Many of the multi-cable transits have had the MCT blocks or insulation systems damaged or improperly modified. Vessel is to be fully surveyed for these issues. During periodic inspection of the vessel the installed rescue boat was found to have several areas of suspect hull plating. During inspection the marine inspector pressed a finger against the bottom hull on a suspect area and pushed all the way through the hull. Further inspection with pressure applied to the hull revealed systemic wastage resulting in holes down both the port and stbd sides approximately 12” from the keel. The rescue boat was found to be substandard and no longer fit for service in current condition. The cause of the failures in the hull are likely the result of substandard weekly and monthly shipboard lifesaving maintenance checks coupled with the boats significant age and design.
