLY WITH THE APPLICABLE DESIGN, EQUIPMENT, PERSONNEL, AND PROCEDURES OF PARTS 33CFR151/155/156. VESSEL HAD AN INSTALLED OVERBOARD DISCHARGE FOR THE SINK/GRAYWATER IN THE FREEZER VOID

PROTECTION MUST BE PROVIDED, IAW 46CFR116.970, FOR ALL EXHAUST INSTALLATIONS. THE OUTBOARD STBD MAIN DIESEL ENGINE’S INBOARD EXHAUST PIPE HAD AN APPROXIMATELY 8” SECTION OF UNPROTECTED PIPING.

RING LIFE BUOYS SHALL BE MARKED IAW THIS PART. THE RING LIFE BUOYS ON THE PRT QUARTER AND THE STBD SIDE OF THE WHEELHOUSE WERE NOT LEGIBLY MARKED.

ALL CABLE/WIRE SHALL BE INSTALLED WITH METAL SUPPORTS SPACED NOT MORE THAN 24-INCHES APART. CABLE/WIRING THROUGHOUT THE VESSEL WERE SUPPORTED WITH ZIP TIES, NOT METAL SUPPORT BRACKETS AS REQUIRED; INCLUDING THE CABLES FOR THE ISOLATION TRANSFORMER IN VOID #2.

ALL CONNECTIONS SHALL BE MADE IN ENCLOSURES; ENCLOSURES SHALL BE WATERTIGHT IF IN AN AREA SUBJEST TO WATER EXPOSURE. MULTIPLE CONNECTIONS IN THE BELOW DECK SPACES, AS WELL AS ON THE EXTERIOR OF THE VESSEL AND UPPER DECK WERE NOT MADE IN JUNCTION BOXES, TO INCLUDE CONNECTIONS FOR A LIGHTING FIXTURE IN VOID #2.

ALL CONNECTIONS SHALL BE MADE IN ENCLOSURES; JUNCTION BOXES SHALL BE WATERTIGHT IF IN AN AREA SUBJEST TO WATER EXPOSURE. MULTIPLE DEAD END WIRES LOCATED IN THE ENGINE ROOM, OUTSIDE DECK NEAR STBD CABIN DOOR, UPPER DECK, AND VOID SPACES. REMOVE OR PLACE INTO A CONNECTION, IN AN ENCLOSURE.

EACH UNGROUNDED SYSTEM MUST BE PROVIDED W/A SUITABLY SENSITIVE GROUND DETECTION SYSTEM. VESSEL DOESN’T HAVE A GROUND DETECTION SYSTEM.

PENETRATIONS IN WATERTIGHT BULKHEADS MUST HAVE MEANS TO MAKE THEM WATERTIGHT AND BE PROTECTED BY METAL COVERING/OTHER SUITABLE MEANS IF SUBJECT TO MECHANICAL ABUSE. CABLE PASS-THROUGH FROM VOID 2 TO THE FOREPEAK (FWD OF COLLISION BULKHEAD) IS NOT PROTECTED FROM THE MECHANICAL ABUSE OF THE SHIFTING HULL AND HAS LOST WATERTIGHT PUTTY THAT WAS INSTALLED IN THE HOLE.

EACH EPIRB SHALL BE TESTED MONTHLY & TESTS LOGGED, HAVE A BATTERY THAT IS NOT EXPIRED, AND BE REGISTERED. THE EPIRB WAS NOT BEING TESTED/LOGGED AND HAD AN EXPIRED REGISTRATION & BATTERY. EPIRB SHALL BE MAINTAINED IAW THIS REGULATION OR REMOVED FROM VESSEL.

VESSELS SHALL BE MAINTAINED IAW GOOD MARINE PRACTICES/STANDARDS ACCEPTABLE TO THE OCMI. THE VESSEL HAD MULTIPLE HOSES SUPPORTED THROUGHOUT THE VESSEL & BILGES.

LIFE FLOATS KEPT ONBOARD THE VESSEL SHALL BE MAINTAINED IN AN OPERATIONALLY READY CONDITION. THE LIFE FLOATS WERE INCORRECTLY MARKED, THE RETRO-REFLECTIVE TAPE WAS DETERIORATED, THE LIFE-LINES WERE MISSING, AND THE PADDLES WERE DETERIORATED AND NOT MARKED. LIFE FLOATS TO BE MADE OPERATIONALLY READY OR REMOVED FROM THE VESSEL.

A FIRST AID KIT SHALL COMPLY WITH THE REQUIREMENTS OF 46CFR160.041. VESSELS SUPPLIED FIRST AID KITS DID NOT HAVE ALL OF THE ITEMS REQUIRED BY 160.041.

NONMETALLIC FLEXIBLE HOSE SHALL MEET THE REQUIREMENTS OF 46CFR56.60-25 (SBCHAPTER F [SAE J1942]). PROVIDE DOCUMENTATION/SPECIFICATIONS SHEET ON HOSES ATTESTING TO MEETING THE REQUIREMENTS.

Make proper repairs to cracks found in 3 spots. 1) along the deck plate of aluminum tube on the forward section of pulpit. 2) hand rail on starboard side bow. 3) benche seating in front of the pilot house.

A vessel of more than 65 ft must carry a minimum of three life buoys of not less than 24 inches in diameter. Two rings are damaged beyond repair and must be replaced. 46 CFR 180.70(a)(3)

A vessel on Lakes, Bays, and Sounds and Rivers must carry three red hand distress flares and three orange hand smoke distress signals. All pyrotechnics on board are expired. 46 CFR 180.68(b)

The decks separating accommodation and machinery spaces shall be of "A" class construction. Replace fire boundary material in engine space in way of overhead soft patch. 46 CFR 92. 07-10(c)

A vessel must carry adequate and up to date charts and U.S. Coast Pilot. Charts were outdated and U.S. Coast Pilot was not on hand. 46 CFR 184.420

The operator of each self-propelled vessel 12 meters or more in length shall carry on board and maintain a copy of Navigation Rules. No copy was on board. 33 CFR 88.05

Provide one line diagram of electrical circuit used for toilet solenoids as per 46 CFR 110.25. Installations of materials shall be in accordance with reg 46 CFR 110. 46 CFR 110. 25

Provide proper operation of #4 visual alarm.

Ensure all bilge alarm connections are in suitable junction boxes.

Provide 39 additional adult lifejackets. Pax count reduced by 39.

Provide valid FCC station license.

Provide proper documentation of senior deckhand training program and designation.

Engine room inspection yielded MDE cooling water piping on MDE#1 (port outboard)had improper repair.Vessel is not authorized to use engine until proper repairs are made.

Vessel loss propulsion due to a fuel system issue, cause is currently unknow.

Bilge alarm panel is not providing audible or visual alarm for forepeak and engine room spaces. Vessel must demonstrate proper operation to an attending CG Marine Inspector.

Vessel bilge high level alarm at the operating station is damaged. Owner must replace with alarm panel to meet the requirement of the regulation.

Stability letter was not posted at the operating station. Vessel shall post the stability letter as required.

Vessel did not have Emergency Instructions posted. Vessel must generate and post Emergency Instructions as required by the regulation.

The hydrostatic release unit for the EPIRB has expired. Replace with a new unit meeting 46 CFR 122.740

Owner/operator shall demonstrate compliance with 46 CFR Part 16 Chemical Testing Program by completing an audit performed by a CG Marine Inspector.
