A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Vessel's licensed master/owner did not have medical certificate available for inspection. Provide proof of medical certificate or correspondence from NMC attesting to valid medical certificate.

Employers must ensure that they and their crew-members meet the requirements of 46 CFR Part 16. Vessel could not provide proof of crew members enrollment in a drug and alcohol program required by 46 CFR Part 16. Provide proof of crew enrollment in required drug and alcohol program as required by 46 CFR Part 16 to attending Marine Inspector.

A vessel must carry either a first-aid kit approved under approval series 46CFR 160.041 or kit w/ equivalent contents and instructions. Items in kit were expired/ missing. Make kit in compliance with regulations.

Vessel must meet survival craft requirements as per the table in 46 CFR 180.200(c). Vessel's current IBA is being serviced and there is no primary lifesaving onboard the vessel. The vessel may not operate under its COI until satisfactory primary lifesaving has been installed. This CG-835 will remain in effect until such time that the vessel is able to prove satisfactory primary lifesaving.

An adult life jacket must be provided for each person carried on board a vessel. Due to 04 life jackets being found not serviceable, vessel was short 02 adult life jackets required by the vessel's COI. Vessel's COI requires 44 adult life jackets, but currently only has 42. When vessel is operating on a Coastwise route, vessel may only allow a total of 42 persons, not 44, until vessel acquires 02 more satisfactory life jackets.

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. vessel is currently outside its 3 month anniversary date for its annual inspection, technically making the vessel’s COI invalid. This annual inspection will need to be completed prior to resuming commercial service under its current COI. Reconsideration is being made due to availability of dry-dock scheduling, and crew emergency. This reconsideration is made this one time due to these circumstances. To avoid future enforcement, all efforts should be made by the vessel to complete its required annual inspection within 3 months before or after its COI anniversary date of 19 October.

INSTALL A LIFEJJACKET DONNING PLACARD FOR ALL LIFEJACKET STYLES CARRIED ONBOARD THE VESSEL TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

There were numerous cracked rail stanchions as well as wasted bolts in way of the stanchions that need to be replaced.

Install sea painter and weaklink for lifefloats, prior to the carriage of passengers.

Provide B-II portable fire extinguisher for machinery space in place of B-I

Provide valid Certificate of Documentation.

Provide valid FCC Safety Certificate and Marine Radio Operator Permit.

Provide proof of enrollment in annual drug and alcohol testing program.

Provide valid tide and current tables.

Provide 6 red flares and 6 orange smokes (or appropriate mix for coastwise route).

Provide valid EPIRB.

Provide sanitary conditions for crew and passengers.

Provide proof of annual service of all portable fire extinguishers.

Port main propulsion engine inop. Provide sat operational test of both main propulsion engines.

Provide valid master's license for minimum of 100 gt.

AREA ON BOW IN WAY OF WHERE ANCHOR RIDES UP WAS FRACTURED.

HYDRAULIC OIL HAD BEEN SPILLED IN FORWARD STORES AREA.

EXHAUST MANIFOLD AND VENT HOSE HAD HOLES IN THEM.

PROVIDE PROOF OF DECK HAND ENROLLMENT IN DRUG TESTING PROGRAM.

Clean oily water from bilge in engine room.

Repair elecrical short in anchor windlass switch. Show proper opertion in both directions.

Provide proof of a drug testing program. Submit program to MSO Inspection department for review.

Provide one adult PFD, vessel is restricted to a total of 49 persons_x001C_ until corrected.

Provide placard near both MSD overboards to state "do not open within 3 miles of shore".

Provide some form of locking device for both overboard discharge valves forMSD.

Treat/clean furring on all engine room overhead structures.

PLACE REFLECTIVE TAPE ON TWO LIFE PRESERVERS IN ORDER TO COMPLY WITH REQUIREMENTS SET FORTH ON VESSEL COI, IN REGARD TO NUMBER OF LIFE PRESERVERS REQUIRED. UNTIL SUCH TIME AS THIS REQUIREMENT IS MET, NO MORE THAN 48 TOTAL PERSONS MAY BE ABOARD VESSEL WHILE ON LB&S ROUTE.

PLACE AND POST VALID FCC RADIOTELEPHONY CERTIFICATE ABOARD VESSEL

INSTALL HEAT DEFLECTOR SHIELDS UNDER EXISTING RACOR FUEL FILTERS TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

INSTALL SECURING DEVICES ON ALL HATCHES EXPOSED ON WEATHER DECK TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

PROVIDE COAST GUARD APPROVED LIFE FLOAT OR BUOYANT APPARATUS WITH 15-MAN CAPACITY OR MORE. DEVICE SHOULD BE PROPERLY MARKED AND INSTALLED. DEVICE MUST BE PROVIDED PRIOR TO CARRYING PASSENGERS FOR HIRE ON A COAST- WISE ROUTE.

SERVICE ALL PORTABLE FIRE EXTINGUISHERS.

PROVE OPERATION OF EMERGENCY FUEL SHUT OFF ON PORT SIDE.
