Each vessel that relies on electricity to power the navigational equipment must be arranged so that the equipment can be energized from at least two sources of electricity. Vessel's electronic charts only had one source of electricity. 46 CFR 129.310(a)(1)(iv)

Fire main piping must be of a suitable corrosion-resistant material. Fire main piping outside the wheelhouse must be replaced due to the level of corrosion. 46 CFR 132.110(a)(3)

The operator must secure each marine sanitation device in a manner which prevents discharge of treated or untreated sewage. Vessel's current configuration does not have an acceptable method of securing the device. 33 CFR 159.7

Each OSV must have, at the pilothouse, controls to monitor the pressure in the fire main. Vessel does not have a pressure gauge for the fire pump in the wheelhouse. 46 CFR 130.430

Each liftboat must have aboard an operating manual approved by the Coast Guard as complying with this section. Vessel's level indicator (tilt) alarm's visual alarm for 1.5 and 2.5 degrees was inoperable. 46 CFR 134.170(a)

Paint lockers and similar compartments must be constructed of steel or be wholly lined with steel. Paint locker was constructed of aluminum. Next dry dock inspection. 46 CFR 127.220(c)

A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Medical certificates must be issued and will remain current for the following periods of time, unless otherwise noted on the certificate. Vessel was employing an individual who did not possess a valid medical certificate.

No vessel may be navigated unless it has in its service and onboard the crew complement required by the COI. Vessel had employed in its service an individual who did not possess a valid credential and therefore the vessel did not meet their minimum manning requirement as per the COI.

Electrical equipment on a vessel must be designed, installed, and maintained to protect crew members, offshore workers, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment. The starboard generator overspeed alarm and shutdown did not function as required. This is a repetitive occurrence which has been witnessed by the Coast Guard for two years in a row. Vessel to conduct trouble shooting and complete satisfactory test in the presence of USCG Inspectors.

Any discharge of oil or oily mixtures into the sea from a ship other than oil tanker or from machinery space bilges of an oil tanker is prohibited except: 4) ship has in operation oily-water separating equipment, a bilge monitor, bilge alarm, or combination thereof as required by part 115 subpart b of this chapter and 5) oily-water separating equipment is equipped with a 15 ppm bilge alarm; for US inspected ships. Vessel had oily-water in bilge area with an electric bilge pump with float switch that is directly connected to an overboard discharge. The 480 Volt electrical panel in the starboard deck locker was missing protections against electrical hazards including fire and electrical shock. Vessel to repair or replace missing part to eliminate aforementioned risk.

Each portable fire extinguisher must be inspected and maintained to ensure that they are in good general condition. The hose attached to extinguishers at station 9 and 11 showed signs of deterioration.

Nothing in this part shall be construed as limiting the inspector from making such tests or inspections as he or she deems necessary to be assured of the safety and seaworthiness of the vessel and the inspection procedures for marine-engineering systems contained in subchapter F of this chapter apply to the inspection of vessels regulated under subchapter L of 46 CFR. The fuel hose running to the port generator was in poor condition and in need of replacement.

Each alarm required by 46 CFR 130.460 must be of the self-monitoring type that will both show visibly and sound audibly. The alarm panel on the bridge and one of the two alarm panels in the engine room did not display all of the required visual indicators.

The owner of a vessel shall ensure that such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key. Vessel was not equipped with a master key control system. 46 USC 3106(a)

Each crew berthing area shall be equipped with information on the procedures and resources to report crimes, including sexual assault and sexual harassment. Reporting procedures and resources for sexual assault and sexual harassment were not posted in crew berthing. 46 USC 11101(5)(B)

The owner or operator of a non-tank vessel must review the vessel response plan (VRP) annually. This review must occur within 1 month of the anniversary date of Coast Guard approval of the VRP. Vessel did not submit evidence of review to the USCG. 33 CFR 155.5070(a)

An owner or operator of a ship to which this subpart applies must review the shipboard oil pollution emergency plan annually and submit a letter to Commandant (CG-MER) certifying that the review has been completed. This review must occur within 1 month of the anniversary date of Coast Guard approval of the plan. Vessel did not submit evidence of review to the USCG.

Each vessel assigned a loadline must have loadline markings and deck-line markings permanently scribed or embossed as required by subchapter E of this chapter. The vessel’s deckline was found to be painted the same color as the hull. Vessel crew painted the deckline white, contrasting the dark hull. Deficiency rectified.

