VICTORY I

Detectors must be responsive to heat, smoke, or other products of combustion, flame, or any combination of these factors. MI discovered fire detector at casino cashier (03-19) was not operable. Replace with in-kind detector and make ready for re-inspection.

Double doors shall be so arranged that either door may be closed and latched independently. MI discovered FSD's 8 and 19 do not close properly. Adjust doors and hold backs so they operate as designed.

An electric apparatus must be designed, constructed, and installed to prevent any person from accidentally contacting energized parts. MI discovered in the port Forward A/C room, an electrical system is wired incorrectly. Correctly wire the system and make ready for re-inspection.

There shall be at least two means of escape from all general areas accessible to the passengers or where the crew may be quartered or normally employed. Engine room FWD port side escape hatch inoperable. Repair engine room port side forward escape hatch.

Portable fire extinguishers and their stations must be numbered in accordance with § 78.47-30 of this subchapter. Label new fire extinguishers with corresponding station numbers.

The bilge pumping system must be capable of operation under all practicable conditions after a casualty whether the ship is upright or listed. Bilge system overboard valve was found to be inoperable. Repair or replace with in kind valve.

Each vessel must carry lifebuoys approved under approval series 160.150. Replace retro reflective material, replace line to self-igniting light and mark each stowage position with “Life Buoy” or with the appropriate symbol from IMO Resolution A.760(18).

Each lifebuoy stowage position must be marked with either the words “LIFEBUOY” or “LIFE BUOY”, or with the appropriate symbol from IMO Resolution A.760(18). Install approved stickers at each station.

Means shall be provided for closing all other openings to the space protected from outside such space. In this respect, relatively tight doors, shutters, or dampers shall be provided for openings in the lower portion of the space. The construction shall be such that openings in the upper portion of the space can be closed off either by permanently installed means or by the use of canvas or other material which is normally carried by the vessel. Close hole on upper portion of EDG space.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel reported stack fire while underway during normal operations. Provide the CG a company letter head stating the new procedures that are in place to prevent additional stack fires.

Each fireman's outfit must consist of one self-contained breathing apparatus, one lifeline with a belt or a suitable harness, one flashlight, one flame safety lamp, one rigid helmet, boots and gloves, protective clothing, and one fire ax. Procure flashlights and fire axes for each crew member on fire team.

A complete recharge shall be carried for each self-contained breathing apparatus, and a complete set of spare batteries shall be carried for each flashlight. The spares shall be stowed in the same location as the equipment it is to reactivate. Add additional bottle to inventory.

Each flexible cord and cable must meet the requirements in Sections 4.4.2. and 4.4.6 of IEEE 45.8-2016, Article 400 of NFPA 70, ANSI/NEMA WC-70, or ANSI/UL 62 (all incorporated by reference; see § 110.10-1 of this subchapter). No flexible cord may be used except:(1) As allowed under Sections 400.10 and 400.12 of NFPA 70; and(2) In accordance with Table 400.4 in NFPA 70. Vessel had unapproved power strips installed in multiple locations on board. Use approved power strips or install junction boxes for permanent installations.

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment required by the regulations in this subchapter is provided, maintained, and replaced as indicated. CO2 hoses were not properly labeled. Document required CO2 hose test and label hose IAW NFPA 10 Chapter 7.4

Vessel captain made reports that the vessels port MDE secured itself while transiting inport/mooring. Vessel also had a fire in the stbd MDE stack. Provide a repair plan to the local OCMI/marine inspector and make vessel available for visit/inspection prior to getting underway with passengers.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel captain made reports that the vessel made contact with their concrete mooring pier. After further investigation, two openings in the hull were discovered on the port quarter/steering compartment. One is approx. 12.5" long and the other is 4" long. Provide a repair plan to the local OCMI on details of repairs and testing procedures for approval. Make vessel available for non-destructive testings in repair areas.

Each inflatable lifesaving appliance and marine evacuation system must be serviced—(i) Within 12 months of its initial packing. Vessel was missing one IBA IAW COI. Overall passenger count is reduced to 1300 until serviced IBA is on board vessel.

Except as provided in paragraphs (b) through (d) of this section, this subpart applies to vessels with watertight doors in bulkheads that have been made watertight to comply with the flooding or damage stability regulations in this subchapter. STBD side deck 4 door is not weather tight. Properly make repairs on door.

