In addition, each pressure vessel listed on the Certificate of Inspection that is fitted with a manhole or other inspection opening so it can be satisfactorily examined internally, must be opened for internal examination every 5 years. Both pressure vessels were in use an unable to be inspected internally. Vessel will make UPV available for inspection to the satisfaction of Coast Guard MI.

If your vessel's Certificate of Inspection is renewed less often than annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor. Both pressure vessel relief valves were unable to be tested due to the unknown accuracy of the installed pressure gauge. Vessel will renew relief valves or satisfactorily test in the presence of a Coast Guard MI.

Red warning signals. During transfer of bulk cargo while fast to a dock, a red signal (flag by day and electric lantern at night) shall be so placed that it will be visible on all sides. While transferring bulk cargo at anchor, a red flag only shall be displayed. Red light mounting post broken at its base and light/post were hanging by the electrical conductor. Vessel shall repair to the satisfaction of Coast Guard MI.

he capacity of the ship's service generating sources must be sufficient for supplying the ship's service loads... Installed generator was inoperable and the barge is relying on a portable generator. Vessel shall repair to the satisfaction of Coast Guard MI.

The inspection for certification shall include an inspection of...machinery and equipment. The inspection shall be such as to insure that the...main and auxiliary machinery, electrical installations...and other equipment is in satisfactory condition and fit for the service for which it is intended. While attempting to start the generator the exhaust pipe was found leaking at the piping near the exhaust manifold. Vessel shall repair to the satisfaction of Coast Guard MI.

The cargo pump discharge must have a relief valve that is (a) Fitted between the cargo pump discharge and the shut-off valve, with the relief valve discharge piped back to the cargo pump suction or returned to the tank; and (b) Set at the maximum design pressure of the piping and discharge hose, or less. Cargo pump relief valve #2 starboard was found with the set screw at the top of the valve damaged. Vessel shall repair to the satisfaction of Coast Guard MI.

No person may operate, install, or reinstall an air compressor in a cargo area described in 46 CFR 32.35-15 of this subchapter. Marine Inspector observed a wheeled air compressor affixed to the cargo tank top within the cargo area as defined in 46 CFR 32.35-15. Air compressor is to be removed from the cargo area. 46 CFR 35.35-85

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Marine Inspector identified two ring buoy lights that were not working, which need to be repaired or replaced.

All portable and semiportable extinguishers on board tank vessels shall be of an approved type. The forward and center CO2 fire extinguishers have deteriorated hoses with several cracks, which need to be verified by an approved fire servicing facility as satisfactory for service or replaced.

Each hose must be marked with one of the following: For oil products, the words “OIL SERVICE”. The three transfer hoses onboard are not marked with the words "OIL SERVICE". 33 CFR 154.500(e)(2)

Each hose also must be marked with the following, except that the information required by paragraphs (f)(2) and (3) of this section need not be marked on the hose if it is recorded in the hose records of the vessel or facility, and the hose is marked to identify it with that information: (1) Maximum allowable working pressure; (2) Date of manufacture; and (3) Date of the latest test required by 33 CFR 156.170. The three transfer hoses onboard do not have the maximum allowable working pressure marked on them. Two of the three transfer hoses don't have marked or recorded the date of manufacturer. Lastly, the test pressure is marked on all three transfer hoses, which must be removed. 33 CFR 154.500(f)(1)-(3)

Approved portable extinguishers must be installed in accordance with table 34.50–10(a) of this section. The barge is missing the following two approved portable fire extinguishers (PFEs): One 40-B PFE for the radar emergency generator space on the bow to be in the vicinity of the exit, one 40-B PFE for the port side forward cargo pump prime mover. 46 CFR 34.50-10(a)

The locality and arrangement of such additional means of escape shall be determined by the inspectors as will in their judgment best carry out the purpose for which this provision was made. Install adequate illuminated markings showing means of escape from berthing area to escape hatch. Provide evidence or markings to Marine Inspector.46CFR32.02-1

Electrical installations on vessels must ensure: (1) Maintenance of services necessary for safety under normal and emergency conditions. The generator battery was unable to hold a charge and failed to start the generator. Vessel must determine the cause of the failure and provide satisfactory repairs to the Marine Inspector. 46 CFR 111.01-1

Each launching station, or each two adjacent launching stations, must have an embarkation ladder approved under approval series 160.017. Port side life raft launching station was not equipped with an embarkation ladder. Vessel must install an embarkation ladder meeting the requirements of CFR to the satisfaction of a Marine Inspector. 46 CFR 199.110(f)

