A means of communication shall be provided between the navigation bridge and the steering gear compartment. PSCO identified the means of communication between the port steering compartment and bridge was unrecognizable. 74SOLAS(04)II-1/29.10

Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space or group of spaces at least one of which shall give access to a stairway forming a vertical escape. PSCO identified a storage locker converted to an office in Camp Carnival kids area. 74SOLAS(81Amend)II-2/28.1.2

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed an energized electrical wire in the STBD Azipod compartment. PSCOs also observed open electrical wiring in Aft Emergency Mooring Station, FWD Fan Room located on deck 5, and other electrical storage lockers throughout the vessel. 74SOLAS(04)II-1/45.1.3

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. PSCO observed large amounts of combustibles and other items stored in A/C 1001 as well as other A/C spaces and electrical lockers throughout the ship. 74SOLAS(81Amend)II-2/26.2.2

Where the emergency source of electrical power is a generator, it shall be... started automatically upon failure of the electrical power and shall be automatically connected to the emergency switchboard. Vessel notified USCG of emergency generator breaker not able to switch on emergency genrator and transfer load. Provide USCG with open oceans plan for starting emergency generator and switching load. 74 SOLAS (20) II1/ 42.3.1.2

Before the ship leaves port and at all times during the voyage, all life-saving shall be in working order and ready for immediate use. The PSCO observed the port side rescue boat self-lowering device was entangled and not ready for immediate use. 74SOLAS(20)III/20.2

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers to identify the routes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. The PSCO observed the low location photoluminescent was blocked by construction material on deck 03 FWD starboard side. 74SOLAS(01)II-2/41.2.4.7

The construction of all doors and door frames in "A" Class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. Such doors and door frames shall be constructed of steel or other equivalent material. The PSCO observed an A-60 FSD, #302 (Brig door), had a 1" diameter hole in the center of the door where a camera was previously installed. 74SOLAS(81 Amend)II-2/30.2

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Portable extinguishers which have been discharged shall be immediately recharged or replaced with an equivalent unit. The valve at fire station #284 was frozen and the valve wheel at fire station #278 was stripped. 74SOLAS(20)II-2/14.2.1.2

PSCO found escape routes not identified with signage in areas within the casino area. 74SOLAS(01)II-2/41-2.4.7

Eletrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO identified unapproved cable spliced directly into cable box preventing electrical box closure. 74SOLAS(04)II-1/40.1.3

The minimum fire integrity of all bulkheads and decks shall be prescribed in tables 26.1 to 26.4. PSCO found missing A-60 insulation in electrical locker on deck 9 at frame 35. 74SOLAS(81 Amend)II-2/26.1

Eectrical installations shall be such that electrical services for safety will be ensured under variouis emergency conditions. Dead and exposed wiring was found not in a junction box. 74 SOLAS(04) II-1/40.1.2

Escape route marking must enable passengers to idenitfy the routes of escape, and readily identify the escape exit. the signage for deck 10 starboard dining area near frame 87 was missing. 74 SOLAS(01) II-2/28.1.10

Before the ship leaves port and at all times during the voyage, all alife-saving appliances shall be in working order and ready for immediate use. The rudder angle indicator for lifeboat #16 was found in-operable. 74 SOLAS(20)III/20.2

In passenger shios, doors other than power-operated watertight doors, shall be so arranged that positive closure is assured in case for fire in the space, by power-operated closing arrangements or by the provision of self-closing doors capable of closing against an inclination of 3.5 opposing closure. Fire doors 1202 and 1204 were found not capable of latching. 74 SOLAS(81 Amend.)II-2/11.2.3

Before the ship leaves port and at all times during the voyage all life-saving appliances shall be in wokring order and ready for immediate use. Starboard side engine for lifeboat #8 would not start. 74 SOLAS(97)III/20.1.1.1

Means shall be provided to close all openings which may admit air to or allow gas to escape from a protected space. Door gasket for EDG space was found deteriorated which would allow the escape of gases from the space. 74 SOLAS(01)II-2/5.1.4

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Deck 4 and deck 9 stairway guides were unfamiliar with passenger evacuation duties. Lifeboat coxswain for lifeboats 6 and 19 were unfamiliar with operation of lifeboat. Lifeboat 9 crew were unfamiliar with safety equipment in the lifeboat.

