Fire extinguishing equipment installed on a vessel in excess of the requirements of §§ 181.400 and 181.500 must be designed, constructed, installed, and maintained in accordance with a recognized industry standard acceptable to the Commandant (CG-ENG-4). Vessel has installed Engine room heat detector in addition to an interconnected fire detection system. Heat detector was unable to produce audible alarm and operating station. Vessel is to demonstrate proper operation of Heat detector within 14 days to satisfaction of attending Marine Inspector. Until system is installed and proved working vessel is to test interconnected fire detection system from inside engine space prior to getting underway and conduct hourly rounds of the engine room.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The starboard propulsion shaft is broken where it connects to the reduction gear. The propulsion shaft must be renewed with a propulsion shaft suitable for the intended service of the vessel and acceptable to the cognizant Officer in Charge Marine Inspection.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. The repairs to the starboard propulsion shaft must be inspected and approved by the cognizant Officer in Charge Marine Inspection.

Vessel's current MMSI# does not match MMSI# registered with NOAA for EPIRB. It is mandatory that each EPIRB be registered with NOAA before installation and that information be kept up-to-date. Update MMSI# on EPIRB registration with NOAA so it matches FCC Station License. 47CFR80.1061(f)

Vessel master was unable to provide medical certificate. A mariner must provide evidence of meeting the medical and physical standards. 46CFR10.302

FCC Station License is expired. Stations in the maritime service must be licensed by the FCC either individually or by fleet. Acquire new FCC Station License. 47CFR80.13

COD expired; vessel is awaiting replacement from NVDC. The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Once received provide proof of COD on board vessel. 46CFR67.313

Rotating machinery in engine compartment do not have guards. An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Install proper protection over rotating machinery in engine compartment.

Vessel's current MMSI# does not match MMSI# programmed in VHF-DSC radio. The vessel must be equipped with a VHF-DSC radiotelephone installation meeting requirements of 47CFR80.1101(c)(2). Update MMSI# programmed in VHF-DSC so it matches FCC Station License. 47CFR80.905(a)
