Each ring life buoy on a vessel must be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Ring life buoy exterior plug sealing the hollow inner foam core to prevent water from entering is missing making it not as per the subchapter Q approval. Replace ring life buoy by June 8, 2026, to the satisfaction of the attending Marine Inspector. 46 CFR 180.70(b)(1)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Relays located in the engine room for the Fireboy engine automatic shutdown are exposed and tied to a wire loom. Install relays in an approved enclosure to prevent them from being damaged to ensure proper operation of the Fireboy automatic engine shutdown requirement to the satisfaction of the attending Marine Inspector.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Flying bridge port side under console electrical toggle switches and a box with an electrical gauge are not mounted and are hanging by their electrical cables. Mount these electrical components to the satisfaction of the attending marine inspector.

Each pressure type wire connector and lug must meet UL 486A or other standard specified by the Commandant. The use of twist-on type wire nuts is permitted under the following conditions: (1) The connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration; and (2) Twist-on type connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Numerous twist-on type connectors throughout the vessel electrical system. Meet the regulation requirement above or remove the twist-on connectors and meet the requirements as described by 46 CFR 183.340(g) to the satisfaction of the attending Marine Inspector.

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Several blade type fuses are installed throughout the vessel electrical system. Replace with UL approved cartridge fuses to the satisfaction of the attending Marine Inspector.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First-aid kit missing 5 iodine swabs. Provide 5 iodine swabs to the satisfaction of the attending Marine Inspector.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Crew aboard vessel was missing an individual with a valid marine operator permit. Provide in accordance with 47 CFR part 80 to the satisfaction of the attending Marine Inspector.

EAP training program: An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Captain is in a supervisory position and has not received the required 60-minute training. Provide proof of the training required as described by 16.401(b) to the satisfaction of the attending Marine Inspector. 46 CFR 16.401(b)

EAP education program: Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Community service hot-line telephone numbers and company policy regarding drug and alcohol use in the workplace are not posted. Post information as described above to the satisfaction of the attending Marine Inspector. 46 CFR 16.401(a)

A pre-engineered fixed gas fire extinguishing system must: (3) Automatically shut down all power ventilation system and all engine that draw intake air from within the protected space. Fireboy for starboard engine room inoperable. Repair or replace fireboy and provide a certified technician service report and demonstrate all shutdowns, alarms, and overrides to the satisfaction of the attending Marine Inspector. Operations are prohibited until deficiency is rectified. 46 CFR 181.400(a)(1)

The equipment required for lifefloats and buoyant apparatus must meet the following specifications:(1) The lifeline and pendants must be as furnished by the manufacturer with the approved life float or buoyant apparatus. Replacement lifelines and pendants must meet the requirements in Subpart 160.010 of this chapter. Pendants must be fitted approximately 450mm (18 in.) apart around the outside of each buoyant apparatus. Each pendant must be at least 6mm (1/4 in.) diameter, at least 3.5m (12ft.) long, secured in the middle, and have a breaking strength of at least 2400 N (540lb.) Each pendant must be made up in a hank, and the hank secured by not more than two turns of light twine. 2/2 buoyant apparatus missing pendants. Affix pendants appropriately to each buoyant apparatus to the satisfaction of the attending Marine Inspector. 46 CFR 180.175 (f)(1) 46 CFR 160.010-4 (g)

An EAP training program must be conducted for the employer's crew members and supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Vessel Captain did not receive supervisory training. Provide a written record of supervisory training conducted for company supervisory personnel to the satisfaction of the attending Marine Inspector. 46 CFR 16.401(b)

The float-free link required by § 180.130(a)(1) must be:(1) Certified to meet § 160.073 in subchapter Q of this chapter, or other standard specified by the Commandant. Float-free link for the stbd life float not a 160.073 certified link. Replace float-free link for the stbd life float with certified link meeting 160.073. Vessel passenger carriage restricted to 30 total number of persons permitted on board until issue is resolved to the satisfaction of the attending Marine Inspector. 46 CFR 180.137(b)(1)

