Remote fuel shut-off stations must be marked in clearly legible letters at 1 inch high. Shut-off station on the portside must be marked to satisfaction of USCG Marine Inspector.

Flexible nonmetallic hose rated at SAE J1527 may be used in lube oil, fuel oil and fluid power systems, subject to the following requirements: For the purpose of flexibility only, flexible hose installed in lengths of not more than (30 inches) and subject to pressures of not more than 5 psig. Marine Inspector discovered SAE J1527 rated fuel line lengths exceeding 30 inches on the port side. Must conduct the following: -Evaluate all flexible fuel lines from the fuel tanks to the main engines. a. All flexible fuel line runs that are longer than 30 inches and more than 5 psig must be SAE J1942. b. All other flexible fuel line runs that are up to 30 inches may be rated at SAE J1527 (A1, A2, B1, B2). -Must make arrangements to meet the requirements or submit a Special Consideration in accordance with 46 CFR 175.550.

All vessels must have navigational lights that are in compliance with the proper sections of the Nav rules. Vessels port nav light is no longer illuminating. Provide proper operation of the vessel's navigational lights. Vessel is reduced to day light operations

Stowage of each life float must be secured with a CG-approved weak link that is proper strength for the capacity of the survival craft. The vessel's weak link no longer has approved label and markings on it. Provide serviceable weak link and prove proper installation.

Vessel aft shrouds are secured to the tangs of a chain plate via SS bolt. Due to the bolts beginning to rust the integrity of the hardware is no longer in satisfactory condition. Provide new stainless steal hardware for the aft shrouds.

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. The buoy is missing the waterlight. Restricted to daytime operations only.

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. The light must be a floating waterlight approved under approval series 161.010 or other standard specified by the Commandant. The floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. The water light is extinguished, and the lanyard is of insufficient length.

For a life float - Approved under approval series 160.027 15 person Life float onboard is cracked and not serviceable. Persons permitted reduced to 30 when operating beyond 3nm on a limited coastwise route.

Each vessel certificated to operate on a limited coastwise route in warm water must be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board. One life float is condemned and unserviceable, vessel is restricted to operations within three miles from land and must carry a 406MHZ EPIRB, until life float is replaced.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Signs of hydraulic fluid leakage observed around the stbd steering ram. Replace or rebuild to prevent hydraulic fluid leakage.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018. 30 adult and 1 child size life jackets were not marked with appropriate retroreflective material. Provide appropriate markings to the satisfaction of the attending marine inspector. Passenger count reduced to 25 adult and 4 child until corrected.

The painter connecting the life float to the float-free link shall have a breaking strength of at least 680 kilograms (1,500 pounds), except that if the capacity of the life float or buoyant apparatus is 50 persons or more, the breaking strength must be at least 1,360 kilograms (3,000 pounds). The painter on the vessel is deteriorated and shall be replaced.

Each ring life buoy on a vessel must be approved in accordance with 160.050 in subchapter Q. Grab line on life ring buoy is wasted. Replace.

At least one ring life buoy must be fitted with a lifeline. Lifeline missing. Install IAW 46CFR180.70(c).

A vessel must carry one floating waterlight, unless it is limited to daytime operation. Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Install floating waterlight.

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The nylon locknut that is used to secure the starboard upper shroud is not fully seated as the bolt is too short. Repairs shall be made so that the nylon lock nut is fully seated to prevent it from backing off.

Rails should consist of evenly spacing between courses and the spacing between courses shall be not greater than 12 inches. The courses on the port quarter of the vessel are wasted and shall be repaired.

Exhaust piping must be properly supported by noncombustible hangers or blocks. The exhaust piping on the port side is not secured.

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The hydraulic steering hoses on the port side are secured with wasted rope and shall be properly secured to prevent damage.

A positive shutoff valve is shall be installed at the hull penetration when an electric bilge pump is installed. The electric bilge pump installed in the port side fuel tank compartment does not have a positive shutoff valve installed. Install positive shutoff valve at hull penetration and ensure that the discharge hose is properly supported.

