Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by noncombustible hangers or blocks. Exhaust cooling line on stbd fwd, and both port side engines, are zip tied to framing and/or engine. Replace support with adequate securing mechanism.46CFR182.430(b)

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot). Stbd forward railing stanchion has a crack at base.46CFR177.900(b)

Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI identified multiple loose/disconnected electrical cables under bridge operating station. Additionally standing water was observed in the same location. Remove any obsolete electrical cables. Remove water, identify source of water ingress, and make proper repairs.46CFR183.200(b)

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Stbd fuel tank strainer is cracked. Repair and test bilge pick up with Marine Inspector present. 46CFR182.510(b)

Except on a vessel limited to daytime operations, the compass must be illuminated. Compass light is inoperable. Vessel restricted to daylight operations. 46CFR184.402(c).

04 holes were located in the port engine room aft bulkhead where previous equipment was mounted. Make the bulkhead water/vapor tight.

Whenever damage or deterioration to the hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks and may require the vessel to be drydocked or taken out of service to assess the extent of the of the damage, and to effect permanent repairs. Marine Inspector found pitting in the #2 void stbd side in the first sections on both sides of the CVK and in the #3 void stbd side in the first section on both sides of the CVK. Operator made temp repairs to voids. Operator is to make permanent repairs by cropping and renewing all areas were temp repairs were installed at the next Drydock in Mar 2024.

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. Replace missing / damaged lagging on the port aft engine where the exhaust exits the turbo charger there is a section of exposed dry exhaust.46 CFR 177.970

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. (b) Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Vessel did not have back up paper charts or secondary electronic charts. 46 CFR 184.420

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit is missing the ophthalmic ointment or eyewash. 46 CFR 184.710

Except as otherwise provided in this section, rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks, which provide strength and support equivalent to fixed rails. The lifelines previously installed on the starboard aft of the vessel was missing the lower courses. Operator is to reinstall lifelines to original condition.

The engine shutdown for the starboard fwd engine did not shutdown when the CO2 engine shutdown was initiated. Repair/renew to original condition prior to departure.

The heat detector in the generator room appears to have been tested using an open flame. The detector was blackened and cracked. Repair/renew the detector to original condition.

All four main engines had exposed exhaust piping that exceeded 200 degrees F. Insulate all piping that exceeds 140 degrees F on engines.

Dead ended wires from old camera system were hanging in starboard engine room. Remove and properly terminate all dead ended wires.

The light fixture installed in the generator space was installed with a exposed "extension cord" style plug. Re-install cable properly as per regulations.

A New light fixture was installed in the generator space. Provide proof that the fixtures meet UL-595 as required.

At next dry dock provide inspection for repairs to Hull areea approximately 12' x 10' of tunnel cropped and renewed as well as inboard sideshell from tunnel down approximately 18" and 5' from transom forward

Replaced both port and starboard time delay bottles. Both trips at around 24 and 27 seconds on a 30 second bottle

Port aft and starboard aft main engines have exhaust leaks. Vessel master and crew were unaware of length of time leaks existed. Repair exhaust systems to roiginal condition.

Racor filter for generator was leaking fuel near base of filter and had a plastic plug. Replace plug with one of proper material for filter and repair leak.

Generator has an oil leak at the rear seal causing oil to contaminate generator windings creating a fire risk.Oil leak must be repaired and generator windings must be cleaned prior to vessel operating.

Vessel logs do not show vessel crew has conducted drills on vessel or similar vessel within three months. Ensure vessel crew conducts drills and logs as appropriate.

Prove property operation of starboard forward main engine jet bearing

Vessel is required to have an operational self priming power driven fire pump; vessel's fire pump was seized and failed to operate.

Vessel's required high water level alarms failed to operate.

Vessel is required fire detection system failed to operate.

Vessel's bilge dewatering system was unavailable for testing.

An EPIRB's battery shall be replaced prior to its expiration date; renew the EPIRB's expired battery. Vessel is restricted to voyages within 3nm of land.

Vessel must have a means of shutting down a propulsion engine from the pilothouse; the port aft engine failed to secure.

Each entrance to a space protected by co2 must be properly marked; the stbd engine room entrance warning label was unreadable.

The port fwd engine flywheel was uncovered and did not have adequate protection for personnel.

All electrical circuits must be clearly marked/identified; the main breaker located at the generator was unlabeled.

Prove port aft engine secures when CO2 is discharged,

Repair five cracked hull welds to the satisfaction of the attending MI prior to carrying passengers.

Repair ~12 cracked welds connecting hull plating to stiffeners to the satisfaction of the attending CG MI prior to carrying passengers.

Broadcasting incorrect AIS information.

Provide a flame screen for the port fuel tank vent.

Provide an current Certificate of Documentation.

Post an SPV Decal in an area visible to passengers.

Provide proposal to repair hull plate pin holes identified in the port jet room.

Each connection to a conductor shall be made in an enclosure. Make all bilge pumps, floats, and miscellaneous salt water pump wiring connections in an enclosure.

Each conductor shall be held with metal supports not more than 24" apart. Provide supports for the sewage pump power supply conductors in the #3 port void.