Weekly inspection and test. (1) Each survival craft, rescue boat, and launching appliance must be visually inspected to ensure its readiness for use. Weekly inspections of the lifesaving equipment were not being conducted or logged. Vessel must conduct and log weekly inspections of lifesaving equipment to the satisfaction of a Marine Inspector. 46 CFR 199.190(d)

The owner, master, or person-in-charge of a tank vessel must ensure that portable and semi-portable extinguisher are inspected and maintained in accordance with NFPA 10. Monthly inspections required by NFPA 10 were not being conducted or logged. Vessel must conduct and log monthly inspections of firefighting equipment to the satisfaction of a Marine Inspector. 46 CFR 31.10-18(a)

Weather locations. Class I, Division 1 locations may have only approved intrinsically safe, explosion proof, or purged and pressurized electrical equipment. Vessel had an intrinsically safe light in the starboard side hydraulic prime mover room that was missing a fitting compromising the integrity. Repairs are to be made to the satisfaction of the Marine Inspector. 46 CFR 111.105-31(l)

Cargo pump pressure gauges are required on the discharge side of the cargo pump iaw 46 CFR 32.50-5. Cargo pressure gauges were not available for inspection.

Cargo piping is excessively wasted in certain areas. Vessel operator must complete guaging and submit repair proposal, and complete repairs to the affected piping. 46 CFR 56.04-2

Only cargo grade D and lower may be loaded in tanks 1port, 4stbd, and 3stbd until broken sight glasses are replaced. 46CFR32.20-20

Make temp repairs to damage on bow, port side center, as a result of the collision with the T/B GULL.

INSTALL AUDIBLE ALARM TO INDICATE A LOSS OF ALL POWER TO THE TANK LEVEL ALARM SYSTEM.

INSTALL 1/2" X 1" DOWELL AT THE TWELVED CLOCK POSITION ON THE ANSI 150 VAPOR RECOVERY PLANGE ON THE PORTSIDE FACING AFT IN THE WAY OF#2 PORT CARGOTANK.

PROVIDE [name withheld] WITH DOCUMENTATION OF TESTING OF CARGO HOSES, RELIEF VALVES AND CARGO PUMP PRESSURE GAUGES, PRIOR TO LOADING OR UNLOADING OF CARGO.

REPAIR AUTOMATIC SHUTDOWN FOR NUMBER ONE CARGO PUMP AND PROVE PROPER OPERATION.

REPLACE BOLTS IN THE REACH ROD FLANGE IN TO THE DRAIN VALVE IN THE STAR- BOARD AFT PEAK. THE BOLTS FOR THE FLANGE WERE MISSING AND A WIRE WAS IN- STALLED INSTEAD. THERE WAS TOO MUCH FREE MOVEMENT OF THE FLANGE BECAUSE OF THE WIRE WHICH PREVENTED PROPER OPERATION OF THE DRAIN VALVE.

REPLACE THE VALVE WHEEL TO THE DRAIN VALVE IN THE STARBOARD AFT PEAK. THE VALVE WHEEL IS CORRODED TO THE EXTENT THAT COMBINED WITH THE MISSING BOLTS THE VALVE DOES NOT OPERATE PROPERLY.

CONDUCT HYDROSTATIC TEST TO CARGO HOSES AND CARGO PIPING PRIOR TO TRANS- FERRING CARGO.

DRILL STOP HOLES IN 8" CRACK IN BRACKET WELD IN PORT AFT PEAK, CRACK IS 3 FEET FROM FORWARD BULKHEAD, 18" UP FROM THE TURN OF THE BILGE. THE VESSEL HAS A DENT ON THE OUTSIDE OF THE HULL. VESSEL'S OPERATORS WERE NOT SURE WHEN THE DENT OCURRED.

CONDUCT NON DESTRUCTIVE TESTING TO THE AREA OF THE CRACKED WELD AND THE AREA FORWARD OF THE WELD IN THE AFT PEAK. THE CRACK IS 3 FEET FROM THE FOR-WARD BULKHEAD, 18" UP FROM THE TURN OF THE BILGE. RESULTS OF THE NDT ARE TOBE PROVIDED TO [name withheld] PRIOR TO TAKING ON CARGO.

PROVIDE ON BOARD A CURRENT IOPP FORM B CERTIFICATE AS REQUIRED BY 33 CFR 151.