FSD 12.09 in the spa/steam room is a primary means of escape and was unable to be opened. Until door is operating as designed, no passengers can enter the Steam Room.

All doors and frames in such bulkheads shall be of non-combustible materials and shall be so constructed and erected as to provide substantial fire resistance to the satisfaction of the administration. FSD 835 within the main galley had a locking mechanism removed rendering the door to no longer meet the structural fire protection requirements.

FSD 506 had two holes in the door due to missing screws in the handle assembly, and FSD 378 has holes in the door due to a peephole being removed.

Doors and door frames in "B" class divisions and means of securing them shall provide a method of closure which shall have resistance to fire as far as practicable equivalent to that of the divisions except that ventilation openings may be permitted in the lower portion of such doors. and such opening or openings shall not exceed 0.05 square m. When such openings is cut in a door it shall be fitted with a grill made of a non-combustible material. FSD 506 in crew stairwell wont latch.

Only public toilets, lifts, lockers of non-combustible materials providing storage for safety equipment and open information counters may be located within the stairway enclosure boundaries. PSCO observed storage of cleaning supplies in crew stair 290 on deck 7.

Means shall be provided whereby normal operations of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. PSCO were notified that the STBD azipod become inoperable due to a failed electrical card in the cyclo converter. provide class report to Coast Guard.

PSCO observed large crate of bananas blocking exit from storage room along I-95. PSCO observed multiple large carboard boxes inside crew stairwell, forward of the main stage near fire station 239. 74 SOLAS 81 Amend II-2/28.1.2 10 c

Below the bulkhead deck two means of escape, at least one of which shall be independent of watertight doors, shall be provided from each watertight compartment or similarly restricted space or group of spaces. PSCO observed large linen bin blocking escape from pressing room within the main laundry room. 74 SOLAS 81 Amend II-2/28.1.1

The construction of all doors and door frames in "A" class divisions,with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. Such doors and door frames shall be constructed of steel or other equivalent material. PSCO observed sliding fire door did not close. 74 SOLAS 81 Amend II-2/30.2

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Coxswain for lifeboat number 19 was unfamiliar with procedures for clutching in engine. Four (04) assigned boat crew for lifeboat number 8 were unfamiliar with procedures for propulsion and emergency steering. 74SOLAS(20) III/19.2.1

Before the ship leaves port and at all times during the voyage, all life-saving applicances shall be in working order and ready for immediate use. PSCO observed cracked bilge hose on lifeboat number 19. 74SOLAS(20) III/20.2

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Three (3) stairway guides in forward passenger stairway, decks 5, 6, and 7 were unfamiliar with ship's procedures for guiding passengers to muster stations who do not have sail and sign cards in their possession. 74SOLAS(20) III/19.2.1

While ships are operating within an emission control area, the Sulphur content of fuel oil used on board ships shall not exceed the following limits: .10% mm on and after 1 January 2015. On 08 December 2022, vessel notified Sector Jacksonville that several of their MGO fuel tanks were found to be contaminated with HFO. Engine exhaust is being run through vessel's approved exhaust gas scrubbing unit, however vessel's boiler exhaust does not. The vessel's incinerator exhaust also does not run through the exhaust gas scrubbing unit, but it is not being used since the incident was discovered. Vessel is continuing to burn potentially contaminated fuel in the boiler inside an Emission Control Area. MARPOL VI/14.4.3

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board... Modifications were made to the HFO tanks to comply with MARPOL Annex VI. MGO tanks are still connected to HFO tanks via a common rail pipe. During internal transfer of HFO, ship's crew overfilled FO7 and the Overfill tank. HFO backed up into MGO tank 09, which contaminated vessel's day tank in the process. 74 SOLAS (20) I/11 MARPOL I/6.3.4

The company should establish procedures to ensure the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirement which may be establish by the company. The objective nature of deficiencies 1 and 2 demonstrate the vessel is not being maintained to international standards, bringing into question the validity of the safety management certificate issued pursuant to the ISM code. An external audit is required to be submitted within 14 days. 74 SOLAS (20) IX/5 ISM A/10.1

Every oil fuel pipe which, if damaged, would allow oil to escape from a storage, settling or daily service tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. PSCOs observed fuel oil quick closing valves 4, 5, 6, and 7 unable to be remotely closed from outside the machinery space. 74 SOLAS (97) II-2/15.2.5

Every lifeboat shall be so arranged that an adequate view forward, aft, and to both sides is provided from the control and steering position for safe launching and maneuvering. PSCOs observed lifeboat numbers, 5, 7, and 8 front and side windshields to be hazed over, impeding visibility. (74 SOLAS (97) III/41.7.13)

Fire dampers shall operate automatically but shall also be capable of being closed manually from both sides of the bulkhead. PSCOs observed fire damper in EDG space was unable to be fully closed. (74 SOLAS (97) 11-2/16.2 2).

