To assist in casualty investigations, cargo ships, when engaged on international voyages, shall be fitted with a VDR which may be a simplified voyage data recorder (S-VDR). PSCO's were notified and identified that the VDR is not fully functional.

All items of rescue boat equipment...shall be secured within the rescue boat. PSCO observed rescue boat items in a storage locker.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to ship or persons onboard. PSCO observed operation of the class approved temporary repair to the rescue boat davit.

BEFORE THE SHIP LEAVES PORT AND AT ALL TIMES DURING THE VOYAGE, ALL LIFE-SAVING APPLIANCES SHALL BE IN WORKING ORDER AND READY FOR IMMEDIATE USE. PSCOS OBSERVED NON-APPROVED RESCUE BOAT DAVIT OPERATION. RESCUE BOAT DAVIT IS MISSING ITS ELECTRIC MOTOR AND IS BEING OPERATED BY A PULLEY SYSTEM ATTACHED TO A MOORING WINCH. 74 SOLAS (97)III/19.2

Where penetrations of watertight bulkheads are necessary for access arrangements are to be made to maintain the watertight integrity. PSCO observed the port and starboard quick acting watertight doors from the engine room to have loose dogs.

Rapid recovery of the rescue boat shall be possible when loaded with its full compliment of persons and equipment. Vessels rescue boat winch is outfitted with an electric motor for launching and recovery that is not operational.

the following basic principles underlie the regulations in this chapter and are embodied in the regulations as appropriate, having regard to the type of ships and the potential fire hazard involved: .3 Restricted use of combustiable materials. PSCO observed on multiple occasions oil and fuel oil stored in non-approved containers and approval containers scattered throughout the engine room space. Additionally, PSCO observed paint storage hidden in port side void of engine room. 74 SOLAS (97) II-2/2.2.3

Every ship of 100 gross tonnage and above, and every ship which is certified to carry 15 or more persons, and fixed or flooding platforms shall carry a garbage management plan which the crew shall follow. PSCO observed immense ammount of trash in multiple areas of the engine room. Trash was incorrectly stored in some cases. MARPOL V/10.2

The machinery boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and constructed adequate for the service for which they are installed... PSCO observed excessive damage and wastage to clorifier and boiler hot well resulting in extreme water leakage and possible watchstander burns. 74 SOLAS (04) II-1/26.1

The company and the ship shall comply with the requirements of the international safety management code. For the pupose of this regulation, the requirements of the code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions or the relevant rules and regulations and with any additional requirements which may be established by the company. In accordance with the vessel's safety management system: the environmental program includes a commitment to continnual improvement and prevention of pollution, complywith relevant environmental legislation and regulation. the previouslynoted deficiency 2 demonstrates that they crew is not following their safety management system. An external audit is required. 74 SOLAS (20) IX/3.1 ISM Code Part A/ 10.1

Each lifeboat shall be provided with an appliance which is capable of launching and recovering the lifeboat. Lifeboat stuck in position and unable to be recovered. Refer to 74SOLAS(97)CH III/15.2

PSCO observed a soft patch on a drain pipe leading into the steering gear room. Additionally, PSCO found 02 steam return pipes leaking with fabricated plastic catch basins in main machinery space. 74SOLAS(20)I-II/a

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position. PSCO observed several cylinders stored improperly throughout the vessel's engine room. 46CFR147.60(b)(1)

The means for securing and maintaining weather tightness shall be to the satisfaction of the administration. The arrangements shall ensure that the tightness can be maintained in any sea conditions. Hate cover securing devices in vicinity of bays 26, 30, 34, and 38 starboard side are warped and do not serve the intended purpose. Other locking devices for those hatch covers meet the requirements. ICLL(88) 16/4

The means for securing these doors weathertight shall consist of gaskets and clamping devices or other equivalent means. Door in vicinity of rescue boat does not close in a manner that provides for weather-tightness. ICLL(88) 12/1

Doors required to be self-closing shall not be fitted with hold-back hooks. Doors to the ship’s office and hospital space are identified as self-closing on FCP. Both have holdbacks not fitted w/ remote release. 74 SOLAS(97 cons) II-2/47.2

The fire resistance of doors shall as far as practicable, be equivalent to that of the division to which they are fitted. Doors to steering gear space and engine storeroom have key-locking mechanisms removed leaving opening in the doors. Door on main deck to cable trunk does not close completely. 74 SOLAS(97 cons) II-2/47.1

…the minimum fire integrity of bulkheads and decks shall be as prescribed in table 44.1…flammable liquids found stored inside boundary of machinery space. 74 SOLAS(97 cons) II-2/44.1

Efficient guard rails…shall be fitted to all exposed parts of the freeboard and superstructure decks. Guard rail in vicinity of bays 19 and 21 found broken. ICLL (88) 25/2

Satellite EPIRB shall be annually tested for all aspects of operational efficency, on cargo ships, 3 months before or aftger the anniversary date of the cargo ship safety radio certificate. PSCO found that the annual test of EPIRB was expired 04 months beyond the anniversay date. 74SOLAS (14 cons ed) IV/15.9.1.2

A rescue boat shall be fitted with an inboard or outboard engine on the rescue boat. The outboard engine was not operational and the ship had a dispensation from Liberia until 29FEB2016 PSCO could not test the outboard engine on the rescue boat durin the course of examination. 74 SOLAS (97 cons ed) III/47.1.7
