When the exhaust cooling water system is separate from the engine cooling water system, a suitable warning device, visual or audible, must be installed at the operating station to indicate any reduction in normal water flow in the exhaust cooling system. MI observed that the sensor used to determine exhaust overheating was disconnected on both engines.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. MI observed an alarm fault indicating water in fuel system at operating station.

A lighting fixture must be installed as follows: Each lighting fixture and lamp holder must be fixed. MI observed loose light fixture overhead and centerline in engine room.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment under all practical positions. MI observed bilge piping mounting bracket on the starboard forward of engine room bulkhead was broken from the weld seam.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including inspection and operation of all watertight closures in the hull, decks, and bulkheads including through hull fittings and sea valves. MI observed water intrusion through a pin hole on PORT rudder post tube. Submit repair plan and make repairs to the satisfaction of attending MI. 835 Issued.

A pre-engineered fixed gas fire extinguishing system must automatically shut down all engines that draw intake air from within the protected space. MI observed generator did not secure when fixed fire extinguishing activated. Make repairs to the satisfaction of attending MI. 835 issued.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. MI observed cracks at the top of the PORT and STBD quarters. Submit repair plan and make repairs to the satisfaction of attending MI. 835 issued.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including inspection and operation of all watertight closures in the hull, decks, and bulkheads. MI observed crack fully penetrating the deck at the aft scuttle. Submit repair plan and make repairs to the satisfaction of attending MI. 835 issued.

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. MI observed engine hatches did not complete vapor barrier. Make repairs to the satisfaction of attending MI.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including inspection of all fuel system. MI observed deteriorated fuel hose on generator. Do not use generator until repairs have been approved by MI. 835 issued.

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturer's instructions. MI observed broken top mounting strap on cylinder. Make repairs to the satisfaction of attending MI. 835 issued.

A lighting fixture must be installed as follows: Each lighting fixture and lampholder must be fixed. MI observed loose light fixture in engine room. Make repairs to the satisfaction of attending MI. 835 issued.

Electrical equipment used in the following locations must be dripproof in a machinery space. While in engine room, observed that bilge pump was open faced. Please provide documentation that electrical equipment is drip proof to the satisfaction of the marine inspector. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. While in engine room, observed electric bilge pump plugged into an outlet within engine room space posing an unsafe electrical condition under flooding conditions. The rules concerning shapes shall be complied with by day. Observed vessel did not have day shapes on board. Please provide evidence of day shapes purchased and on board in compliance with inland navigation rules. 33CFR83.20(d)

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with manufacturer's instruction. After testing of fireboy system both main engines shut down, but the STBD engine would restart without the override system initiated, yet the port would not. Repair system IAW manufacturer's recommendations and to the satisfaction of the attending marine inspector.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. Discovered cracks in the transom on port and STBD corners at deck level that continued into the steering compartment. Make repairs to the satisfaction of the attending marine inspector.
