Each survival craft must be stowed— So as not to require lifting from its stowed position in order to launch, except that— A survival craft that weights 185 kilograms (407.8 pounds) or less may be lifted not more than 300 millimeters (1 foot) in order to launch. In addition to the requirements of paragraph (a) of this section, each liferaft must be stowed as follows: Each liferaft must be arranged to permit it to drop into the water from the deck on which it is stowed. A liferaft stowage arrangements meets this requirement if it— (i) Is outboard of the rail or bulwark;(ii) Is on stanchions or on a platform adjacent to the rail or bulwark; or (iii) Has a gate or other suitable opening large enough to allow the liferaft to be pushed directly overboard and, if the liferaft is intended to be available for use on either side of the vessel, such gate or opening is provided on each side of the vessel. Vessel's liferaft was located so that it must be lifted more than 1 foot to launch. In addition, vessel's liferaft was not stowed to permit it to drop into the water from the deck on which it is stowed. Modify liferaft stowage to the satisfaction of the MI. 46 CFR 141.310 46 CFR 199.130 (a)(7)(ii) & (c)(3)

Each towing vessel must have either a self-priming, power-driven, fixed fire pump, a fire main, and hoses and nozzles; or a portable pump, and hoses and nozzles. A fixed fire pump must be capable of: Delivering water simultaneously from the two highest hydrants, or from both branches of the fitting if the highest hydrant has a Siamese fitting, at a pitot-tube pressure of at least 344 kilopascals (kPa) (50 pounds per square inch (psi)), and a flow rate of at least 300 liters per minute (lpm) (80 gallons per minute (gpm). Vessel's fixed fire pump reached a maximum pressure of 34 psi. Repair to the satisfaction of the MI. 46 CFR 142.325 (a)(1)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel's port generator had a water leak. Repair to the satisfaction of the MI. 46 CFR 143.205(a)

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. This may be accomplished by an installed detection system, or by using individual battery-operated detectors meeting UL 217. Detection systems or individual detectors must be kept operational at all times when the crew is onboard the towing vessel. Vessel did not have operational smoke detection in any berthing spaces. Replace to the satisfaction of the MI. 46 CFR 142.330 (b)

Personnel dispatched from the vessel or that are working in an area on the exterior of the vessel without rails and guards must wear a life jacket meeting requirements in 46 CFR 141.340, an immersion suit meeting requirements in 46 CFR 141.350, or a work vest approved by the Commandant under 46 CFR subpart 160.053. Vessel crew members were found wearing work vests that are not of the approved type. Replace work vest to the approved type or submit for special consideration. 46 CFR 140.430 (a)

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Excess coolant and/or fuel was identified in the engine room bilge. Remove to the satisfaction of the MI. 46 CFR 142.220 (a)

The Rules concerning shapes shall be complied with by day. Vessel was missing required day shapes on board. Procure and place day shapes applicable to the vessel on board. 33 CFR 83.20 (d)

A power-driven vessel when pushing ahead or towing alongside, except as required by paragraphs (b) and (i) of this Rule, shall exhibit: (i) two masthead lights in a vertical line;(ii) Sidelights; and (iii) Two towing lights in a vertical line. Only one masthead was operational. Replace to the satisfaction of the MI. 46 CFR 83.24 (a)(c)

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: Search light, controllable from the vessel's operating station and capable of illuminating objects at a distance of at least two times the length of the tow. Vessel's search light was inoperable. Replace to the satisfaction of the MI. 46 CFR 140.725 (b)

Any towing vessel with alternating watches (shift work) or overnight accommodations must identify the duties and duty stations of each person onboard during an emergency, including:(1) Responding to fires and flooding;(2) Responding to emergencies that necessitate abandoning the vessel;(3) Launching survival craft;(4) Taking action during heavy weather;(5) Taking action in the event of a person overboard;(6) Taking action relative to the tow;(7) Taking action in the event of failure of propulsion, steering, or control system;(8) Managing individuals onboard who are not crewmembers;and (9) Managing any other event or condition which poses a threat to life, property, or the environment. Vessel's posted emergency duties and duty stations were lacking required information in regard to specific emergencies. Update to the satisfaction of the MI. 46 CFR 140.405 (b)

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. The line throwing appliance must have:(1) The equipment on the list provided by the manufacturer with the approved appliance; and(2) An auxiliary line that:(i) Is at least 450 meters (1,500 feet) long;(ii) Has a breaking strength of at least 40 kilonewtons (9,000 pounds-force); and(iii) Is, if synthetic, of a dark color or certified by the manufacturer to be resistant to deterioration from ultraviolet light. The vessel was missing 03 line throwing appliances and auxiliary line. Acquire to the satisfaction of the MI. 46 CFR 141.385 (b)

Each watertight door and watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”. Multiple signage markings were faded on the watertight doors. Repair to the satisfaction of the MI. 46 CFR 144.160 (e)

Immersion suits or anti-exposure suits must be stowed so they are readily accessible. The stowage positions must be marked with either the words “IMMERSION SUITS” or “ANTI-EXPOSURE SUITS” as appropriate, or with the appropriate symbol from IMO Resolution A.760(18). Signage was faded on the stowage location for the immersion suits. Repair to the satisfaction of the MI. 46 CFR 199.70 (c)(2)