All lifesaving appliances provided in this part shall be fitted with retro-reflective material where it will assist in detection and in accordance with the recommendations of the Organization. PSCOs observed retro-reflective material on the topsides of all lifeboats need replacement due to fading of material. (74 SOLAS (97) 111/30.2.7)

For a period of 36 hours, emergency lighting at every muster and embarkation station and over the sides as required by regulation 111/11.4. PSCOs observed emergency lighting was not illuminated at embarkation stations for lifeboats 10, 18, and 20. (74 SOLAS (04) 11-1/42.2.1.1).

11199- Other ( life saving) -Partially enclosed lifeboats... shall be fitted with a permanently attached foldable canopy... The canopy shall so arranged that: the occupants can escape in the event of the lifeboat capsizing. Seven lifeboats were observed to be modified whereby the canopy was secured in multiple additional locations due to damage to the attached hoop-to-frame mounting, restricting the ability to quickly open the enclosure in the event of capsizing. SOLAS 97 111/42

-07101- Fire prevention structural integrity- The hull, superstructure, structural bulkheads, decks, and deckhouses shall be construed of steel or other equivalent material and shall be according to the integrity and insulation standards given in the tables of regulations 26 and 27. Combustibles materials, including share/expired distress signals,were found stowed in a category 10 fan space on deck 5, main vertical zone 7. SLAS 01 11-2/23.1

04104 - Emergency Systems - Alleyways, stairways and exits giving access to the muster and embarkation stations shall be lighted. The low location lighting crew corridors was not operating. SOLAS97CHIII-2/11.5

07120 - Fire Safety - Below the bulkhead deck two means of escape, at least one of which shall be independent of watertight doors, shall be provided from each watertight compartment or similarly restricted space or group of spaces. PSCO observed means of escape in the main laundry ironing room obstructed and padlocked. SOLAS97CHII-2/28.1.1

07101 - Fire Safety - The hull, superstructures, structural bulkheads, decks, and deckhouses shall be constructed of steel or other equivalent material and shall be according to the integrity and insulation standards given in the tables of regulation 26 and 27. Combustible materials were found stowed in a category 10 fan space. SOLAS97CHII-2/23.1

The doors shall be self-closing... and shall have an approximately uniform rate of closure of no more than 40s and no less than 10s with the ship in the upright position. FSDs 1206, 326, 364, 359, & 337 did not close properly. SOLAS97CHII-2/30.4.1

03112 - Scuppers, inlets, and dischargees - While operating within three nautical miles from shore, discharges of graywater are prohibited unless they meet the effluent standards in 40 CFR 133.102. The vessel discharged 22 metric tons of graywater while in-port (within 3 nautical miles) through their ballast water line. 33 USC § 1251

11118 - Life Saving Appliances - On passenger ships each lifejacket shall be fitted with a light complying with the requirements of paragraph 2.2.3. of Code. PSCO's observed numerous inoperable lifejacket lights not activating, but within expiration date of the manufacturer. Vessel is to conduct 100% accountability for all lifejacket lights prior to next US port. Vessel is to identify operational status of all lifejacket lights and develop a plan to fix all lights. SOLAS14CHIII/22.3

09209 - Working and Living Conditions - Electrical installation shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO's observed an open live circuit board in the pool pump room on Deck 09. PSCO's observed broken latch on panel door located in electrical locker 3-258. SOLAS97CHII-1/40.1.3

09209 - Working and Living Conditions - Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. Junction box 9621 CC JB 202 in forward crew stairwell found broken, free from mount, and hanging loosely. SOLAS04CHII-1/45.5.5

07199 - Fire Safety - Every oil fuel pipe, which, if damaged, would allow oil to escape from a storage, settling or daily service tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. Valve to HFO suction to booster unit for ME 3-4 did not close during remote activation. SOLAS97CHII-2/15.2.5

