The main steering gear and rudder stuck shall be: capable of putting the rudder over from 35 degrees on one side to 35 degrees on the other side with the ship at its deepest seagoing draught and running ahead at maximum ahead service speed... PSCO’s noticed starboard side steering gear hydraulic ram did not allow the rudder to go past 25 degrees on the rudder angle indicator. 74 SOLAS (04) II-1 / 29.3.2

Resolution: Rams were serviced and reinstalled. They are now capable of putting the rudder 35 degrees port and starboard.

Each emergency generating set arranged to be automatically started shall be equipped with starting devices approved by administration with stored energy capability of at least three consecutive starts. A second source of energy shall be provided for an additional three starts within 30 min unless manual testing can be demonstrated to be effective. PSCO’s observed emergency generator only had one source of energy and no manual starting arrangements. 74 SOLAS (04) II-1 / 44.2

Resolution: Both steering pumps along with the hydraulic tank, have been re-located to the center of the steering gear compartment. Two joystick lever controls – one for each pump, have been installed onto the aft transverse bulkhead of the steering gear compartment.

Hydraulic power-operated steering gear shall be provided with the following: a low –level alarm for each hydraulic fluid reservoir to give the earliest practicable indication of hydraulic fluid leakage. Audible and visual alarms shall be given on the navigation bridge and in the machinery space where they can be readily observed. PSCO’s observed no low-level audible alarms in the machinery space. 74 SOLAS (04) II-1 / 29.12.2

Resolution: A new audible low-level alarm was installed for the reservoir in the engine room.

Two means of escape shall be provided from each machinery space. In particular, the following provisions shall be complied with: Two sets of steel ladders as widely separated as possible, leading to doors in the upper part of the space similarly separated and from which access is provided to the appropriate lifeboat and liferaft embarkation decks. PSCO’s observed that the escape door leading to upper deck had a lock on it and required a key to open it. 74 SOLAS (97) II-2/ 28.3.1.1.1

Resolution: The locks have been removed and replaced by an easily removable pin which is capable of locking the escape door from the inside.