Each first-aid kit must meet the requirements in ISO 18813:2006(E) paragraph 4.12 (incorporated by reference, see § 199.05). Both kits in port and stbd rescue boats are expired. Procure new kits.

The searchlight's power source must be connected to the searchlight using watertight electrical fittings. Port rescue boat searchlight does not properly work. Repair electrical fitting on rescue boat.

Tests and inspections of the lifesaving equipment shall be carried out during each inspection for renewal of certification and periodic inspection, and shall include, as applicable, a demonstration of — The proper condition and operation of lifeboats and rescue boats, including engines and release mechanisms. All strap hooks on rescue boats are deteriorated. Replace hooks with in-kind equipment.

The installation of all systems of an electrical engineering or interior communications nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J (Electrical Engineering) of this chapter. The lighting in the aft AC/mooring compartment; one light is out and the other is not properly mounted to the overhead. Make necessary repairs to both light fixtures.

All enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. There is a trash bag over a ventilation motor in the aft AC/mooring compartment. Make proper corrections in the system.

Nontank vessels subject to this subpart may not —Continue to operate on the navigable waters of the United States if— The period of the VRP authorization has expired. Vessel had expired Nontank VRP. Provide valid documentation.

In addition to the emergency power sources required under this subchapter, each self propelled vessel must have at least two electric generating sources. The #2 generator failed while the vessel was underway. The emergency generator failed to start automatically. Demonstrate proper operation to the satisfaction of an attending Marine Inspector. 46 CFR 111.10-3

The hull, structural bulkheads, decks and deckhouse shall be constructed of steel or other equivalent metal construction of appropriate scantlings. Wasted areas along the STBD side at the main deck shall be investigated to determine the extend of the wastage, area near signage aft of middle and area fwd at the turn of the bow, fwd of the rescue boat. Proposal shall be submitted in one week from issuance. 46 CFR 72.05-10 (a) Code 705 c

Radio lifesaving appliance installations and arrangements must meet the requirements of 47 CFR part 80. Three VHF radios for survival craft require sealed new back up batteries for use during emergency. Provide to the satisfaction of the attending MI. 46 CFR 199.60 (a) Code 16 c

Radio lifesaving appliance installations and arrangements must meet the requirements of 47 CFR part 80. Search and rescue transponder (SART) should have the name of the vessel plainly marked or painted on its label and be stowed in a place where it can be rapidly deployed. Vessel’s current SART expired in August of 1996. 46 CFR 199.60 Code 16c

The hull, structural bulkheads, decks and deckhouse shall be constructed of steel or other equivalent metal construction of appropriate scantlings. Wastage under signage on STBD side at the 01 deck needs to be investigated to determine if hull side shell has been affected where the lip overhang attaches to the main deck side shell. Repair to the satisfaction of the attending MI. 46 CFR 72.05-10 (a) Code 50 c

The hull, structural bulkheads, decks and deckhouse shall be constructed of steel or other equivalent metal construction of appropriate scantlings. Make permanent repairs to the hole in the overhead in way of FSD 14. 46 CFR 72.05-10 (a) Code 50 c

Ferrous pipe used for saltwater service must be protected against corrosion by hot dip galvanizing or by the use of extra schedule material. Main deck STBD side over head has a deck drain with a soft patch installed, replace piping in kind to original to the satisfaction of the attending MI. 46 CFR 56.60-3 (a) Code 50 c

Fire control diagram showing location and type of all required fire screen insulation, including main fire zone and subdivisions, stairway and elevator enclosures, control space enclosures, etc. and type of all doors in such subdivisions and enclosures. Comprehensive typical details of fire control installations, ventilations diagram, alarm systems, detecting systems, extinguishing systems, and patrol routes. Hinged doors in the A class bulkhead adjacent of FSD 26 and 27 do not meet the standard of A class fire doors. FSD13, 14, 21 and 22 are not shown on the FCP as being in an A class boundary. 46 CFR 71.65-5 (d) Code 50 c

Arrangements shall be made whereby water in the compartments will drain to the suction pipes. Remote closure of the forward bilge valve does not operate as designed in the female head port side. Provide proper access to the remote-control handle. 46 CFR 56.01-1 Code 50c

