All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Marine inspector observed inoperable stern light. Vessel is restricted to daylight operations. Repair light within two weeks. 46 CFR 183.420

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI identified multiple dead end wires under the operating station. Rectify within 7 days. 46 CFR 183.200 - 705C

A power-driven vessel underway shall exhibit: A sternlight. The sternlight did not have sufficient illumination. Restricted to daylight hours until new light is installed. 33 CFR 83.23 (a)(iv) - 705C

Each flare carried to meet this section must be stowed in A portable watertight container marked as required by § 185.614 of this chapter, carried at the operating station. MI observed that the container latch was broken. Rectify within 3 days. 46 CFR 180.68 (e) - 705 C

An adult life jacket must be provided for each person carried on board a vessel. MI identified 05 life jackets that were no longer serviceable. Owner replaced with spares. 46 CFR 180.71 (a) - 10C

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. MI observed several life jackets and the ring buoy markings were faded or missing. All were marked by the owner. 46 CFR 185.604 (b) - 10C

An adult life jacket must be provided for each person carried on board a vessel. (b) In addition, a number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided, or such greater number as necessary to provide a life jacket for each person being carried that is smaller than the lower size limit of the adult life jackets provided to meet this section. - 46 CFR 180.71 MI observed 8 unserviceable adult life jackets and 5 unserviceable child life jackets. Vessel passenger count reduced to 36 adult passengers and zero children until new serviceable life jackets are purchased.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry — (1) Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and (2) Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. - 46 CFR 180.68 MI observed 01 orange distress signal was expired. Vessel may not operate with passengers until it has 12 total distress signals on board.

At least one ring life buoy must be fitted with a lifeline. - 46 CFR 180.70 MI observed the ring life buoy's lifeline was severely deteriorated. Lifeline must be replaced prior to the carriage of passengers.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. - 46 CFR 185.604(b) MI observed illegible letters on the ring life buoy.

At each initial and subsequent inspection for certification, the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition and fit for the service for which they are intended. - 46 CFR 176.25-15 MI observed disconnected grounding wire and damaged wiring for the shaft seal bilge pump sump.

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. - 46 CFR 16.401(b) MI observed vessel's supervisory personnel could not produce documentation proving they had received at least 60 minutes of training.

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel was due for renewal COI inspection on 11MAR24 and has yet to receive a renewal COI inspection. Vessel shall not operate with passengers until new COI has been issued. - 46 CFR 176.100

The owner or managing operator shall make a vessel available for drydock examinations and internal structure examinations for certification. Vessel has lapsed the drydock and internal structure exam expiration dates, 31MAY23, on its COI. Allow inspection for satisfactory drydock and internal structure exam credit prior to the carriage of passengers. 46 CFR 176.600

An adult life jacket must be provided for each person carried on board a vessel. During inspection identified (05) life jackets that severely faded almost to a white color. Reduced passenger count to (37) passengers. 46 CFR 180.71(a)

Each ring life buoy with a floating water light must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the water light around the body of the ring lifebuoy. Vessel's lanyard was 60 feet long. 46 CFR 180.70(d)(2)

Except as otherwise provided in this section every vessel must be fitted with a suitable magnetic compass designed for marine use, to be mounted at the primary operating station. Except on a vessel limited to daytime operations, the compass must be illuminated. Vessel's compass was not illuminated. Vessel is limited to daytime operations. 46 CFR 184.402(a)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel's log book was not available to inspect. 46 CFR 185.520(f), 46 CFR 185.728(c)

The operator of each self-propelled vessel 12 meters or more in length shall carry, on board and maintain for ready reference, a copy of these Rules. Vessel did not have a Navigation Rules Book on-board. 33 CFR 83.01(g)

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. Vessel did not have valid COI posted. 46 CFR 176.302

An adult life jacket must be provided for each person carried on board a vessel. 02 adult life jackets were found in poor condition. 46 CFR 180.71(a)

A pre-engineered fixed gas fire extinguishing system must: Be installed in accordance with the manufacturer's instructions. Maintenance pin was attached, preventing the system from discharging once activated. 46 CFR 181.420(a)(4)

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation. Vessel's VHF-DSC radio was found inoperable. Vessel is limited to navigation not more than 1,000ft from the nearest land. 47CFR80.905

Each portable fire extinguisher must be of approved type. 46CFR181.500. 02 fire extinguishers overcharged with guage in the red.

All vessels must have navigation lights that are in compliance with the applicable sections of the international and inland navigation rules. 46CFR183.420 Vessel did not have port running light installed, limited vessel to day time operations.

Beaching has not been authorized on this vessel. Due to self-admission of beaching, haul vessel out for inspection of hull prior to carrying passengers IAW 46 CFR 176.600(d)

A type-1 lifejacket shall be provided for each person on board. Ten lifejackets were condemned. Reduce passenger count to 32 IAW 46 CFR 180.71

Remote fuel shut-offs must be marked in clearly legible letters. Remote fuel shutoff not marked.

PRIOR TO THE CARRIAGE OF PASSENGERS, CONDUCT YOUR 2ND ANNUAL INSPECTION. VESSEL WAS NOT READY FOR INSPECTION. POC THOUGHT IT WAS, BUT WHEN ONBOARD, FOUND SEVERAL DEFICIENCIES BEFORE STARTING INSPECTION. ADVISED POC TO GO THROUGH VESSEL.

Replace all retro-reflective tap on ring life buoy. It's all faded and falling off.

Update all nautical publication. POC had the same box of publication that he showed during his other vessel's inspection. Advised POC that he needed publications for both boats, not just one set.

Properly secure the cable-to-bottle end of the pull cable. After serviceing, POC never hooked the cable up to the extinguisher, so if pulled, the FFFE system wouldn't be activiated.

Lable life preservers with the vessel's name and seperate child life preservers from adult life preservers. All jackets are stowed in the OVHD, but they have been mixed together after the yard. POC needs to orginaize stowage and also remove the old vessel's name off them. Some jackets have two vessel's names showing.

Provide one Type B-II portable fire extinguisher. Missing a bottle.