Painter. The painter must:(i) Be of at least 30.5 meters (100 feet) in length, but not less than 3 times the distance between the deck where the life float or buoyant apparatus it serves is stowed and the lowest load waterline of the vessel. Life float painter was less than 100 feet. Provide 100 at least 100 feet of synthetic line that is of a dark color or is of a type certified to be resistant to deterioration from ultraviolet light. Vessel passenger carriage restricted to 30 total number of persons permitted on board until issue is resolved to the satisfaction of the attending Marine Inspector. 46 CFR 180.175(f)(3)(ii)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel's two all-round lights in a vertical line, the upper being red and the lower white as described by navigation rule in 33 CFR 83.26(c)(i); not illuminating. Repair lights and prove proper operation to the satisfaction of the attending Marine Inspector. Vessel restricted to day light fishing using day shape prescribed by this Rule. 46 CFR 183.420

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that:(1) The pump is listed by an independent laboratory as meeting the requirements in UL 1113. Vessel submersible electric bilge pumps are not UL 1113 approved. Replace pumps with UL 1113 approved pumps or meet Old T bilge pumping arrangements described in 46 CFR 182.25-10 to the satisfaction of the attending Marine Inspector. 46 CFR 182.520(e)(1)

When the flexible section is a nonconductor of electricity, the metallic sections of the filling pipe separated thereby shall be joined by a conductor for protection against static spark when filling. Both port and stbd filling pipe flexible section are missing the conductor for protection against static spark. Provide conductor for protection against static spark to the satisfaction of the attending Marine Inspector. 46 CFR 182.15-30(d) (Old T)

A battery charger intended for connection to a commercial supply voltage shall employ a transformer of the isolating type. An ammeter that is readily visible shall be included in the battery charger circuit. Installed battery chargers on board the vessel are missing ammeters. Install ammeters or replace battery chargers with models with a built-in ammeter to the satisfaction of the attending Marine Inspector. 46 CFR 183.05-20(f) (Old T)

Flexible or portable cords such as SO, STO, and similar types cannot be used for permanently installed wiring. Portable cord used for extending a circuit to provide power to surge protector device. Remove portable cord and plug surge protector device directly into the outlet or permanently wire to the satisfaction of the attending Marine Inspector. 183.10-20(a)(2)

Batteries shall be so located that the gas generated in charging will be easily dissipated by natural or induced ventilation. Batteries should not be located in the same compartment with a gasoline tank or gasoline engine, but where location elsewhere is impracticable, they shall be effectively screened by a cage or similar structure to minimize the danger of accidental spark through dropping a metal object across terminals. Battery under ladder leading to forward compartment missing battery box cover. Provide proper battery box cover to the satisfaction of the attending Marine Inspector. 46 CFR 183.05-20(a) Old T

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Fuse located on the stbd forward engine room bulkhead, single stud terminal block, and all wire terminal blocks missing protective covers. Install protective covers to the satisfaction of the attending Marine Inspector. 46 CFR 183.01-5 Old T

Vapor barriers. Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. Cable penetrating the forward engine room bulkhead above the Fireboy cylinder is not vapor tight. Make vapor tight to the satisfaction of the attending Marine Inspector. 46 CFR 177.405(e)

Each hatch exposed to the weather must be watertight; except that, the following hatches may be weather tight:(1) Each hatch on a watertight trunk that extends at least 12 inches (30.5 centimeters) above the weather deck.(2) Each hatch in a cabin top.(3) Each hatch on a vessel that operates only on protected waters. Vessel route is Limited Coastwise. All deck watertight hatch gaskets are wasted allowing water into the interior of the vessel. Replace gaskets and test watertightness tot eh satisfaction of the attending Marine Inspector. 46 CFR 171.124(a)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Numerous dead end cables throughout the vessel. Permanently remove or encapsulate with electrical tape to the satisfaction of the attending Marine Inspector. 46 CFR 183.01-5 (Old T)

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. Secure sewage valve in a manner which prevents discharge of treated or untreated sewage by one of the acceptable methods described by 33 CFR 159.7(b) to the satisfaction of the attending Marine Inspector. 46 CFR 184.704

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. 1) Remove all oils, lubricants, and paints from the vessel. Only minimum quantities of oil and lubricates essential for daily operations can remain on board; 2) Remove all excess gear and equipment being stored on board that is not essential for daily operations; 3) Secure for sea all equipment and supplies remaining aboard to prevent their shifting while the vessel is at sea. To the satisfaction of the attending Marine Inspector. 46 CFR 176.830(a)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The rails on the periphery of the deck fasteners holding power has degraded making the rails subject to excessive deflection. Renew the periphery deck rail fasteners to make the rails strength as designed and constructed to the satisfaction of the attending Marine Inspector. 46 CFR 176.830(a)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Welds on sections of the rails on the periphery of the deck are cracked. Reweld to the satisfaction of the attending Marine Inspector. 46 CFR 176.830(a)

