Each survival craft must be: stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. Inflatable life rafts were improperly secured with straps and a clip, preventing float-free operation and automatic inflation. Route restricted to lakes, bays, and sounds. Reconfigure life raft stowage using approved hydrostatic release units to enable float-free and automatic inflation. Limited to LBS. 705 "Prior to" removing route restriction. 46 CFR 180.130(a)(2)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. The portside fuel shutoff valve was not fully operable; the reach rod did not adequately engage with the handwheel, preventing proper opening and closing of the valve. Rectify the valve control mechanism to ensure full and reliable operation to the satisfaction of the attending marine inspector. 46 CFR 182.20-40(b)(3)

All machinery must be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Observed leak in steering linkage at the helm, posing a potential fire hazard and risk of vessel's ability to maneuver safely. Repair leak to the satisfaction of the attending marine inspector. 46 CFR 182.01-5(a)

The intent of this part is to ensure that the scantlings, material, fastenings, workmanship and arrangement of each vessel are such as to provide for safe operation in accordance with the terms of its certificate of inspection. The rudder arm mounts on both the starboard and port sides have cracked welds. Cracked welds were observed at all four corners of the starboard-side entrance and at the bottom two corners of the port-side entrance to the accommodation space. Weld repairs performed on various structural frames—particularly those located within the steering gear void—were incomplete. Welding was only carried out on the top side of the frames, with no welding performed underneath, leaving visible cracks. Perform weld repairs in accordance with standards ensuring full weld continuity on all affected joints. All repair work is to be conducted under the oversight and to the satisfaction of the attending marine inspector. 46 CFR 177.01-5(a)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. The stowage compartments for adult life jackets were not labeled to indicate the quantity or size of life jackets contained within. Label all life jacket stowage compartments clearly to indicate both the number of life jackets stored and their designated size. 46 CFR 185.604(f)

As appropriate for the intended voyage, all vessels must carry adequate and up-to-date: Charts; Sailing directions; Coast pilots; Light lists; Notices to mariners; Tide tables; Current tables; and all other nautical publications necessary. Charts were outdated, and several required navigational publications were missing or not readily available. Obtain and maintain up-to-date nautical charts (either electronic or paper formats are acceptable) and ensure all required navigation publications are onboard. 46 CFR 184.420

On a vessel of more than 65' in length a commercial fire hose is required IAW 181.320(b)1. One fire hose was found to have a leak. Replace IAW 181.320(b)(1) to the satisfaction of the Marine Inspector. 46 CFR 181.320.b(1)

Each hatch exposed to the weather must be watertight, except that the following hatches may be weathertight.. each hatch that provides access to crew or passenger accommodations. Foam weatherstripping needs to be replaced due to missing or damaged to ensure weathertight. 46 CFR 177.13-1 46 CFR 171.124(a) & (c)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel's alcohol testing devices on board were all expired. Vessel to get sufficient number of alcohol testing devices readily accessible on board the vessel.

A vessel must comply with the applicable requirements for any radio installations, as set forth in 47 CFR part 80. Vessel is missing a MF-DSC or MF capable operating on 2670 kHz. Vessel is restricted to Coastwise operations not more than 20 nautical miles offshore. Install MF-DSC or MF radio capable operating on 2670 kHz to the satisfaction of the attending Marine Inspector.

Be provided with inflatable buoyant apparatus of an aggregate capacity that will accommodate at least 67% of the total number of persons permitted on board. Vessel is missing 01 inflatable buoyant apparatus. Vessel is limited to 34 total persons on board including crew when operating on Oceans Route.

A vessel must comply with the applicable requirements for any radio installations, as set forth in 47 CFR part 80. Vessel is missing a MF-DSC or MF capable operating on 2670 kHz. Install correct radio to the satisfaction of the attending Marine Inspector.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either:A pressure type connector on each conductor; A solder lug on each conductor; A splice made with a pressure type connector to a flexible lead or conductor; or;A splice that is soldered, brazed, or welded to a flexible lead or conductor. Vessel had wires on starboard side center void on A/C unit not in an enclosure. Repair to the satisfaction of the attending Marine Inspector.

A vessel must be provided with a satisfactory arrangement for draining and watertight compartment, other than small buoyancy compartments, under all praticable conditions. Suction in stbd void did not produce suction when engaged with pump. Repair to the satisfaction of the USCG Marine Inspector. 46 CFR 182.500(a)

WHen operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by EPA, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. Vessel's sewage overboard valve is ceased in open position. Repair to the satisfaction of the USCG Marine Inspector. 33 CFR 159.7(b)

All vessels must have navigation lights that are in compliance with the applicable sections of the international and inland navigation rules. Vessel's stern light was not operational. Repair to the satisfaction of the USCG Marine Inspector. Vessel restricted to daylight operations until rectified. 46 CFR 183.420

An adult lifejacket must be provided for each person carried on board. Vessel has 60 lifejackets adult size on board. Repair or replace additional lifejackets to the satisfaction of the USCG Marine Inspector. Vessel restricted to 60 adults allowed until rectified. 46 CFR 180.71(a)

A vessel may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel's COI was found to be expired as of 27AOR2022 and must be renewed Prior to the Carriage of Passengers. 46 CFR 176.100(a)

No major repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge and approval of the Officer in Charge, Marine Inspections. Vessel began making alterations to engine room for re-power project without approval of OCMI. Vessel shall not carry passengers until re-power project is complete and machinery meets all operational requirements to the satisfaction of the attending USCG Marine Inspector. 46 CFR 176.20-1(a)

All vessels except non-self propelled vessels shall be provided with suitable steering apparatus. Port steering ram bracket is missing bolts and detaching from support. Prove proper steering operation for port steering apparatus. 46 CFR 182.30-1(a) 10,C

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Port engine room bilge suction is inoperable. Prove proper operation of port bilge suction. 46 CFR 182.30-1(a) 60, c

The bilge pipe piercing the collision bulkhead shall be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from above the weather deck. Screw-down valve in port and starboard forepeak voids is inoperable. Prove proper operation of valve in forepeak voids. 46 CFR 182.25-5(d) 60, c

Mechanically propelled "L" vessels shall comply with the provisions of subchapter F of this chapter. Rigid non-metallic materials are acceptable for use in bilge piping systems on all other vessels. Port engine room bilge piping is constructed of PVC. Provide metallic piping in port engine room bilge system, replacing PVC piping and area with "JB Weld" patch.

