Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Vessel reported inoperable auxiliary generator. Provide root cause analysis and technician report to MI satisfaction, prior to the carriage of passengers. 46 CFR 119.100

The fire pump must be capable of delivering a single hose stream from the highest hydrant, through the hose and nozzle at 50 psi. Stbd fire pump had insufficient pressure at approximately 20-30psi. 46 CFR 118.300 (b)

Piping, valves, and fittings must be securely supported, and where necessary, protected against damage. Co2 Piping in the Port Steering compartment is corroded. 46 CFR 118.410(d)(3)

Exhaust gas must not leak from the piping or any connections. Stbd Fwd outboard engine exhaust has a leak. 46 CFR 119.430 (b)

At each inspection for certification...all observed unsafe, practices, fire hazards, and hazardous situations must be corrected. Oil was leaking fwd of the aft port engine. 46 CFR 115. 830 (a)

A duct penetrating an A-class fire control boundary and opening into a space formed by that boundary must be equipped with a fire damper. Aft bulk head has vents with no dampers. 46 CFR 116.610 (f)(4)

The installation of fuel lines, including pipe, tube, and hose, must comply with the requirements of this paragraph. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. Linkage for fuel shut-off valve in STBD AMR broke during testing. 46 CFR 119.455

The design of all exhaust systems must ensure minimum risk of injury to personnel. Protection must be provided in compliance with § 116.970 of this chapter at such locations where persons or equipment might come in contact with an exhaust pipe. Port generator missing lagging on exhaust at elbow of turbo outlet. 46 CFR 119.430

Each distribution panel or switchboard must be totally enclosed and of the dead front type. Breaker #30 in power panel 203, located in STBD AFT crew space on main deck missing blank. 46 CFR 120.330(b)

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. The fire dampeners in the port and starboard engine rooms did not close all the way.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. There were 03 problems identified with the vessel's emergency lighting system: 1. Port side main deck emergency light was not working 2. Multiple emergency lights were not labeled 3. Upper deck starboard aft emergency light cover was missing and was covered by a paper plate.

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows:Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. The engine room did not have the correct CO2 warning signage posted per the regulations.

Electrical equipment on a vessel must be installed and maintained to: Provide services necessary for safety under normal and emergency conditions. There was aninstance of potentially overloaded circuits due to multiple items plugged into surge protectors in the snack area near the wheelhouse.

Electrical equipment on a vessel must be installed and maintained to: Minimize accidental personnel contact with energized parts. There were 02 problems identified in the wheel house: 1. Port wheel house control system was not closed. 2. The fuse box below the wheel house was left ajar.

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. There were cut electrical cables between the port side AMR and engine room spaces. Pull the unused wires and fill the holes to ensure the space is vapor tight.

An enclosed or partially enclosed space within a vessel must be adequately ventilated in a manner suitable for the purpose of the space. The starboard vent in the aft jet room was painted shut.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. The Port and starboard engine room bilge was filled with an excess amount of liquids and needs to be cleaned.

A vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel has not completed annual inspection. 46 CFR 115.500

Doors in A-Class fire control boundaries must meet the following requirements: Holdback hooks are not allowed. If installed, a holdback mechanism for a door must allow the door to be released locally, upon signal from a control space, and upon disruption of the power system. Vessel has several holdback devices in A-Class boundaries.

A fire control plan (FCP) must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge, embarkation stations, the location of fire protection/emergency equipment, compartment titles and hazard classification of accommodation and service spaces, and structural fire protection boundaries. Vessel has a Fire Control Plan that is not approved by [name withheld], this FCP is missing hazard classification of accommodation and service spaces and structural fire protection boundaries.

Except when paragraph (b) of this section is applicable, the station bill of a vessel of more than 19.8 meters (65 feet) in length required to have more than four crew members, must set forth the special duties and duty station of each crew member for various emergencies. The duties must, as far as possible, be comparable with the regular work of the individual. The duties must include at least the following and any other duties necessary for the proper handling of a particular emergency. The Station Bill that is currently onboard does not include roles for all of the crew positions (i.e. Mate) and does not include all of the equipment that the vessel currently uses (fire pump and rescue equipment). Additionally, during the drills witnessed during this inspection, crew did not follow the steps and roles listed on the Station Bill. Company to develop relevant Station Bill for crew positions and actual roles that crew should carry out.

A structure is considered open to the weather when an exterior end bulkhead of the structure is open and is not fitted with any means of closing. Aft tonnage opening interior in concession room is blocked by permanently fitted cupboards and is not considered in the clear in accordance with 46 CFR 117(d)(2)(i). Stairway tonnage opening has permanently affixed bike bike track at the forward lower corner preventing its removal. Make corrections to tonnage openings to the satisfaction of attending MI by next credit drydock. 46 CFR 69.117(d)(2)

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. MI found last drill was conducted on November 2021 and the crew was unfamiliar with some the equipment onboard and the proper use (i.e. recovery equipment and first aid/CPR equipment). 46 CFR 122.520(a)

A-Class bulkheads must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the structural bulkhead. The Fire Control Plan identifies the bulkhead at the lower deck refreshment station as an A-0 Boundary; however, the bulkhead (a tonnage opening) has been modified to accept a permanent large penetration into the refreshment station.

