For a life float—Approved under approval series 160.027 or other standard specified by the Commandant. Buoyant apparatus may be used to meet requirements for life floats if the buoyant apparatus was installed on board the vessel on or before March 11, 1996, and if the buoyant apparatus remains in good and serviceable condition. Vessel has two buoyant apparatus installed that were manufactured in 2021. Vessel is required to replace buoyant apparatus with life floats approved under approval series 160.027 prior to 2026 operating season.

Drydock examinations in excess of the periodic examinations required may be of the same scope; a periodic examination shall include the propellers and shall be of sufficient scope to determine that these items are in a satisfactory condition for the service intended. Each propeller has damage; replace each propeller at next scheduled drydock inspection so vessel is in a satisfactory condition for the service intended.

Drydock examinations in excess of the periodic examinations required may be of the same scope; a periodic examination shall include the underwater hull and appendages and shall be of sufficient scope to determine that these items are in a satisfactory condition for the service intended. Multiple insets on the port bow in the vicinity of the FWD hold compartment were discovered. Make necessary repairs at next scheduled drydock inspection so vessel is in a satisfactory condition for the service intended.

A vessel is inspected for compliance with the standards required by this subchapter. For items not covered by standards in this subchapter, inspections shall be in accordance with standards acceptable to the cognizant Officer in Charge, Marine Inspections, as good marine practice. In the application of inspection standards due consideration will be given to the hazards involved in the operation permitted by a vessel's Certificate of Inspection. Thus, the standards may vary in accordance with the vessel's area of operation or any other operational restrictions or limitations. Vessel ran aground near Ballast Island. Vessel's current material condition has not been verified and will require a damage survey by a Coast Guard Marine Inspector from the local OCMI prior to being authorized to operate in a commercial status. Vessel is not authorized to carry any passengers until authorized to do so by the local OCMI.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Two(2) of the lifefloats on board the vessel are deteriorated to the point they are no longer serviceable. Additionally, some of the other lifefloats required further inspection to ensure they are still serviceable. Vessel shall replaced the two deteriorated lifefloats and have the other remaining lifefloats (6) removed from their stowage position so they can be further inspected by a CG marine inspector. Vessel's passenger capability has been reduced to two-hundred and sixty-four (264) passengers until the vessel receives the two new lifefloats that have been purchased and the stowage has been verified by a CG marine inspector. 46 CFR 180.175

Each paddle must be of at least 1,220 millimiters (4 feet) in length, lashed to the life float to which it belongs and buoyant. Some life float paddles are below four (4) feet in length. Replaced paddles that do not meet this criteria. 46 CFR 180.175(f)(2)

Life floats and buoyant apparatus must not be stowed in tiers more than 4 feet high. When stowed in tiers, the separate units must be kept apart by spacers. No spacers were installed to separate life floats. 46 CFR 180.137(f)

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Propeller shafts and front starboard engine do not have guards to protect personnel. Installed a means to protect personnel from rotating machinery by April 2025. 46 CFR 177.35-15

The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practicable. Exhaust and overhead bulkhead in the engine room is missing required insulation. Installed removed insulation to original condition. 46 CFR 177.10-5

A proper certificate issued by the Federal Communications Commission shall be regarded as evidence that the radio installation, if any, is in compliance with the requirements of that agency. Vessel did not have a certificate on board. 46 CFR 184.25-1(b)

The main steering gear and rudder stock must be of adequate strength for and capable of steering the vessel at maximum ahead service speed. Vessel experienced loss of steering. Necessary repairs must be made to steering system prior to operating with passengers.

High Level Float switch did not illuminate light in pilot house upon activation. Replace or properly repair bilge high water level float switch.

The freshwater line that supplies water to the vessel toilets is wasted and holed in way of the #5 WTB in the exhaust piping space just forward of the sewage waste tank and steering space. Toilets are unable to receive fresh water for flushing. Make permanent repair to fresh water line to the satisfaction of the attending Coast Guard Marine Inspector prior to carrying passengers.