Weak link- Marking- Each link certified by the manufacture to meet the requirements of this subpart must have a corrosion resistant waterproof tag attached to it that has the following information. Buoyant Apparatus/Person Capacity/ Breaking Strength/ Meets USCG/ Made by/ Date. Cannot be read. all must be replaced prior to carrying passengers.

Wet Exhaust Leak- The engine head, block, and exhaust manifold shall be water jacketed. Port side MDE has water leaking from exhaust flange at the inner hull penetration. 46 CFR 182.20-10(b)(1)

Employers must ensure that they and their crewmembers meet the requirements of this part. Pre-employment drug testing in accordance with 46 CFR 16.210 has not been conducted for the crew minimum manning. Results must be in prior to carrying passengers. 28 July 2020, Vessel has min crew to conduct 12 hour operations, Def stays until all crew test results come in.

Invalid station bill- the station bill required by this paragraph (a) of this section must set forth the special duties and duty station of each crew member for various emergencies. Posted station bill listed MMC ratings that this vessel does not employ.

Life Jacket Placards- Placards containing instructions for donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel has four different types of life jackets and only one type of placard posted. Vessel must have a placard for each type of life jacket prior to carrying passengers.

Lagging- Protective gratings shall be provided at such location where persons or gear might come in contact with the exhaust pipe. (Diesel 182.20-15 sends to gas regs) STBD MDE is missing lagging at the fan area, STBD SSDG and Port SSDG have missing lagging at various areas of the exhaust piping. 46 CFR 185.15-20(g)

Eng Room Fuse Panel- All electrical installations shall be to the satisfaction of the officer in charge of marine inspections. STBD Machinery space has three fuse panels the forward one has missing labeling, the second and third one have open fuses allowing for accidental shock. Must be blanked off for protection of crew. 46 CFR 183.01-15 (a)

Hydraulic pump guard- Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Port machinery space has a hydraulic pump without guards on rotating machinery. 46 CFR 177.35-15

Cable Run- All electrical installations shall be to the satisfaction of the officer in charge marine inspections. 3rd void space with air handler has cable running port to stbd held up by zip ties. Proper connectors must be installed. 46 CFR 183.01-15 (a)

In general compliance with the standards established by a recognized classification society will be considered satisfactory evidence of the structural adequacy of the vessel. Port bow railing cracked, stbd forward void three cracked structural welds, #2 stbd void cracked weld above potable water tank overhead, Port forward void space two cracked welds on structural beams, #2 port void cracked weld above potable water tank overhead. 46 CFR 177.10-1 (a)

Buoyant apparatus must be approved under approval series 160.010 or other standard approved by the commandant. All buoyant apparatuses are missing approval under 160-010 or other standards approved by the commandant. However all buoyant apparatuses are serviceable. All apparatuses must be replaced or evidence shown that they are approved under 160.010 or other standards.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Fire Fighting drills must also be documented for review by the Coast Guard. Training logs for drills were not compiled prior to July 2020. Logs for all drills shall be completed moving forward.

Portable watertight containers for distress flares and smoke signals shall be clearly marked in legible contrasting letters at least 5 inches high stating, Distress Signals. Container must be marked IAW above standards.

Each vessel that operates beyond three nautical miles from coastline or on the high seas must have a FCC-type accepted Cat 1 406 mhz epirb, installed to float free. Hydrostatic release unit doesn't have expiration date affixed.

Vessel dry dock due 30 APR 2020. Prior to carrying passengers, a credit dry-dock must be conducted to the satisfaction of local OCMI.

01199- Any vessel of at least five net tons wholly owned by a citizen or citizens of the United States is eligible for documentation under this part. This includes, but is not limited to, vessels used exclusively for recreational purposes and vessels used in foreign trade. Vessels Certificate of Documentation expired SEP17.

05104-Vessels operated beyond the 20 nautical mile limitation specified in paragraph (a)(1) of this section, but not more than 100 nautical miles from the nearest land, must be equipped MF–DSC frequency transmitter meeting the requirements of §80.1101(c)(3) The MF or MF–DSC transmitter and receiver must be capable of operation on 2670 kHz. Vessels MF radio microphone does not operate. Vessel is restricted to 20 NM of land.

The part of the exhaust system between the point of cooling water injection and the engine manifold shall be properly insulated. Insulation on the port SSDG was improperly installed along with sections missing.

At each initial inspection, the firefighting equipment shall be inspected. Vessels fixed firefighting system did not show proper annual service records. Vessel shall provide prof of annual service.

Vessel must comply with the structural design requirements of the aluminum hull vessel standards. Gouge and weld identified areas (5) Port Forward (5) STBD Fwd (2) STBD Laz and (2) Fwd Bullwork rail.

Piping including exhaust gases with temp exceeds 150 degrees must be suitably insulated where necsessary to prevent injury. Replace exhasut lagging Port and Stbd Eng in Kind.

Vessel w/ installed toilet facility must have a marine sanitation device that complies with 33 CFR 159. Repair hole in MSD discharge pipe above deck.