07104 - Fire Safety - Fire doors in main vertical zone bulkheads designed to reopen upon contacting an object in its path shall reopen sufficiently to allow clear passage. FSD 917. SOLASII-2/30.4.2

07104 - Fire Safety - The doors shall be self-closing and be capable of closing with an angle of inclination of up to 3.5 degrees opposing closure, and shall have an approximately uniform rate of closure of no more than 40s and no less than 10s with the ship in the upright position. 04104 - Emergency Systems - In addition to the emergency lighting required by regulations II-1/42 and III/11.5, the means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators. Several sections of photoluminescent strips were inoperable. Several sections were found in the Lido galley passage way on deck 10. SOLAS97CHII-2/28.1.10

02105 - Structural Conditions - The steering gear compartments shall be arranged for suitable working conditions in the event of hydraulic fluid leakage. Excess hydraulic fluid was observed around the steering pumps, piping, and both azipods. SOLAS04CHII-1/29.13.2

07105 - Fire Safety - Doors and door frames in "B" class divisions shall have resistance to fire equivalent to that of the divisions. "B-15" door in electrical locker 3-258 had wastage and would not prevent the passage of flame and smoke. SOLAS97CHII-2/31.1.1

11129 - LIFE SAVING APPLIANCES - BEFORE THE SHIP LEAVES PORT AND AT ALL TIMES DURING THE VOYAGE, ALL LIFE-SAVING APPLIANCES SHALL BE IN WORKING ORDER AND READY FOR IMMEDIATE USE. PSCO's OBSERVED LIFEBOAT #18's PROPELLER WOULD NOT ENGAGE AND COULD NOT BE CLUTCHED IN AHEAD OR ASTERN. 74 SOLAS 1997 CON. III/19.2 - 10c

07105 - FIRE SAFETY - FIRE DOORS IN MAIN VERTICAL ZONE BULKHEADS AND STAIRWAY ENCLOSURES SHALL BE DESIGNED TO RE-OPEN UPON CONTACTING AN OBJECT IN ITS PATH AND SHALL RE-OPEN SUFFICIENTLY. PSCO's OBSERVED THAT FSD # 1004 DID NOT RE-OPEN WHEN THE CRASH BAR WAS ACTIVATED. 74 SOLAS 1997 CON. II-2/30.4.2 - 16c

07105 - FIRE SAFETY - FIRE DOORS IN MAIN VERTICAL ZONES AND STAIRWAY ENCLOSURES SHALL BE SELF-CLOSING. PSCO's OBSERVED THAT FSD# 231 & 391 WOULD NOT PROPERLY CLOSE. 74 SOLAS 1997 CON. II-2/30.4.1 - 10c

04103 - EMERGENCY SYSTEMS - EMERGENCY LIGHTING IN ALL SERVICE AND ACCOMMODATION ALLEYWAYS, STAIRWAYS, AND EXITS SHALL PROVIDE LIGHT FOR A PERIOD OF 36 HRS. PSCO OBSERVED NUMEROUS EMERGENCY LIGHTS INOPERABLE OR NOT CONNECTED, DUE TO INCOMPATIBLE CONNECTORS AND INOPERABLE LED DRIVERS. 74 SOLAS 2004 CON. II-1/42.2.1.3 - 50a,c

13199 - MAINTENANCE OF CONDITIONS AFTER SURVEY - THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO ENSURE THAT THE SHIP WILL REMAIN FIT TO PROCEED TO SEA WITHOUT DANGER TO SHIP OR PERSONS ON BOARD. PSCO OBSERVED LUBE OIL PURIFIER #1, 2, 3, & 5 WITH TEMP. REPAIRS MADE WITH PERMATEX. 74 SOLAS 2004 CON. I/11(a) - 16a,c

13199 - PROPULSION AND AUX MACHINERY - THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO ENSURE THAT THE SHIP WILL REMAIN FIT TO PROCEED TO SEA WITHOUT DANGER TO SHIP OR PERSONS ONBOARD. PSCO OBSERVED DG# 2 WITH EXCESSIVE OIL LEAKS. 74 SOLAS 2004 CON. I/11 (a). - 16a,c

Rescue boat shall be stowed in a state of continuous readiness for launching within 5 minutes. Port Rescue boat would not start. Form P issued by class designating #8 lifeboat as the temporary rescue boat. 74SOLAS(97) III/14.1. 50ac.

