A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel; following survey after stbd side propulsive machinery & hydraulic system failures, system has been locked out, pending machining and installation of replacement parts. Vessel is permitted 60 days of continued operation with the current propulsive capabilities. During this time, IAW D9-WI-DOM-(004)(05), in addition to the letters submitted by the master and C/E attesting to vessel's suitable maneuverability, vessel shall maintain (1) a fully functional bow thruster; and (2) a minimum of two operational generator. 46 CFR 91.30-1

No repairs or alterations affecting the safety of the vessel with regard to machinery shall be made without the knowledge of the Officer in Charge, Marine Inspection; submit a repair plan - inclusive of intended dates and repair location - to MSU Duluth and, if a different unit, the cognizant OCMI in whose zone the repairs to the stbd propulsive machinery and hydraulic system will be completed. 46 CFR 91.45-1

Vessel reported voluntary shutdown of SSDG prime mover due to an unknown mechanical issue (based upon abnormal noise coming from engine). Vessel is authorized to proceed from Superior, WI directly to Burns Harbor, IN where technicians are scheduled to attend for troubleshooting and repair. Repair, and subsequent satisfactory testing, must be completed to the satisfaction of the local OCMI (prior to departure from the Port of Burns Harbor, IN).

Temporary emergency load emergency lighting shall provide enough lights to allow passengers and crew to find their way to open deck. Two e-lights in the cargo tunnel are not operating. 46 CFR 112.15-1

Each vessel must carry not less than 12 rocket parachute flares. Crew identified need for new flares and ordered new ones; however, they have not been received. 46 CFR 199.60(c)

Machinery guards or covers are required for all rotating machinery capable of injuring personnel. Stbd inboard air compressor is missing guard or cover over its rotating belt. Do not operate machinery until guard or cover is installed. 46 CFR 58.01-20

Periodic safety tests must demonstrate proper operation of system and procedure to be followed and the expected result. The overspeed and high temp PSTPs for the emergency generator do not reflect onboard testing procedures. Update PSTP to reflect onboard configuration and submit to CG Marine Safety Center for approval. 46 CFR61.40-6

Emergency instructions must include information on the essential actions they must take in an emergency. Instructions do not state how crew will disembark the vessel. 46 CFR 199.80(c)(3)

Clad welding was found on #3 cargo hold forward bulkhead approximately 12" area up approximately 20' on the port side and numerous clad welded areas under vertical pipe on the stbd side. Vessel agreed to remove all clad welding on forward bulkhead #3 cargo hold by crop and renew prior to 2027 operating season. 46 CFR 91.40(3)(b)

Vessels operating in freshwater must conduct an internal structural exam every 5 years. Bow thruster room was not made available for inspection. Make bow thruster available for inspection prior to 01APR2026. 46 CFR 91.40(3)(b)

Protection of passengers, crew, and other persons and the vessel from electrical hazards. Circuit board in navigation light panel is not secured. Electrical panel 104 in wheelhouse has outdated labels and obsolete fuses. 46 CFR 111.01-1(a)(2)

The integrity of a stairtower shall be maintained by "A" class dorrs. The accomadation deck door won't shut properly. 46 CFR 92.07-10

In addition to the emergency power sources required under part 112 of this section, each self-propelled vessel must have at least two electric generating sources. The vessel reported a loss of power to the ship service generator. The vessel is equipped with four installed shaft generators capable of carrying the electrical load; however, their operation prevents the engine from being used as a source of propulsion. The vessel is required to complete satisfactory repairs to the ship service generator to restore it to normal operational status. Repairs to the ship service generator shall be to the satisfaction of attending ABS surveyor or Coast Guard Marine Inspector. 46 CFR 111.10-3

The COD on board was expired by 3 months. Provide valid COD prior to engaging in coastwise trade.

The vessel was not able to retrieve the rescue boat after it was launched and had to manual recover the vessel with the hand crank. Must demonstrate proper launch and recovery of rescue boat prior to departure.

Watertight door indicator on bridge was inoperable for STBD watertight door from engine room to tunnel. Door labeled "stbd bulkhead 129" on bridge panel.

The vessel's port rudder made contact with the dock when approaching FBS for winter layup. Provide evidence of proper alignment to the satisfaction of attending USCG MI prior to departure.

Emergency lighting was identified inoperable in the following spaces: port and stbd aft house stairwells, embarkation light near aft stbd liferaft.

Watertight door indicator on the bridge for port aft door from engine room to tunnel was inoperable. Crew was able to rectify prior to USCG departure.

1 fire hose was found to have a leak during testing and needs to be replaced. Vessel has required number of hoses on board per COI to operate.

STBD rudder actuator was leaking excessive hydraulic fluid out of the top and bottom seals.

Pressure vessel for the seal belt had the relief valve installed in the air line as opposed to the vessel (as required).

All valves with reach rods in the forward machinery space were seized and not working.