Electric installations on vessels must ensure…maintenance of system integrity through compliance with the applicable system requirements. The motor for the forward capstan is leaking oil onto the deck below. 46 CFR 111.01-1 Code 50c

All glass in windows or airports shall be at least ¼” thick and shall be fitted in steel or other equivalent framing. Aluminum frames and resilient gaskets may be used if steel retaining clips are installed in the bulkhead to keep the window in place if fire damages the gasket. Replace or renew all missing or broken window retaining clips. 46 CFR 72.05-30 NVIC 9-97 Code 50c

An STCW endorsement, will be issued or renewed only when the candidate for certification as an able seaman also produces satisfactory evidence, on the basis of assessment of practical demonstration of skills and abilities, of having achieved or maintained within the previous 5 years the minimum standards for basic training. Members of the deck department and engineering department that have had the basic training and are awaiting endorsements to the MMC may work. Proper endorsements will be required no later than the next quarterly inspection last day of June 2025. 46 CFR 12.05-3 Code 705 c

An STCW endorsement will be issued only when the candidate provides evidence of training completed, having maintained the minimum standards of competence for the following; Personal Survival Tech, Fire Prevention and Firefighting, Elementary First Aid and Person Safety and Social Responsibilities. Any licensed officer in the engineering department may only work when this training has been completed and submitted to the National Maritime Center for endorsement. All officers who completed the training and are working must have the valid endorsement by the next quarterly inspection last day of June 2025. 46 CFR 11.202 Code 705 c

Gray Water/ Each device must be free of design defects such as rough or sharp edges that may cause bodily injuries or that would allow substances to escape to the interior of the vessel. The gray water tank has wasted areas on the bottom of the tank. Gauging report shows areas up to 43%, owner has applied doublers to the tank bottom in way of leaks during the shipyard period and without CG approval. Upon review of gauging reports and due to system design (no means to bypass tank) the vessel will be permitted to defer proper insert repairs of the tank bottom. Captain or licensed officer will be required to inspect the tank bottom. This shall be completed before the first cruise of the day and after the last cruise of the day. If any leaks are found it shall be reported to the OCMI for reevaluation of this deferment. Gray water tank bottom shall be renewed at next dry dock with proper insert plates. 33 CFR 159.95 Code 705 c

Instrumentation details. Demand instrumentation displays must ve clearly readable and immediately available to operators/ Flooding alarm. The bilge alarms in the engine room are mislabeled. There is one stating forepeak that is not connected to anything and one label bow which is actually the forepeak tank. Correct to the satisfaction of the attending MI. 46 CFR 62.25-20 (c) Code 50 c

Bulkheads or decks composed of steel shall be suitable stiffened and intact. STBD side main deck wasted lower rail near boat davit. Repair to the satisfaction of the attending MI. 46 CFR 72.05-10 Code 50 c

Each self propelled vessel of this section must have an electronic or mechanical engine order telegraph system from the navigation bridge to the engine room. The vessels PORT EOT on the bridge is off by two spots, repair to the satisfaction of the attending MI. 46 CFR 113.35-3 (a) Code 50 c

All emergency equipment shall be maintained in an operative condition, it shall be the responsibility of the master and chief engineer to ascertain a sufficient number of crew are familiar with the operations of the equipment. The test of the EOT between the pilot house and the engine room was not adequate. Conduct training and implement procedures for proper testing of the engine order telegraph system. Retest to the satisfaction of the attending MI. 46 CFR 77.30-5 (d) Code 50 c

The material, design, construction, workmanship and arrangements of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards as set by ABS. The STBD engine local controls are not working. Correct to the satisfaction of the attending MI. 46 CFR 58.05-1 (a) Code 50 c

An inspection of passenger and crew quarters, toilet and washing spaces, serving pantries, galleys, etc... shall be made, in general at least once a month. There is a clogged deck drain in the fwd men's head on the stbd side main deck, inside the closet. There is a clogged deck drain in the lower galley middle of space. The stairway leading to the lower galley is very dirty with grease and grim. Clear all to the satisfaction of the attending MI. 46 CFR 71.45-1 Code 50 c

Each panelboard must meet section 17.1 of IEEE 45-2002. The circuit breaker panel labeled 401 in the chiller flat has open blanks covered with duct tape. Replace with proper blanks. 46 CFR 111.40-1 Code 50 c

