Fire extinguishing equipment installed on a vessel in excess of the requirements of §§ 181.400 and 181.500 must be designed, constructed, installed, and maintained in accordance with a recognized industry standard acceptable to the Commandant (CG-ENG-4). Vessel has installed Engine room heat detector in addition to an interconnected fire detection system. Heat detector was unable to produce audible alarm and operating station. Vessel is to demonstrate proper operation of Heat detector within 14 days to satisfaction of attending Marine Inspector. Until system is installed and proved working vessel is to test interconnected fire detection system from inside engine space prior to getting underway and conduct hourly rounds of the engine room.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The starboard propulsion shaft is broken where it connects to the reduction gear. The propulsion shaft must be renewed with a propulsion shaft suitable for the intended service of the vessel and acceptable to the cognizant Officer in Charge Marine Inspection.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. The repairs to the starboard propulsion shaft must be inspected and approved by the cognizant Officer in Charge Marine Inspection.

Vessel's current MMSI# does not match MMSI# registered with NOAA for EPIRB. It is mandatory that each EPIRB be registered with NOAA before installation and that information be kept up-to-date. Update MMSI# on EPIRB registration with NOAA so it matches FCC Station License. 47CFR80.1061(f)

Vessel master was unable to provide medical certificate. A mariner must provide evidence of meeting the medical and physical standards. 46CFR10.302

FCC Station License is expired. Stations in the maritime service must be licensed by the FCC either individually or by fleet. Acquire new FCC Station License. 47CFR80.13

COD expired; vessel is awaiting replacement from NVDC. The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Once received provide proof of COD on board vessel. 46CFR67.313

Rotating machinery in engine compartment do not have guards. An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Install proper protection over rotating machinery in engine compartment.

Vessel's current MMSI# does not match MMSI# programmed in VHF-DSC radio. The vessel must be equipped with a VHF-DSC radiotelephone installation meeting requirements of 47CFR80.1101(c)(2). Update MMSI# programmed in VHF-DSC so it matches FCC Station License. 47CFR80.905(a)

Renew port engine alternator belts. (torn), prior to the carriage of passengers.

Install proper USCG approved SAE 2006 exhaust hose on 8 inch stbd exhaust piping in the hold compartment, prior to the carriage of passengers.

Provide 3 labeled fire buckets with lanyards, prior to the carriage of passengers.

Install detachable clip with 3-6 ft of floating line on ring buoy light, prior to the carriage of passengers.

Properly plug bulkhead penetrations between the hold and steering compartment, prior to the carriage of passengers.

Renew out of date items and provide eye wash for first aid kit, prior to the carriage of passengers.

Move life ring buoy to more accessible area, prior to the carriage of passengers.

Install covers over starting batteries in engine space to prevent exposure to water. Ref: 46CFR183.350

Renew center engine propulsion shaft hull and strut bearings, prior to carrying passengers. Ref: 46CFR176.600

Install fuel oil shutoff valves on the engine room fuel day tank as required by 46CFR182.455. Fuel shut-off valves are required on all fuel tanks. Reach rods (with flame shields) are required to shut off fuel, valves are located more than 12" into machinery space. See 46CFR182.455(b)(4).

Provide plans to make all hatches on the weather deck on vessel (that are lower than 12" in height) watertight. The current engine room hatch covers are between 8" & 10" in height and are not watertight. If other weather deck hatches have a combing >/= to 12" in height the hatches are only required to be weathertight, to the satisfaction of the attending CG Marine Inspector. Ref: 46CFR171.124

Provide a new magnetic compass deviation table.

Seal MDE exhaust piping bulkhead penetrations IWO hold and rudder compartment.

Repair broken F/O cutout valve for F/O tank in rudder compartment.

Provide valid Certificate of Documentation which reflects the correct "Registered length" of the vessel as defined in 46 CFR 69.53

Renew wasted reach rod for emergency shut off valve to diesel supply tank for stove.

MAKE PERMANENT REPAIRS TO THE PITTED TRAILING EDGE OF THE PORT RUDDER.

PROVIDE A RETAINING LINE FOR THE RUDDER COMPARTMENT HATCH COVER.

REPLACE BROKEN STRAINER FOR BILGE SUCTION LINE IN THE LAZARETTE.

SUBMIT REPAIR PROPOSAL FOR PORT BOW COLLISION DAMAGE. MAKE VESSEL AVAILABLE FOR INSPECTION AFTER ALL DAMAGE HAS BEEN CROPPED OUT, AND PRIOR TO COMMENCING FIBERGLASS AND STRUCTURAL WORK. REPAIR PROPOSAL AND ALL REPAIRS SHALL BE COMPLETED TO THE SATISFACTION OF THE OCMI.

Provide a check valve for each bilge suction line, or stop-check valves at the manifold.

REPLACE RETRO-TAPE FOR LIFEFLOATS, TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

PAINT, STENCIL AND REPLACE LINES ON AFT 15 MAN LIFE FLOAT TO MAKE AS ORIGINAL.

PROVE WATERTIGHT INTEGRITY VIA HOSE TEST OF COMBING SURROUNDING ENGINE ROOMACCESS.

PROVIDE SECURING LATCH FOR ENGINE ROOM ACCESS HATCH.

COMPLETE SATISFACTORY TEST OF THE MAIN ENGINES AND BILGE PUMPS AND SUBMIT PROVIDED STATEMENT TO [name withheld].

LINE THE BATTERY BOX WITH LEAD, FRP OR OTHER SUITABLE MATERIAL RESISTANT TOTHE ELECTROLITE.

MAKE VESSEL AVAILABLE FOR TESTING OF THE STEERING GEAR.

PROVIDE DOUBLE CLAMPS FOR THE WASHDOWN PUMP THROUGH-HULL FITTING IN THE FISHHOLD, AND THE THROUGH-HULL FITTING FOR THE 4-71 IN THE ENGINEROOM.

REMOVE THE WET ROTTED WOOD AT THE AFT END OF THE WHEELHOUSE ON THE TOP SIDE.

MAKE ENGINEROOM AND FISHHOLD HATCHES WATERTIGHT.

PROVIDE RETAINING DEVICES FOR THE ENGINEROOM AND FISHHOLD HATCHES.

PROVIDE MEANS OF RETENTION FOR THE ENGINEROOM AND FISHHOLD HATCHES.

PROVIDE A CURRENT EPIRB BATTERY AND CONDUCT OPERATIONAL TEST OF THE SAME. UNTIL THIS IS SATISFACTORILY COMPLETED, NO OCEAN VOYAGES MAY BE TAKEN, ONLYCOASTWISE VOYAGES, NOT MORE THAN TWENTY (20) MILES FROM A HARBOR OF SAFE REFUGE.

MAKE PERMANENT REPAIRS TO 15-MAN LIFEFLOAT AT AN APPROVED FACILITY. UNTIL THIS IS SATISFACTORILY COMPLETED, NO MORE THAN THIRTY (30) PERSONS MAY BE CARRIED ON A COASTWISE TRIP, NOT MORE THAN TWENTY (20) MILES FROM A HARBOR OF SAFE REFUGE.

MAKE PERMANENT REPAIRS TO THE OPENING IN THE COLLISION BULKHEAD, IN ACCORDANCE WITH CURRENT REGULATIONS.

PROVIDE THREE (3) ORANGE DAY FLARES.

ATTACH LAZARETTE HATCH TO VESSEL.

PROVIDE CURRENT CERTIFICATE OF DOCUMENTATION.

MAKE VESSEL AVAILABLE FOR EXAMINATION OF NEW GANGWAY.