The sewage tank vent line on the port side of the sewage tank in steering space is wasted and holed that may cause raw sewage to leak into space. Make permanent repairs to the vent line to the satisfaction of the attending Coast Guard Marine Inspector prior to operating any sewage or waste generating equipment.

Permanent repairs that were completed to the #5 WTB are unable to be nondestructively tested until the sewage tank vent line can be permanently repaired. Once vent line repairs are completed, conduct satisfactory nondestructive testing to the repaired #5 WTB welds in the presence of the attending Coast Guard Marine Inspector prior to carrying passengers.

Forward tank bulkhead that is integral to the #5 WTB is severely holed and wasted in it's entirety. Entire tank perimeter in the rudder space (all three (03) sides) is severely holed and wasted approximately 8" to 10" up from bottom plate. Viewing from the tank exterior (internal ladder wasted) the internal vessel longitudinal structural members are wasted beyond acceptance. Vessel transverse and longitudinal structural members in the rudder space surrounding and IWO the sewage tank are wasted beyond acceptance. Submit a repair proposal to crop out to good metal and renew wasted structural members and bulkheads prior to departure.

In the forward hold, two sections, of athwart-ships framing, were identified to be cropped and renewed. Submit a repair proposal to crop out to good metal and renew wasted structural members prior to departure.

Descale, clean, and evaluate the #5 WTB IWO Sewage tank and bottom plate, external to tank in the rudder space. Submit findings to attending Coast Guard Marine Inspector prior to departure.

Due to the unknown subtraction, addition and movement of weight on board this vessel, a Dead Weight Survey is required to be conducted and the calculations provided to the OCMI and/or the Coast Guard Marine Safety Center for review and issuance of a new stability letter. This must be done prior to carrying passengers.

Mount 02 fire extinguishers in Engine Room in an approved marine type bracket.

Seperate adult and child size PFD's on upper deck storage in order to prevent mixing of sizes.

Discovered a 10" diameter welded circular insert in the collision bulkhead located on the stbd side near the top section. Insert is not welded from the fwd side (welded on aft side only). Properly weld the insert with a full penetration weld so as to ensure watertight integrity. (46CFR179.320)

There is a 2" pipe (open on both ends and unused) that penetrates the collsion bulkhead and continues into the fwd hold compartment. Properly blank off or remove pipe and ensure that collision bulkhead watertight integrity is maintained. (46CFR179.320)

There is a 1" pipe located in the stbd overhead of the fuel tank space and the engine room that has multiple soft patches throughout its length. Vessel representative believed that the pipe is associated with the potable water system and possibly no longer used. Make permanent repairs to pipe, replace, or remove. (46 182.700-.730)

Provide adequate and up-to-date navigation charts for operational area. (46CFR184.420)

Noted multiple portable fire extinguishers that were hanging from "J-Hooks" only without proper supporting brackets as provided by the manufacturer. Provide proper mounting brackets so as to prevent unnecessary movement. (46CFR181.520)

The portable CO2 fire extinguisher located on the port-side of the wheelhouse has a cracked/damaged sheathing on the discharge hose. Replace discharge hose or replace extinguisher. (46CFR176.810)

The upper deck is not marked for maximum number of passengers per the stability letter. Properly mark passenger count allowed on upper deck. (46CFR185.602(g))

The watertight door in the trunk leading from the main deck to the engine room does not operate or seal properly due to mis-alignment, poor condition of the mechanical components, and condition of the gasket. Demontrate operability and watertight integrity of the door upon completion of repairs and alignment. (46CFR171.113)

All 10 lifefloats are linked to a single painter attachment. 46CFR180.137(e) requires that the combined weight of each group of lifefloats (attached to a single painter) must not exceed 400 lbs. Re-configure lifefloat painters so that this weight restriction is not exceeded. Reference above cite for full details of painter installation procedure.

Majority of lifefloats are in poor condition due to deterioration of the outer coverings and attached components. Provide written plan of action to address servicability through authorized repair methods and materials. Plan should include a timeline to address repairs to outer skin covering, restoration of color, and replacement of deteriorated attachments.(46CFR180.15)