Electrical equipment on a vessel must be designed, installed, and maintained to protect crew members, offshore workers, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment. The windings on both generators are caked with dust and dirt, presenting a fire hazard. The shrouds on both generators are damaged presenting a personnel hazard. Replace/repair shrouds and clean the generator windings. 46CFR129.200(b)

Each OSV must carry at least one rescue boat and must be approved under approval series 160.156. Vessel’s crew experienced difficulties starting the engine. Conduct assessment and complete needed repairs. 46CFR133.135(a)

Each OSV must carry at least one rescue boat. Propeller on outboard engine displayed impact damage resulting in a missing portion and crack on the flukes. Renew/repair propeller. 46CFR133.134(a)

Each OSV must carry lifebuoys approved under approval series 160.150 or 160.050. Retroreflective tape on numerous ring buoys was found improperly installed. Renew type I retroreflective tape. 46CFR133.70(a)

Each lifejacket must be marked with type I retro-reflective material approved under approval series 164.018.Retroreflective tape on numerous lifejackets was found wasted. Renew type I retroreflective tape. 46CFR133.70(b)(3)(ii)

Each vessel must be designed and constructed to minimize fire hazards, as far as reasonable and practicable. MI observed that the bulkhead between the deck locker and the laundry room / upper engine room was not gas tight. 46CFR127.220(a)

Drug testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in the Serious Marine Incident (SMI). The collection of drug-test specimens of each individual must be conducted within 32 hours of when the SMI occurred. MI observed that the vessel did not have means onboard to conduct drug testing within 32 hours from an SMI. 46CFR4.06-3

The operator of a vessel with a capacity of 250 or more barrels of oil, hazardous material, or liquefied gas as regulated in Table 4 of 46 CFR part 154 shall provide transfer procedures that meet the requirements of this part and part 156 of this chapter for transferring—(a) To or from the vessel; and (b) From tank to tank within the vessel. MI observed that the total tank capacity of the vessel was greater that 250 barrels yet they did not have oil transfer procedures onboard. 33CFR155.720

Each vessel must be designed and constructed to minimize fire hazards, as far as reasonable and practicable. MI observed significant grease build up in the galley exhaust hood. 46CFR127.220(a)

Each lighting fixture in an electrical system operating at more than 50 volts must comply with UL 595. There are cracks on the light cover in the galley. Replace light cover. 46 CFR 129.410(d)

AIS must be maintained in effective operating condition which includes the accurate broadcast of a properly assigned MMSI number. The vessel does not have their current MMSI number programed into their AIS or VHF radio. Update the MMSI number in the AIS and VHF to match the current MMSI number for vessel. 33CFR164.46(d)(2)

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel under way. MI observed that the refrigerators, washers, and dryers were not secured. 46CFR129.220(a)

At least one lifebuoy on each side of the OSV fitted with a buoyant lifeline that is resistant to deterioration from ultraviolent light. Buoyant lifeline on second deck STBD ring buoy was wasted. Renew lifeline. 46cfr133.70(a)4(i)(E)

No person may operate any vessel equipped with installed toilet facilities unless it is equipped with an operable Type II device. The MSD aerator is inoperable and the vessel owner is utilizing the wrong chlorine tablets, minimizing the MSDs functionality. Repair aerator and urchase/utilize approved tablets per manufacturers manual. 33CFR159.7(a)(1)

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel under way. The electrical connections of all of the engineering alarm panels are loose. Tighten the electrical connections. 46CFR129.220(a)

If your vessel's COI is renewed less often than annually, the MI must check the setting of the safety or relief valves on all pressure vessels, twice every 5 years; relief device was missing servicing information.

Each box for a small battery installation must have openings near the top to allow escape of gas. If the installaiton is in a non-environmentally controlled location, the installation must prevent the ingress of water; battery box for the the port generator had water intrusion.

In addition to the design requirements of paragraph (a), each crane must have the following: Instruments with built-in lighting; boom pressure gauge was not readable.

Anchor chain was seized together in anchor box not allowing for proper deployment. Anchor chain to be pulled from deck storage box and exercised. Pelican hook holding anchor to vessel to also be replaced. 46 CFR 130.240

Vessel equippped with fixed pitched propellers must have on the nav bridge a propeller speed and direction indicator for each shaft. Starboard shaft tachometer was inoperable. 46 CFR 113.37-5.

Draft marks worn on bow. Each vessel must have the drafts of the vessel plainly and legibly marked upon the stern. 46 CFR 131.720

The lifelines on the starboard side lifefloat were deteriorated and un-serviceable.