Gray Water/ each device must be free of design defects such as rough or sharp edges that may cause bodily injuries or that would allow toxic substances to escape to the interior of the vessel. The gray water tank has a wasted top and two doublers on the bottom of the tank. Crop and renew the bottom with inserts IAW NVIC 7-68 and clean the top, gauge it and determine if additional inserts are required. 33 CFR 159.95 (a)(1) Code 50 c

All crew spaces are to be constructed and arranged in a manner suitable to the purpose for which they are intended and so that they can be kept in a clean, workable and sanitary condition. The deck plating on the port side of the chiller flat has a hole that could cause injury and an additional deck plate that is not stable. Correct to the satisfaction of the attending MI. 46 CFR 72.20-15 Code 50 c

Suitable covers shall be installed in way of all exposed dangerous places. The heat lagging on the stbd generator is loose and falling off. Replace it to the satisfaction of the attending MI. 46 CFR 72.40-20 Code 50 c

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he or she deems necessary to be assured of the safety and seaworthiness of the vessel. IT Space on the 4th deck in major need of housekeeping. Excess equipment, supplies, spare wiring, and rubbish made complete inspection impossible. 46 CFR 71.30-10 Code 50 c

Electrical installations on vessels must ensure protection of passengers, crew, other persons and the vessel from electrical hazards. Resecure wire penetration through bottom of j-box on starboard bridge wing and breaker labeled “Beer walk in & 3rd deck walk in” where it enters the bottom and penetrates the deck. 46 CFR 111.01-1 Code 50 c

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port. Marking worn off or missing on several ring lifebuoys. 46 CFR 199.70 (a)(2) Code 50 c

Electrical installations on vessels must ensure protection of passengers, crew, other persons and the vessel from electrical hazards. Makeshift female plug was noted on wire providing power to component below Radar/Chart-plotter. 46 CFR 111.01-1 Code 50 c

Electrical installations on vessels must ensure protection of passengers, crew, other persons and the vessel from electrical hazards. Wire providing power to blower motor on forward, starboard air handler penetrates ducting through unprotected, raw hole. 46 CFR 111.01-1 Code 50 c

Electric installations on vessels must ensure…maintenance of system integrity. Multiple wire kick-pipes penetrating the top deck were found not properly re-installed and one called out during dry dock and not repaired. 46 CFR 111.01-1 Code 50 c

Electrical installations on vessels must ensure protection of passengers, crew, other persons and the vessel from electrical hazards. Ensure coverings and/or enclosures are reinstalled on HVAC Sensor (Top deck), Transformer in forward beer cooler, j-box in aft beer cooler. 46 CFR 111.01-1 Code 50 c

Electric installations on vessels must ensure…maintenance of system integrity through compliance with the applicable system requirements. Party lights installed over the aft deck were not properly installed. Wiring was connected to a timer unit through the device door and did not penetrate the bulkhead, rather ran through the open door. 46 CFR 111.01-1 Code 50 c

Exhaust pipe installations must conform to the requirements of NFPA 302. Vessel to depart on scheduled AM trip with the following conditions: a) Have the crew take and record temps of the stack heat every half hour and advise the duty inspector if the temps rise above 150 degrees, b) restrict usage of the port SSG to only as needed, and c) upon returning to the pier, provide a camera to investigate the condition of the stack and exhaust. Advise the duty inspector of the results prior to departing on the scheduled PM voyage.

In addition to the emergency power source required under part 112 of this chapter, each self propelled vessel must have at least two electric generating sources. While underway with passenger vessel experienced a loss of both SSDG's causing the EDG to take the load. Power was restored and vessel safely moored. Vessel is restricted from operations until cause of the loss of power is determined to the satisfaction of the attending MI. 46 CFR 111.10-3

Bulkheads or decks composed of steel shall be suitably stiffened and intact. The O5 deck is to be properly repaired using good marine practice and in IAW the approved scope of work submitted from Victory Casino Cruises. Repair or renew railing around the O5 deck prior to commencing repairs to the deck.

Ships crew shall be properly licensed for the safe operation of the ship. Engineers do not have required STCW endorsements.

Efficient rails shall be installed at the peripheries of all decks accessible to passengers. Railing on the O4 deck has holes which require repair.

Navigation lights shall be fitted to display proper arc of visibility. Make permanent repairs to the port navigation light support bracket.