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. The vessel has a cracked horizontal frame stiffener aft of the rudder shaft in the port side engine compartment. Submit repair proposal to be approved by the attending USCG marine inspector.

All engines must have at least two means for stopping the engines under any operating conditions. The stop switch for the starboard engine is inoperable. Repair.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Vessel has a non-marine grade, non-UL rated, inverter installed by the public address system. The inverter is not permanently installed and has an adapter that allows three devices to be plugged into its single outlet. The invertor shall be removed. If an invertor is required it shall be installed to the satisfaction of the attending USCG inspector and shall be of proper type. Additionally, an electrical line drawing is required to be submitted and approved by USCG marine inspector prior to installation.

The owner shall be prepared to conduct tests and have the vessel ready for inspection of the following: On a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Vessel did not have a preventative maintenance program or a sail rigging plan onboard as required by USCG NVIC 02-16. Both shall be placed on board.

Shutoff valves, installed so as to close against the fuel flow, mst be ftted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, perferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 milimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Electric olenoid vlaves must not be used, unless used in addition to the manual valve. The vessel's port side emergency fuel shut is inoperable. Please make permenant repairs to the port side emergency fuel shut.

Vessel needs to conduct a third party rigging survey by a recognized rig smith to include all sail rigging components on deck and aloft. A U.S. Coast Guard marine inspector shall be in attendance for this inspection. 46 FR 176.840

Vessel needs to have a valid FCC radio safety certificate onboard. 47 CFR 13.

Vessel needs to update their emergency instruction list to capture their nature of operations.

Submit all information/documentation as requested by MSC email dtd 4JAN2012 in order for a review to be completed for the issuance of a new stability letter to maintain the vsls current person capacity. Failure to submit all required information/documentation to MSC by 4FEB2012 will result in the total number of persons allowed to be reduced from 55 to 47 to be in compliance with the new AAWPP.

Clean up and install supports for all wiring in port forward space. Wiring currently loose and hanging in bilge.

Make port #2 space forward bulkhead watertight. Owner had run wiring through bulkhead and failed to make watertight.

Make engine room spaces vapor tight. Vessel currently has (2) piece bulkheads that leave portions of spaces open to atmosphere potentially rendering fixed fire system ineffective.

Install insulation on covers to both engine room spaces. Insulation appears to have been removed.

Remove all combustibles from engine room spaces. Vessel currently has combustibles stored IVO batteries in spaces.

Remove wet wood from beneath port engine room hatch and repair as original.

Remove wet wood from lower stringers in port engine room and repair as original.

Remove wet wood and repair as original stanchion support in port #2 space.

Remove wet wood and repair as original area under rudder post in steering flat.

Make the following areas available for USCG inspection: (1) port inboard area 3' fwd of transom, 1' above waterline, (2) stbd outboard area approx. 2' above waterline amidships, and (3) port underside of wing deck measuring approx. 12" x 24".

Provide comprehensive rigging survey and detailed description of all components to include sizes, age and replacement schedule.

Installation of the EPIRB under the overhang of the helm cockpit counter is not satisfactory. Mount EPIRB in float free arrangement so there are no obstructions in accordance with 46 CFR 180.64.

Vessel did not have properly operating high level bilge alarms in the forward most compartments on both the port and starboard bow. Vessel did not have high level bilge alarm indicators at or near the helm control station for the port and starboard bow compartments. Install and provide proper operation of high level bilge alarms with a visual and audible indicators for the said compartments.

Found no aspirin tablets and the eye wash solution was expired in the first aid kit. Provide aspirin and eye wash for first aid kit in accordance with 46 CFR 184.710.

Provide proof of 2007 MIS submission for DAPI Program.

Provide copies of employee acknowledgment forms for DAPI Program.

Make props and shafts available for USCG inspection.