The owner of a vessel subject to inspection under section 3301 shall- (1) ensure that such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key of which access shall only be available to the individuals described in paragraph (2); (2) establish a list of all crew, identified by position, allowed to access and use the master key and maintain such list upon the vessel, within owner records and included in the vessel safety management system; (3) record in a log book information on all access and use of the vessel's master key, including- (A) dates and times of access; (B) the room or location accessed; and (C) the name and rank of the crew member that used the master key. Vessel did not have a master key control system in place. Implement to the satisfaction of the MI. 46 USC 3106(a)

There must be an installed or portable bilge pump for emergency dewatering. The bilge pump in the lazarette is not attached and cannot dewater the space. Make repairs to the satisfaction of the attending MI prior to COI issuance. 46 CFR 143.275

All installed bilge piping must have a check/foot valve in each bilge suction that prevents unintended backflooding through bilge piping. There are no check valves installed in the bilge piping system. Make repairs to the satisfaction of the attending MI prior to COI issuance. 46 CFR 143.275

Towing machinery used to connect the towing vessel to the tow must be suitable for its intended service. It must be capable of withstanding exposure to the marine environment, likely mechanical damage, static and dynamic loads expected during intended service, the towing vessel's horsepower, and arrangement of the tow. The high pressure hydraulic hose on the towing gear machinery is cinched between machinery and cannot be inspected for structural integrity. Provide satisfactory evidence of workable conditions of the hydraulic hose to the satisfaction of the attending MI prior to COI issuance. 46 CFR 143.460(c)

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. Multiple watertight doors on the vessel are not watertight. Provide satisfactory evidence of doors being made watertight to the attending MI prior to COI issuance. 46 CFR 144.320(b)

Immersion suits carried on towing vessels must meet the requirements of § 199.70(c) and (d) of this chapter. Each immersion suit or anti-exposure suit must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the immersion suit or anti-exposure suit. One of the immersion suit does not have a working lifejacket light. Make repairs prior to COI issuance. 46 CFR 141.350 (b) 46 CFR 199.70(c)(4)(i)

When the towing vessel is underway, the EPIRB must be stowed in its float-free bracket with the controls set for automatic activation and be mounted in a manner so that it will float free if the towing vessel sinks. The EPIRB does not have a hydrostatic release to allow for automatic activation. Make repairs prior to COI issuance. 46 CFR 141.380(b)

A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 of § 141.370 for specified areas where the vessel operates. The vessel does not have the required number of distress signals for its intended routes (6 day and 6 night). Make repairs prior to COI issuance. 46 CFR 141.375(a)

Unless an exemption is granted under § 26.09 and except as provided in paragraph (a)(4) of this section, this part applies to: Every towing vessel of 26 feet or over in length while navigating. Vessel does not have a valid Bridge to Bridge Radiotelephone certificate. Obtain certificate prior to COI issuance. 46 CFR 26.03(a)(3)

All owners and operators of an existing towing vessel must have onboard a valid COI prior to the specified phase in period. This vessel has not scheduled an inspection or obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection immediately and the vessel may no longer be permitted to operate.

The navigation and shipping laws state that a vessel may not be operated unless certain manning requirements are met. Vessel was not properly manned for inspection. Provide properly documented crew prior to conducting commercial operations.

The following alarms must be provided: Main engine and auxiliary generator low lubricating oil pressure and high cooling water temperature; high bilge levels, low hydraulic steering fluid levels. Alarms must be visible at the operating station. Required alarms not at the operating station.

All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. Pressure vessels’ data plates were painted over and unreadable.

Pressure vessels must be equipped with one or more spring loaded relief valves. The total relieving capacity of such a relief valve must prevent pressure from exceeding the MAWP as established by the manufacturer, by more than 10 percent. Relief valves on both pressure vessels did not have information for their set points.

Each mattress must comply with either: the consumer product safety commission standards for mattress flammability, 16 CFR part 1632, subpart A, and not contain polyurethane foam; or IMO Resolution A.688(17), (see 136.112 of this subchapter) in which case the mattress may contain polyurethane foam. Information was not available onboard to show compliance with either standard. Provide documentation showing the mattresses comply with this part.

Extension cords must not be used as a permanent connection to a source of electrical power. Extension cords used in 01 berthing and in galley near the stove appeared to be permanent installations. Remove extension cords.

Individual circuit breakers on switchboards and distribution panels must be labeled with a description of the loads they serve. Bridge breaker box switches 14, 18 and galley breaker box switches 5, 10, 13, 14 were in ‘on’ position but not labeled.

Each watertight hatch must be marked on both sides in clearly legible letters at least 1” high: WATERTIGHT HATCH-KEEP CLOSED. Lazarette hatch did not have proper marking.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. Approximately seven, five gallon buckets of oil located underneath deckplate near starboard shaft. Remove and properly stow buckets.

Each towing vessel operating on oceans or coastwise routes must have a means to take blood pressure readings, splint broken bones, and apply large bandages for serious wounds. Required equipment was not available for inspection.