Bulkheads or decks of the "A" Class shall be composed of steel or equivalent metal construction, suitably stiffened and made intact with the main structures of the vessel such as shell, structural bulkheads, and decks. During drydock exam, MIs inspected an A-0 boundary per approved structural fire protection drawing and identified over 30 holes in the vessel's O5 deck due to wastage from frames 35-108. This area has previous doubler repairs dating back to May of 2011, which are still currently installed. **O4 Deck is restricted to passengers until rectified. All procedures documented in approved repair proposal, including USCG/ABS approved welders, must be followed. Repairs are to be completed to the satisfaction of the MSU Port Canaveral OCMI. No work is to begin until approval & authorization of the Port Canaveral unified command.

Inspected vessels are issued a Certificate of Inspection (COI) which indicates the minimum complement of officers and crew considered necessary for safe operation. Vessel does not have the appropriate complement of engineers with required STCW endorsements and appropriate to the tonnage of the vessel. Vessel is authorized to sail to Port Canaveral, but must ensure minimum safe manning is met prior to the carriage of passengers. 46 CFR 15.105

All passenger vessel shall have efficient guard rails or bulwarks...at the peripheries of all decks accessible to passengers. Guard rails at the peripheries of the 04 deck passenger areas are holed. **O4 weather deck is restricted to passengers until rectified. 46 CFR 72.40-5(a)

Where the term duct is used in this section, it shall include truks, plenums, and any other type of ventilation piping, chambers, or duct work. Where ventilation ducts are required to meet bulkhead requirements, the space within the duct shall be considered to the same as the space served by the ventilator, and the duct shall be insulated to meet the applicable requirements of tables 72.05-10(d) and 72.05-10(e). The fan plenum for EDG ventilation is missing A-60 insulation. A-60 insulation was removed for repairs and was not reinstalled as per approved SFP drawing. **O4 weather deck is restricted to passengers until rectified. 46 CFR 72.05-50(a) and 46 72.05-50 (c).

Rules concering lights prescribed by these rules shall be exhibited from sunrise to sunset. The port navigation light platform is substantially wasted and may show improper characteristics. 33 CFR 83.20 (c).

Each passenger vessel must carry the following: A combination of lifeboats and life rafts that have the aggregate capacity sufficient to accommodate the total number of persons on board. Vessel has removed one ESS for service. Vessel already had a passenger reduction for two life rafts. Vessels passenger count is further reduced from 1400 total persons to 1300 total persons until the ESS is reinstalled on the vessel. Code 50 c 46 CFR 199.201 (b)

Each Marine Evacuation System required must be deployed from the vessel on a rotational basis. Each marine evacuation system (ESS) must be deployed at least once every 6 years. Provide proof of the last deployment of the four installed marine evacuation systems (ESS) to the satisfaction of the attending MI. 46 CFR 199.190(k)

Every crewmember assigned to duties involving the marine evacuation system must as far as practicable participated in a full deployment of a similar system into the water either on board a vessel or ashore every 2 years but not longer than every 3 years. The four MES (ESS) installed on the vessel have a hydrostatic release that does not actually release the slide. This is to prevent accidental release and holds the release bar in place. Crewmembers were unaware of how it worked. Implement proper training on the system to prevent confusion. 46 CFR 199.180 (d)(9)

Each passenger vessel of 500 GT and over must carry on each side of the vessel at least one rescue boat. No exemption for vessel over 500 GT. Vessel's port side rescue boat will not run. At owner's request, anytime the vessel is away from the dock it will require the M/V CLARA DEE within 3 NM fitted with recuse boat required equipment. This is in effect until the port side rescue boat is repaired to the satisfaction of the attending MI. 46 CFR 199.202 (a)

Bulkhead and decks shall be classed IAW the space categorization in table 72.05-10(d) and meet the required structural fire protection requirements required for that space. Structural fire protection was noted missing in the overhead of the forward fan space, port side main deck. 46 CFR 72.05-10

Each panelboard must have a circuit directory and description of each load. Panel 1B3 directory does not reflect the arrangement inside the panel board. 46 CFR 111.40-11

Cylinders must always be secured and when not in use they must be stowed in a rack in the upright position, with the valve protection cap in place. Secure CO2 soda bottles in the soda room. 46 CFR 147.60