A significant amount of improper/damaged cable penetrations in class A structural fire boundaries and watertight bulkheads were found. Areas include machinery spaces, unloading tunnel, and elsewhere. Cable penetrations in class A boundaries shall be of an approved type and meet class A standards. Installations in watertight bulkheads must also be watertight. Many of the multi-cable transits have had the MCT blocks or insulation systems damaged or improperly modified. Vessel is to be fully surveyed for these issues. A list of issues identified by the inspector was provided to vessel. All cable penetrations are to be corrected with approved materials IAW manufactures instructions. Penetration systems must meet testing in FTP Code under approval series 164.138, or ASTM E-814. 60 days

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel reported a leak from the #8 STB ballast tank to the #8 STB void space. Vessel suspects the leak is on a weld seam between the spaces. Submit a repair proposal and complete satisfactory repair prior to departing the port of Duluth / Superior. 46CFR91.45-1(a)

During periodic inspection of the vessel the installed rescue boat was found to have several areas of suspect hull plating. During inspection the marine inspector pressed a finger against the bottom hull on a suspect area and pushed all the way through the hull. Further inspection with pressure applied to the hull revealed systemic wastage resulting in holes down both the port and stbd sides approximately 12” from the keel. The rescue boat was found to be substandard and no longer fit for service in current condition. The cause of the failures in the hull are likely the result of substandard weekly and monthly shipboard lifesaving maintenance checks coupled with the boats significant age and design.

Throughout machinery spaces and other spaces electrical cables were found dead ended, electrical boxes were open with wires hanging out, and cable packings were improperly installed or damaged. A list of what the inspector found was provided to the vessel. The vessel should be further surveyed to identify and correct additional issues as found. Dead ended cables must be removed or their conductors capped, taped, and then placed in proper junction boxes. Open junction boxes or electrical enclosures on equipment with wires hanging out should have their wires properly capped where missing, taped, and the boxes or enclosures properly closed and secured.

01-13108-The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with Sub-chapter F. The Port Side Controllable Pitch Propeller hydraulic line was leaking causing the vessel to secure the Port Engine temperately to make repairs. A new hydraulic line was installed and Master and Chief Engineer attested to proper operation after testing. Vessel to submit a CG-2692 to [email withheld] - 10C

The ship's service generator has experienced a fault and the unit has been sent off for refurbishment. The vessel does not have a replacement in kind available. While the vessel's primary service generator is off for servicing the vessel may continue operations until the due date of this 835 so long as the conditions outlined in 46 CFR 111.10-4 are met with the existing 4 shaft generators that are on board. Once the service generator has been repaired it is to be reinstalled and inspected and tested to the satisfaction of an attending Coast Guard Marine Inspector.

Vessel Self reported failure of the Ships Service Diesel Generator. Vessel is currently underway from Burns Harbor, IN to Sturgeon Bay, WI utilizing one of its Main propulsion engine shaft generators for ships power. The Upon arrival to Sturgeon Bay, WI the SSDG will be replaced with in-kind genset. Conduct operational test of the newly installed generator to the satisfaction of the local OCMI.

All piping, valves, and fittings shall meet the applicable requirements of subchapter F of this chapter. A leak was identified on the fire main piping in way of cargo hold #1. Affect permanent repairs prior to departure at next port to the satisfaction of the attending marine inspector or class surveyor.

Steering system lost enough hydraulic oil to not operate properly. Prove proper operation of the vessels steering system to the satisfaction of the RCO.

Port IB MDE was running hot due to a reduction in cooling water volume. Prove proper operation of the cooling system to the RCO.

Unable to witness crew fire and abandon ship drills as vessel in laid up status, limited crew onboard during CG MI's attendance. Prior to vessels departure from Sturgeon Bay, vessel required to conduct crew fire and abandon ship drills in presence of a CG MI as required by 46 CFR 91.27-1.

Unable to witness lowering, release, operation and retrieval of the vessels life/rescue boat, vessel in laid up status, limited crew onboard during CG MI's attendance. Prior to vessels departure from Sturgeon Bay, vessel required to lower, release, operate, and retrieve the life/Rescue boat in presence of a CG MI as required by 46 CFR 199.45.

Unable to verify all fire fighting hoses are in good serviceable condition, vessel in laid up status limited crew onboard during CG MI's attendance. Prior to vessels departure from Sturgeon Bay, vessel crew required to charge and demonstrate all fire fighting hoses are in good serviceable condition in presence of a CG MI as required by 46 CFR 95.10-10

Make repairs to correct three damaged/fractured stiffeners on cargo hatch cover #11 and damaged on #7 cargo hatch cover aft port side coaming. Permanent repairs to be completed prior to vessels departure from Sturgeon Bay, to satisfaction of CG MI or ABS Class Surveyor as required by 46 CFR 92.01-10.

All systems of a marine engineering nature shall be in accordance with the requirments of sub-chapter F. The vessels sartboard side remote steering station is not properly transfering command back to the main steering station. Make repairs an test to the satisfaction of class prior to departing Burns Harbor, IN.

Starboard aft steering pump temporary loss of electrical power.

Stbd rudder advances to full right rudder upon shifting to the center console control on bridge Navtronics steering installation. Center console station shall not be used until resolved.

