The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the Company. The technical and/or operational nature of deficiencies 02-08 provide objective evidence of a serious failure of the implementation of the ISM Code. An external audit report satisfactory to the OCMI must be submitted prior to clearance of this deficiency. 74 SOLAS (20)IX/3.1 ISM CODE 10.1

PSCO observed the remote control release cable and brake for the port side lifeboat improperly installed and seized in place by paint preventing the brake from stopping the lifeboat during lowering evolution. 74 SOLAS (97) III/19.2

1 The fire resistance of doors shall, as far as practicable be equivalent to that of the division in which they are fitted. Doors and door frames in "A" class divisions shall be constructed of steel. Doors in "B" class divisions shall be non-combustible. Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight and self-closing. 2 Doors required to be self-closing shall not be fitted with holdback hooks. However, hold-back arrangements fitted with remote release devices of the fail-safe type may be utilized. PSCO observed galley doors, fitted with remote releases, held open with hold back hooks. Additionally, PSCO observed both water tight doors to engine room fitted with hold back hooks. Finally, PSCO observed self-closing doors within main stairwell to engine room, upper deck, C-deck PD/CD, nav bridge PD/CD, and engine room workshop escape trunk unable to close properly. 74 SOLAS (97) II-2/47.1 & 2

All the steering gear components and the rudder stock shall be of sound and reliable construction to the satisfaction of the Administration. PSCO observed active hydraulic oil leaks from all 4 hydraulic rams, hydraulic oil tank, and #1 servo pump. Crew fabricated drip pans to contain leaks. 74 SOLAS (97) II-1/29.2.1

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, PSCO observed active oil leaks and accumulating oil around M/E circulating pump, F/O booster pump, fuel oil purifiers, lube oil pumps, aux engine fuel tank, auxiliary engine #2, and all cylinder heads and fuel pumps on the main engine. 74 SOLAS (04) II-2/26.1

Each lifeboat shall be launched with its assigned operating crew aboard and maneuvered in the water at least once every three months, As far as is reasonable and practicable, rescue boats other than lifeboats which are also rescue boats shall be launched each month with their assigned crew aboard and maneuvered in the water. In all cases this requirement shall be complied with at least once every three months. PSCO observed both lifeboats and standalone rescue boat were last lowered and maneuvered in the water in April. 74 SOLAS (97) III/18.3.6 & 7

The rating or appropriate setting of the overload protective device for each circuit shall be permanently indicated at the location of the protective device. PSCO observed circuit breaker setting for multiple breakers on main switchboard in engine control room missing/illegible. 74 SOLAS (97)II-1/45.6.2

The master, owner, operator, agent, or person in charge of any vessel equipped with ballast water tanks that operates in the waters of the United States must follow these practices: (g) Maintain a ballast water management (BWM) plan that has been developed specifically for the vessel and that will allow those responsible for the plan's implementation to understand and follow the vessel's BWM strategy and comply with the requirements of this subpart. The plan must include— (6) Detailed reporting requirements and procedures for ports and places in the United States where the vessel may visit. The vessel's Ballast Management Plan does not mention US ballast requirements. 33 CFR 151.2050(g)(6)

No person may cause a vessel to enter into or get underway on the navigable waters of the United States unless no more than 12 hours before entering or getting underway, the following equipment has been tested:(1) Primary and secondary steering gear. The test procedure includes a visual inspection of the steering gear. PSCO observed records of pre-arrival checks omitting oil leaks and fabricated drip pans on steering gear. 33 CFR 164.25(a)(1)

Provision shall be made to facilitate cleaning, inspection, and maintenance of main propulsion and auxiliary machinery. PSCO observed significant fuel oil accumulation and leaks found on three of the injectors on the main engine. Additionally fuel oil leaks were identified on ship's generators- to include lubricated oil leaks and pooling beneath generators. 74 SOLAS (04) II-1/26.7

Any required fire detection and fire alarm system shall be capable of immediate operation at all times. PSCO observed failed test on incinerator smoke detector. Additionally observed fault code for fire alarm system, indicating that detector is inoperable. 74 SOLAS (97) II-2/12.1.1

For determining the appropriate fire integrity standards to be applied to division between adjacent spaces, such spaces are classified according to their fire risk as shown in Categories (I) to (II). Table 44.1 requires an A-60 boundary between a machinery space of Category A and a space for the stowage of flammable liquids. PSCO observed bunker samples being stored within the engine room with no boundary between them. 74 SOLAS (97) II-2/44.2.2

Fire extinguishing appliances shall be kept in good order and be available for immediate use at all times. PSCO observed extinguisher located in air conditioning room missing October and November 2023 inspections. 74 SOLAS (97) II-2/21

Oil filtering equipment referred to in Paragraph 2 of this regulation shall comply with Paragraph 6 of this regulation. PSCO observed oil content meter not intact. MARPOL I/14.7