The machinery and associated piping shall be of a design and construction adequate for the service for which they are intended. The #1 ship service generator was found to have excessive fuel oil leaking. 74SOLAS(04) II-1/26.1. 16ac

Doorways and corridors and immediate landings in means of escape shall be sized in the same manner as stairways. Crew property was being stowed ina fashion that blocked escape doorways in vicinity of fire screen door 348. 74SOLAS(97) II-2/28.5.3. 16ac.

Every member with assigned emergency duties shall be familiar with these duties before the voyage. Several crew members lacked the minimum knowledge necessary to assist passengers in evacuation. 74SOLAS(14) III/19.2.1. 16ac.

Fire protection systems shall be in good working order so as to ensure the required performance if a fire occurs, including fire resistant dicisions and protection of openings and penetrations. Fire doors 364, 822, 921, and 1018 did not close properly. 74SOLAS(14) II-2/14.2.1.1.1.

The Condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulation to ensure that the ship will remain fit to proceed to sea without dangers to tpersons on board. The exit passageway for the engine room in the vicinity of fire screen doow 348 is partially blocked by storage shelving. 74SOLAS(14) I/11(a).

Ships, when engaged on international voyages, shall be fitted with a voyage data recorded (VDR). VDR is non-operational. 74SOLAS(2014 Cons.) V/20.1.3

0720 (Firefighting Equipment) - The condition of the ship and its equipment shall be maintained to conform with the present regulations to ensure the ship will remain without danger to the ship. A soft patch was discovered on the firemain topping off pump discharge line in the overhead of pump room #3. 74 SOLAS (01) I/11, 16ac

0800 (Accident prevention) - Fire doors designed to reopen upon contacting an object in its path shall reopen not more than 1 meter from the point of contact. Sliding fire door #917 did not automatically re-open once the safety bar was engaged. 74 SOLAS (01) II-2/30.4.11, 16ac

0610 (lifeboats) - All lifeboat tillers shall be capable of controlling the rudder in case of failure of the steering mechanism. It was discovered the emergency tiller was inoperable on lifeboats: #12, #14, and #17. 74 SOLAS (97) III/41.7.2, 16ac

Bilge wells in periodically unattended machinery spaces shall be monitored in such a way that the accumulation of liquids is detected. A bilge alarm located port side in the A/C compressor room is not operational. 74 SOLAS (2004 CONS.) II-I/48.1

Fire protection systems shall be in good working order so as to ensure the required performance if a fire occurs; Structural fire protection, includind fire-resistant divisions and protection of openings and penetrations. Fire Doors 1403 did not closed properly, sliding fire doors: 819,822 and 843 did not automatically re open once the safety bar was engaged. 74 SOLAS (2014 Cons.) II-2/14.2.1.1.1

All electrical apparatus shall be constructed and so installed as to not cause injury while handled or touched in a normal manner. Examiners found an exposed wire on the open deck adjacent to Lifeboat #4. 74 SOLAS (04 Cons) II-I/45.1.3

There must be no sewage or sewage treatment chemicals remaining on any surface. Two soft patches were found on the exterior side wall of MSD Tank #3. 33 CFR 159.121.

After any survey of the ship has been completed, no change shall be made in the structural arrangments, machinery, or equipment without the sanction of the administration. Multiple soft patches were found on piping in way of the gray water tank. 74 SOLAS (2014 Cons.) I/11(b).

Structural fire protection and protection of openings and penetrations shall be kept in good working order as to ensure required performance if a fire occurs. The fire dampers located in the emergency generator room did not close properly. 74 SOLAS (2014 Cons) II-2/14.2.1.1.1

The Master, Owner, Operator, or person in charge of a vessel equipped with ballast tanks must submit request for approval of Ballast Water Management System to the Commanding Offcer Safety Marine Center.

Doors in A class division shall provide resistance to fire as well as to the passage of smoke and flame. PSCO identified Class III WTDs 2 through 6 on deck 0 are admitting air,gaskets and/or doors will need to be adjusted.

Oil filtering equip shall be provided w/ alarm arrangement to indicate when oil content not exceeding 15ppm can't be maintained. Crew could not demonstrate proper operation of alarm on Turner Designs Hydrocarbon Instruments OWS. Ship's crew is prohibited from the usage of this OWS until proper op is demonstrated & the IOPP certificate form A must be amended to reflect a max thruput of 2.0 m3/h.