During transit the ships service generator failed causing the vessel to switch from main service generator to generator service off one of the propulsion plants. Vessel crew identified blown fuse. After replacement Captain and C/E attested to proper operation of service generator.

Helm station follow-up mode command indicator and rudder response do not match. Prove helm follow-up mode command and wheel posion accurately relay rudder command with rudder response.

#1 port inboard main engine is non operable. Repair engine to the satisfaction of class and the cognizant Officer in Charge, Marine Inspections.

SCBA bottles have overdue hydrostatic test. Provide SCBA bottles with current hydrostatic test.

Unable to demonstrate launching of rescue boat due to vessel moored starboard side to another vessel. In the presence of a CG MI, lower, release, operate, retrieve, and stow vessels rescue boat.

General Alarm placard was missing on starboard side near engine room water tight door in tunnel. Each general alarm must be marked "General Alarm".

Emergency lights were not properly marked. All emergency lights shall be marked with a letter "E" at least 1/2 inch high.

Repair service generator shaft bearing. When completed and tested send C/E report to this office.

Make proper repairs to leak between stareboard #2 ballast tank and adjacent void prior to departing Burns Harbor with cargo. Repairs completed to the satisfaction of an attending class surveyor or attending marine inspector. During transit from Superior to Burns Harbor in ballast, the affected void shall be monitored during each watch. Contact the OCMI should flooding increase.

Indicator light is not available on the bridge to warn when door is open for Class I QAWTD watertight door located in the cargo unloading tunnel stbd side door #4. WTD indicator panel is inadequately labeled to determine which light is for which door. Provide indicator lights for all watertight doors and properly label bridge WTD indicator panel to correspond with the door location and position.

Repair crack in #2 Ballast Tank to the satisfaction of ABS and attending Marine Inspector.

During winter lay up and prior to fit out 2017 Effect permanent repairs to the bulkhead between the port No. 3 and No. 4 bottom were it intersects cargo hold 2, and also adjacent plate in cargo hold 2 frame 54. Repairs shall consist of cropping and renewing fatigued plate and associated welds to good metal. 46 CFR 91.45-1 and 91.45-5

Frame deflection and minor hull inset was discovered on internal frames 5, 6, 8, 9, and 10 between the 1st and 2nd longitudinal members of the #1 port ballast tank. The damage is required to be repaired by the conclusion of the next drydock period or no later than 31Mar2017 to the satisfaction of the cognizant Officer in Charge, Marine Inspection.

Vessel is allowed to make a one time transit to Burns Harbor, IN to effect proper repairs to the leak in the #2 Port Ballast Tank to the satisfaction of the attending USCG Marine Inspector and Class Surveyor prior to departure from port.

Port PMC controls for CPP system failed to return beyond 50% forward position. Make proper repairs to CPP system and test at all control stations to the satisfaction of attending MI.

Make proper repairs to damaged firemain in 1 port ballast tank and conduct hydrostatic test to 1.5 time MAWP, IAW 46 CFR 56.97-40(a)(7).

Make permanent, the repair to bulkhead between #2 starboard ballast tank and adjacent void at frame 39, outboard. Temporary repair completed on 05MAY16. Repair and approval to be to the satisfaction of attending MI and/or ABS surveyor and completed prior to departing from winter lay up.

The vsl currently has two severe fractures in the fire main sustem on the stbd side of the vsl. Vsl is tomake permanent repairs to the fire main system and conduct operational testing prior to departure from the port. Repairs are to be conducted to the satisfaction of the USCG.

Make proper repairs to the hydraulic cargo hatch dogging system and prove proper operation/watertight integrity of #7 hatch prior to departure.

Reinspect and repair as necessary, 9 inch repair of crack: #3 port ballast tank to void, frame #52, to the satisfaction of the attending coast guard marine inspector. To be conducted prior to 31 Mar 2001 drydock inspection.

The Certificate of Inspection (COI) has been surrendered and submitted to MSD Sturgeon Bay while the vessel is in a layup status. Prior to returning to commercial service, conduct a satisfactory Coast Guard inspection iaw 46 CFR 91.25 or 46 CFR 91.27 to reactivate the COI.

Prove proper operation of main engine alarm panel prior to departure from Duluth, Minnesota as per 46 CFR 62.50-20.

All main engine exhaust risers (from manifold to turbo) and turbos missing insulation as originally equiped. Additionally all main engine bellows (between manifold segments) missing shielding. Provide insulation/shielding IAW ABS Rules by 08/03/08.

Install new emergency lights or demonstrate tunnel is arranted with enough emergency lights in accordance with 46 CFR 112.15-5(a) or 112.15-1.

Install and prove operation of a general alarm bell in the engineer's office, second deck, port side-aft, in accordance with 46 CFR 113.25-9.

Make all five main diesel engine pyrometers operational per vessels approved periodic safety perocedures and 46 CFR 61.40-6.

PROVIDE AND INSTALL OIL STRAINERS IN INCINERATOR FUEL SUPPLY PIPING IN ACCORDANCE WITH 46 CFR 63.10-70