Suitable means shall be provided for priming centrifugal bilge pumps which are not of the self-priming type. Both port and stbd bilge pumps took approximately 5 minutes to suitably prime.

Periodic Safety Test Procedures (PTSPs) must be submitted for Coast guard Approval. The vessel’s PSTPs did not go through the required approval process. Once USCG Marine Safety Center reviewed the PSTPs, the local Office in Charge of Marine Inspections (OCMI) must complete the testing of the PSTP procedures in order for them to be “Approved."

All escape hatches and other emergency exits used as a means of escape must be marked on both sides in clearly legible letters at least 2-inches high “EMERGENCY EXIT, KEEP CLEAR.” Emergency exit hatches from both the port and stbd engine-rooms are not clearly legible.

Remote fuel shutoff stations must be marked in clearly legible letters at least 1-inch high indicating purpose of the valve and direction of operations. The installed fuel shutoff valves do not have 1-inch high, clearly legible letters, with direction of operation.

Excluded spaces (tonnage openings) use to calculate gross tonnage, must not: 1) be fitted with shelves or other means designed for securing store; 2) an opening that would otherwise permit the space to be excluded space as fitted with a means of closure; or 3) other features of the space make it possible for the space to be closed. The tonnage openings located on the main deck, stbd aft at the refreshment station are fitted with shelves and counter arrangements that are directly attached to the tonnage opening.

Piping used in vital systems must meet 46CFR 56.60 in subchapter F. Non metallic hose must be SAE J2942. It must be used in lengths not to exceed 30 inches. Non metallic hose must be complete with factory assembled fittings requiring no further adjustment of the fittings on the hose. Hose fittings must comply with SAE J1475. #1, #2, #3, and #4 MDE salt water exhaust cooling lines have non-metallic hoses installed with hose clamps and of lengths in excess of 30 inches. 46CFR 119.710(c), 46CFR 56.60-25(b), 46CFR 56.60(b)(5)

Piping used in vital systems must meet 46CFR 56.60 in subchapter F. Non metallic hose must be SAE J2942. It must be used in lengths not to exceed 30 inches. Non metallic hose must be complete with factory assembled fittings requiring no further adjustment of the fittings on the hose. Hose fittings must comply with SAE J1475. #1, #2, #3, and #4 reduction gear salt water cooling lines are connected via hose clamps. 46CFR 119.710(c), 46CFR 56.60-25(b)(1-2), 46CFR 56.60(b)(5).

Piping used in vital systems must meet 46CFR 56.60 in subchapter F. Non metallic hose must be SAE J2942. It must be in lengths not to exceed 30inches. Non metallic hose must be complete with factory assembled fittings requiring no further adjustment of the fittings on the hose. Hose fittings must comply with SAE J1475. Salt water piping on #1 and #2 ship service generators are connected using hose clamps. 46CFR 119.710(c), 46CFR 56.60-25(b)(1-2), 46CFR 56.60(b)(5)

Penetrations in A-Class fire control boundaries must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetrations must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. Port and starboard machinery space hatches are constructed of uninsulated stainless steel material too thin to meet the requirement.

Penetrations in A-Class fire control boundaries must be constucted to prevent the passage of flame and smoke for one hour. In addition, the penetrations must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. Hatch covers on port and starboard machinery space entry are splitting and seperating on the edges preventing hatch to seal.

Piping, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 degrees celcius, must be insulated where necessary to prevent injuries. #1, #2, #3 and #4 MDE A and B bank exhaust piping have exposed flanges just above where the salt water cooling line enters the exhaust that reach a temperature exceeding the requirement.

Piping used in vital systems must meet 46CFR 56.60 in subchapter F. Non metallic hose must be SAE J2942. It must be used in lengths not to exceed 30 inches. Non metallic hose must be complete with factory assembled fittings requiring no further adjustment of the fittings on the hose. Hose fittings must comply with SAE J1475. #1 and #2 Ship service generator exhaust cooling line is attached utilizing a non metallic hose with hose clamps. 46CFR 119.710(c), 46CFR 56.60-25(b)(1-2), 46CFR 56.60(b)(5).

A vessel must be provided with a bilge piping system that meets 46CFR 56.50-50. Piping used in a vital system must meet 46CFR 56.60. Strainers in both port and starboard ballast tank voids bilge piping contain plastic strainers. Install metallic sea strainers. 46CFR 119.710, 46CFR 56.50-50, 46CFR119.510, 46CFR 56.60.

07 welds in Starboard Void #2 require repair IAW 46 CFR 116.100.

Tonnage openings on the aft starboard side, main deck, are being blocked by seats and bar. Areas must be cleared in accordance with tonnage exemption regulations.

Starboard side engine room lagging is coming off and must be reattached.

Port side engine room has inoperable high level alarm. Provide proposal for checking engine room water level for OCMI approval until issue permanently fixed at next DD.

Provide permanent repair for abandoned transom trim tab assemblies to consist of capped metallic through hulls or crop and renew areas at next drydock. Temp repair consisting of metallic cap on existing non-metallic through hull and marine epoxy inside the shaft tube approved in the interim.

Provide technician's report and prove proper operation of the inboard starboard propulsion system to the satisfaction of the attending marine inspector. Until repairs are made and verified by attending marine inspector an additional deck member is required to be on the bridge as a lookout while the vessel is underway. 46CFR 119.200(a)