All passenger vessels shall have efficient guard rails or bulwarks on decks and bridges as follows: The height of rails and bulwarks shall be at least 39 1/2" from the deck. Stanchion for railing on top deck found wasted and broken free. 46 CFR 72.40-5

In general, compliance with the standards established by ABS will be considered satisfactory evidence of the structural efficiency of the vessel. Repair hole in superstructure on port side of fan room in way of aft mooring deck. Peel back rubberized trim to prove condition of bulkhead at base. 46 CFR 72.01-15

At each annual inspection the passenger and crew quarters, toilet and washing spaces, galleys, serving pantries, lockers, etc. shall be examined by the inspector to be assured that they are in a sanitary condition. Numerous fruit flies were observed in multiple food and beverage spaces. 46 CFR 71.25-40

On vessels required by 112.05-5(a) to have a temporary emergency power source, the following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source. Illuminated signs with the work "EXIT" in red letters throughout a passenger vessel to the direction of escape to the open deck. Exit sign adjacent to the entrance to the Card Room would not remain illuminated when tested. 46 CFR 112.15-1

The prime mover of a generator set must have all accessories necessary for operation and protection of the prime mover. Vessels emergency generator was found with low cooling water in the tank and shut down approximately 2 minutes after taking the load. Vessel will implement preventive maintenance to account for water evaporation. 46 CFR 112.50-1 (a)

ASTM F 1197-00 Standards specification for sliding water tight doors. Incorporated by reference. A red light shall indicate that a door is fully open and a green light shall indicated that it is closed. Vessel's water tight doors were lighted incorrectly and corrected on the spot. 46 CFR 170.015 ASTM F 1197-19

Each passenger vessel must carry the following: A combination of lifeboats and life rafts that have the aggregate capacity sufficient to accommodate the total number of persons on board. Vessel has removed two life rafts and is restricted to 1400 persons until life rafts are replaced on vessel. 46 CFR 199.201 (b)

Vital systems that are automatically or remotely controlled must be provided with instrumentation to monitor system parameters necessary for the safe and effective operation of the system. Murphy switch for low level hydraulic expansion tank forward bow prop is not operating as designed and gives no alarm. 46 CFR 62.25-1 (a)(4)

Electric fired hot water supply boilers that have a capacity not greater than 120 gallons and are listed as approved under UL are exempted from requirements of subchapter F. Small UL approved water heater in the MSD space is no longer serviceable and must be replaced or removed. 46 CFR 53.01-10

The painter for a rigid life raft must have a float-free link meeting the requirements of part 160, subpart 160.073 of this chapter secured to the end of the painter that is attached to the vessel. The painter was secured to the ship. 46 CFR 199.175(21)(ii)

All fire detecting and extinguishing systems, piping, valves and alarms shall be checked to ensure the system in in proper operating condition. Smoke detectors on decks 2 and 3 show alarm after the alarm has been acknowledged. Cleared by Fire Servicing Tech. 46 CFR 71.25-50

Ferrous pipe used for salt water service must be protected against corrosion by hot dip galvanizing or by the use of extra heavy schedule material. The fire main on the STBD aft stairwell on the 3rd deck has a hole and needs to be replaced to the satisfaction of the attending MI. 46 CFR 56.60-3 (a)

Where extinguishing systems or equipment are not required but are installed, the system or equipment and its installation must meet the requirements of this part. In accordance with NFPA 1852 (A.4.7.5) SCBA cylinders must be recertified (hydrostatically tested) every three years for composite bottles and every five years for steel bottles. Vessels SCBA cylinders were last hydrostatically tested in July 2018. Recertify the cylinders to the satisfaction of the attending MI. 46 CFR 76.01-5 (a)

At each annual inspection the passenger and crew quarters, toilets and washing spaces, galleys, serving pantries, lockers, etc. shall be examined by the inspector to be assured that they are in a sanitary condition. The following issues were noted: Standing water in the lower galley refer, trash and crumbs in the pantry and under the pantry at Atlantic Café, trash under cabinets in the sports book bar and gunk in the ice maker of the sports book bar. Clean to the satisfaction of the attending MI. 46 CFR 71.25-40

Electrical installations on vessels must ensure: Protection of passengers, crew and other persons from electrical hazards. The vessel is required to implement a proper electrical lock out tag out procedure with list of items tagged out and all breakers that should not be closed shall be tagged out. Numerous breakers and other electrical devices are simply labeled do not use. Repair to the satisfaction of the attending MI. 46 CFR 111.01-1