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Bow and stern compartment high water alarm and lights not operational. Demonstrate operation to attending marine inspector. 46 CFR 182.530(a)

The OCMI may require the vessel to get underway. Get underway in presence of marine inspector to conduct drills. 46 CFR 176.404(a)

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. One paddle is missing for one LF. Paddles are not lashed to LF. Floating waterlights not fitted with at least 18 feet of line and not attached around the body of LF. Float-free links are missing identification tags. Route restricted to not more than 3 miles from shore. 46CFR180.175(d) 46CFR180.137(b)(2)

A vessel on a limited coastwise route must carry- six hand red flare and six hand orange smoke distress signals. 2 red flare and 3 orange smoke distress signals are expired. Route restricted to lakes, bays, and sound. 46CFR180.68(a)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Medications are expired. 46CFR184.710

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. Vessel's O/O was unable to provide evidence of DAPI program documentation. Provide satisfactory evidence of a chemical testing program. 46 CFR 16.201(a)

A pre-engineered fixed gas fire extinguishing system must: Be installed in accordance with the manufacturer's instructions. Several holes between engine room and adjacent fwd and aft compartments allow fixed fire extinguishing medium to communicate through bulkhead. Make vapor tight to ensure quantity of extinguisher is sufficient. 46 CFR 181.420 (a)(4)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from electrical shock of personnel. Electrical wiring over stbd engine has several exposed and dead end wires. Properly renew or cover wires to prevent potential shock or fire hazards. 46 CFR 183.01-5

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods include - (1) closing each valve and removing the handle; (2) padlocking each valve in the closed position; (3) Using a non-releasable wire tie to hold each valve in the closed position. Vessel’s MSD system was not secured to prevent discharge. Secure to the satisfaction of attending Marine Inspector. 33 CFR 159.7 (c)

Each life float shall be fitted with a life line and shall be equipped with two paddles, a water light and a painter. Vessel's Stbd side life float life line was deteriorated. Replace life line. 46 CFR 180.15-1(a)

Each survival craft must be: Stowed so that when the vessel sinks the survival craft floats free. Vessel’s life floats were secured to the vessel in a non-float free arrangement, with lights detached. Properly stow life float so that the survival craft floats free with lights attached. 46 CFR 180.130(a)(2)

A hatch cover must have securing devices and be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Provide hatch cover securing device for forward watertight hatch. 46 CFR 179.360

An internal structural examination consists of a complete examination of the vessel's voids and ballast, cargo, and fuel oil tanks. Vessel has significant build up on independent fuel tanks which when one area was cleaned, revealed significant pitting. Clean vessels fuel tanks for re-inspection. 46 CFR 176.610 (b)

A vessel must comply with the structural design requirements of one of the standards listed. Vessels longitudinal support under gunwales has significant angel hair rot. Submit repair proposal to satisfaction of attending marine inspector.

A vessel must comply with the structural design requirements of one of the standards listed. Vessels has 4" x 4" longitudinal support beam in rudder room that is rotten. Repair as discussed. 46 CFR 177.300

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction. Fractured welds on fwd railing stanchions. 46 CFR 177.900 (b)

A vessel must comply with the structural design requirements of one of the standards listed. Area of angel hair wood rot found on port stbd side engine room. Repair as discussed. 46 CFR 177.300

No person may operate any vessel equipped with installed toilet facilities unless it is equipped with an operable Type II or III device. Sewage was being drained into holding tank via an unsecured pipe. Install air-tight and watertight connection between drainage pipe from toilet and holding tank. 33 CFR 159.7(a)(1)

Electrical equipment on a vessel must be installed and maintained to minimize electrical hazards. Decommissioned generator in engine compartment was electrically connected to ship. Disconnect generator from vessel's electric circuits. 46 CFR 183.200

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing MMSI and LAT/LONG for DSC operation. 47 CFR 80.905(1)

Any vessel of at least five net tons which engages in coastwise trade must have a Certificate of Documentation bearing a valid endorsement. Vessel is in the process of being sold, COD submitted as part of sale. Provide valid COD. 46 CFR 67.7

The OCMI may require the vessel to get underway. Get underway in presence of marine inspector to conduct drills following drydock. 46 CFR 176.404 (a)

A pre-engineered fixed gas fire extinguishing system must: automatically shut down all engines that draw intake air from within the protected space. Stbd MDE restarted with fireboy simulated activated and override switch not engaged. 46 CFR 181.420 (a)(3)

