When operating on a body of water where the discharge of treated or untreated sewage is prohibited by the EPA under 40 CFR 140.3 or 140.4, the operator must secure each type I, II, or III marine sanitation device (MSD) in a manner which prevents discharge of treated or untreated sewage. Acceptable methods of securing the device include: 1) closing the seacock and removing the handle, 2) padlocking the seacock in a closed position, or 3) using a non-releasable wire-tie to hold the seacock in the closed position. The valve on the line from the MSD to overboard was not properly secured.

The last visible hydrostatic test date stamped or otherwise marked on the 15 lb CO2 portable extinguisher in the engine room is February 2013. NFPA 10 requires hydrostatic testing of portable CO2 extinguishers at 5 year intervals.

Nonmetallic flexible hose must be in accordance with SAE J1942. Nonmetallic flexible hose may be used in vital fresh and salt water systems in lengths not exceeding 30 inches. There is a 4-5 foot section of flexible hose in the fire main supply piping in the engine room. The hose is connected via double hose clamps.