Doors other than those which are normally locked such as from staterooms, fan rooms, lockers, etc.. shall be self closing type capable of closing against a 3 1/2 degree list and numbered. Fire screen door FD 14 did not properly close, repair to the satisfaction of the attending MI. 46 CFR 72.05-25 (b)(9)(i)

There shall be at least two means of escape from all general areas accessible to the passengers or where the crew may be quartered or normally employed. The emergency escape markings on doors ESS 2 and 4 is wasted and must be replaced to the satisfaction of the attending MI. 46 CFR 72.10-5

Electrical equipment must be arranged as far as practicable to prevent mechanical damage to the equipment from the accumulation of dust, oil vapors, steam, or dripping liquids. Id the source of water leaking in card room and repair to the satisfaction of the attending MI. 46 CFR 111.01-3 (a)

MTSA-The VSO must ensure that at least one security drill is conducted at least every 3 months. Vessels last logged security drill was 16 June 2023. VSO must conduct a security drill and properly log it into the security logs to be maintained on the vessel for USCG inspections for at least 2 years. 33 CFR 104.230 (b)

All vessels except unmanned barges shall be provided with satisfactory bilge pumping plant capable of pumping from and draining any water tight compartment except for ballast, oil and water tanks. Vessel could not demonstrated bilge system in two tested compartments, the MSD space and the forward hold number 2. Repair to the satisfaction of the attending MI. 46 CFR 56.50-50 (a) (1)

Machinery capable of injuring personnel shall be provided with adequate guards. Heat shield on jacket water on #2SSG is missing.

It shall be the duty of the owner, master, or person in charge to require all hand portable fire extinguishers maintained and inspected. The seal on CO2 extinguisher #15 in the lower galley was missing.

Pressure vessels shall marked and stamped MAWP, serial number, class, manufactures name, etc. Data plates are painted over and required information is not visible.

The design and arrangement of main propulsion machinery must be equivalent to the standards established by ABS. Self propelled vessels must have an electric or mechanical engine order telegraph system from the bridge to the engine room. The starboard side EOT is out of commission.

A common rail bilge system must have a stop-check valve or both a stop-check and a check valve provided in each branch line. Stop check valves could not be located or identified.

Piping systems shall be installed for the efficient operation of the vessel. Three pipe hangars in the chiller flat found broken or wasted.

In general, compliance with the standards established by ABS will be considered satisfactory evidence of the structural efficiency of the vessel. In the chiller flat, soft patches were found on the longitudinal overhead pipe, disconnected drain pipe from the deck above, and a weeping leak in a pipe leading to the gray water tank.

Each panelbaord must have a circuit directory and description of each load. Panel 1D6 in the upper galley has breakers not listed in the directory.

At each annual inspection the passenger and crew quarters, toilet and washing spaces, galleys, serving pantries, lockers, etc., shall be examined by the inspector to be assured that they are in a sanitary condition. Excess grease observed on the SFP in the upper galley overhead, unknown substance on the deck under the griddle.

Piping systems shall be examined under working conditions as required by the marine inspector. Exhaust fire during voyage caused potential damage to exhaust piping, concealed by the funnel. Upon inspection pipping found in structurally sound condition but was in need of excessive soot removal to prevent re-occurrence of fire.

The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practical. The ship operates on a limited route and at generally reduced speed which causes buildup of soot in the exhaust pipe. Develop and put in a place a plan to remove soot and reduce the likelihood of engine exhaust fires.

Double doors capable of independent operation and latching may have a clearance between the doors not to exceed 1/8 inch. Repair hinges to exterior door causing door to rub on the wheelchair ramp, preventing doors from fully closing. 46 CFR 72.05-25

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. Secure CO2, soda bottles in the soda room. 46 CFR 147.60

All emergency equipment shall be maintained in an operative condition, and it shall be the responsibility of the master and chief engineer to ascertain that a sufficient number of the crew are familiar with the operation of the equipment. Emergency Escape Breathing Device was found to be beyond it's service life.

The respective piping systems installed on ships and barges shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. Noted expansion tank for "Night Generator" missing cap.