Nozzles, flanges and other fittings for pipe connections shall be welded or brazed to the tank. Stbd center fuel tank vent pipe corroded and broken off at tank top. 46 CFR 182.20-25 (a)(5)

Provision shall be made by means of plugs, covers, etc. to percent admission of air into the space. Plugs for engine room ventilation corroded through allowing passage of air. 46 CFR 181.20-35 (a)

All connections shall be made to the battery terminals with permanent type connectors. Connections on batteries made with non permanent connectors. 46 CFR 183.05-20 (c)

The master of a ship shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations. Garbage placard on vessel torn and unservicable. 33 CFR 151.59 (a)

All machinery must be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Flexible hose on port reduction gear oil cooling line deteriorated. Replace in kind. 46 CFR 182.01-5 (a)

A pre-engineered fixed gas fire extinguishing system must: Be installed in accordance with the manufacturer's instructions. 3" hole between engine room and head allow fixed fire extinguishing medium to communicate through bulkhead. Make vapor tight to ensure quantity of extinguisher is sufficient. 46 CFR 181.420 (a)(4)

Provide individual visual alarm for each unmanned spaces with high level bilge alarm. A visual and audible alarm must be provided at the operating station to indicate a high water level in each of unmanned spaces.

Replace worn upper bearings for port and stbd rudder posts. A vessel must be provided with a main steering gear that is of adequate strength and capable of steering the vessel at all service speeds.

Repair fracture on stbd aft corner above lazarette. The weather deck on a flush deck vessel must be watertight.

Repair fracture on port overboard drainage above aft portion of steering compartment. The weather deck on a flush deck vessel must be watertight.

Secure fuel lines in lazarette and steering compartments. Fuel lines must be effectively secured against excessive movement and vibration by the use of soft nonferrous metal straps which have no sharp edges and are insulated to protect against corrosion.

Vessel was 11 days past due for their annual inspection.

Update EPIRB registration call sign to reflect call sign shown on FCC Radio Safety Telephony Certificate and USCG Certification of Documentation.

Conduct maintenance to stop excessive oil leaks on main diesel engines.

Mount life jacket donning placards for the type of life vests kept onboard. Mounted placards depict a different brand/type of life jacket than the life jackets stowed onboard.

Update drug and alcohol policy to identify who the company DER is, and what their role/responsibilities are.

Post stability letter dated 11/04/2009 in addition to the stability addendum issued by Sector Honolulu in 2012.

Provide current and updated navigational charts and other required navigational publications such as U.S Coast Pilot, USCG Light List, etc for the area vessel operates.

Provide instructions list for items in first aid kit.

Provide USCG approved bracket for fire extinguisher located on open bridge.

Remove diesel fuel and lube oil from bilges in engine room and void space aft of engine room.

Clean oil/fuel from submersible bilge pumps and demonstrate proper operation of pumps.

Permanently mount all submersible bilge pumps, and ensure discharge hoses are properly supported.

Replace missing lagging/insulation on port main diesel engine exhaust pipe.

Provide new weak link for PORT lifefloat.

Isolate positive battery cables from steel frames/engine parts.

Ensure collision bulkhead is properly sealed/water tight by completion of upcoming dry dock in Aug 2016.

Ensure both hatches up forward leading down into the forward storage rooms are made watertight and has securing devices attaching the hatch to the hatch frame/coaming by completion of dry dock in Aug 2016

Provide means to make hatch for void aft of engine room watertight by completion of upcoming dry dock in Aug 2016.

Provide visual indicator at the vessel operating station for the high level bilge alarm

Reconnect STBD alternator to provide power to vessel DC loads and charge house battery bank IAW USCG approved one-line diagram. Ensure alternator is properly rated to simultaneously provide power to house loads and charge house batteries.

Repair busted/fractured transverse deck beams in fuel tank space. Beams are located FWD-to-MID STBD side at the hull-to-deck joint where the transverse beams bolt into the longitudinal frames on the side shell.

Remove rusted/corroded bolts for inspection at deck-to-hull joints in each compartment at upcoming dry dock in August 2016. Replace all severely corroded bolts.

Remove loose lag bolts in rail stanchion base for loose stanchion located STBD side outboard of the pilot house at upcoming dry dock in August 2016. Inspect wood in way of lag bolts and make necessary repairs to ensure rail is of adequate strength and deck is sealed to prevent water intrusion.