Passageways and stairway enclosures shall contain only fire resistant furnishings. Stairwell landing/passenger assembly area on lower deck contained a newly installed ticket redemption electronic appliance which is considered a potential source of ignition and not fire resistant

All equipment should be mounted to prevent dislodgment by rolling or pitching. Several cooking and/or heating appliances were found capable of shifting or rolling.

Arrangements shall be made whereby water in the compartments will drain to the suction pipes. Water pumped into the forward hold would not completely drain aft to the installed bilge suction.

Each opening in a watertight bulkhead must have a means to close it watertight. Light could be seen through the Class 1 door onto the forward mooring deck with the door dogged closed.

Each receptacle outlet must be suitable for the environment in which it is installed. Ensure GFCI outlets are installed in exterior locations such as the “Dolphin Bar”.

Ensure all electrical cords and surge protectors that remain plugged into an outlet are not hanging by the plug or other unnecessary tension, and meet the appropriate UL standard. Inventory and ensure all surge protectors installed meet Marine Safety Alert 03-13b and remove any that are UL listed as 1363.

Each lighting fixture must be on a lighting branch circuit. Hard-wire electrical power to overhead lighting/E-light in the “Dolphin Bar”.

Each cable entering a box must be protected, secured and watertight. The shore power cables have conductors that are partially exposed in way of the shore tie plug.

Doors capable of release from a control station shall be designed to automatically close. Fire doors 15, 16, 18, 26 and 33 did not close as designed.

All fire detecting and extinguishing systems, piping, valves and alarms shall be checked to ensure the system in in proper operating condition. The seal on fire pump 2 leaks excessively.

A vessel at anchor shall exhibit where it can best be seen…At or near the stern and at a lower level than the light prescribed in paragraph (i) of this Rule, an all-round white light. Aft anchor light obstructed by installed IT equipment.

The construction of lights and shapes and the installation of lights on board the vessel must satisfy the Commandant, U.S. Coast Guard. Starboard sidelight had a broken latching/securing mechanism.

It shall be the duty of the owner, master, or person in charge to require and have performed at least once in every twelve months the tests and inspections of all hand portable fire extinguishers, semiportable fire extinguishing systems, and fixed fire extinguishing systems on board…and… shall keep records of such tests and inspections showing the dates when performed. Inspection records for exhaust hood fire systems were not available.

Each launching appliance must be arranged so that...During preparation and launching, the survival craft, its launching appliance, and the area of water into which it is to be launched are illuminated by lighting supplied from the vessel's emergency source of electrical power. Lights provided for launching liferafts were frozen or swiveled loosely and couldn't be secured in place, directed at the water.

The accuracy of the bilge alarms must be checked... according to the manufacturer's instructions. The calibration certificate for the bilge alarm, which certifies the date of the last calibration check, should be retained onboard for inspection purposes. The calibration certificate expires in one day.

At each annual inspection the passenger and crew quarters, toilet and washing spaces, galleys, serving pantries, lockers, etc., shall be examined by the inspector to be assured that they are in a sanitary condition. Beverage concentrate was observed on decks and fruit flies observed in bars.

Each rescue boat must carry the equipment listed in this paragraph...first aid kit approved under 160.041. Both first aid kits contained expired medications.

Install proper wire penetrations for conduit elbow behind 2nd deck aft bar.

Ensure GFCI outlets are installed in all locations where required (i.e. galleys and bars within 6 ft of a sink), to include the following observed. a. 4th deck aft bar below sink b. Crew galley IWO small sink and stand-up refrigerator c. 2nd deck, aft bar.

Properly secure outlet box within cabinet on 3rd deck port side of the atrium and E/R port side overhead by air supply.

Deep clean crew galley, including all greasy surfaces and make maximum available areas of interior grease-hood exhaust ducting visible for internal inspection.

Make proper repairs to the MSD to include: securing the aft macerator pump, securing macerator pump piping, repairing the pressure gauge connection to the piping, and securing the upper access plate.

The following Fire Screen Doors did not fully close and or latch: FSD 1, 2, 3, 6, 9, 11, 12, 14, 19, 21, & 35.

Doors which are used while at sea are to be sliding watertight doors capable of being remotely closed from the bridge and are also to be operated locally from each side of the bulkhead. Indicators are to be provided at the control position showing whether the doors are open or closed. The WTD indicator at the remote operating station was inoperable.
