Each ring life buoy on a vessel must: Be approved in accordance with § 160.050 in subchapter Q of this chapter. Per 46 CFR 160.050-3 the grab line shall be 3/8" diameter polyethylene, polypropylene or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 lbs. Vessel ring life buoy has a broken grab line, replace to the satisfaction of the attending MI. 46 CFR 180.70 (b)(1)

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Drills should be conducted at a minimum in accordance with 46 CFR 185.420 at least monthly. All drills shall be logged or otherwise documented for review by the Coast Guard upon request. Drill entry shall include the following: Date of drill and training and general description of scenario or training topics. Vessel had no drills logged. Correct to the satisfaction of the attending MI. 46 CFR 185.524

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandon ship or recover of persons who have fallen overboard, in accordance with 46 CFR 185.420 every three months. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following: Date of drill and training and general description of drill scenario and training topics. Vessel has not documented drills, correct to the satisfaction of the attending MI. 46 CFR 185.520

For each marine casualty required to be reported by 46 CFR 185.202 the owner, agent, master, or person in charge of the vessel shall determine whether there is any evidence of alcohol or drug use by individuals directly involved in the casualty. Alcohol testing for serious marine incidents defined by 46 CFR 4.03-2 must be conducted within 2 hours in accordance with 46 CFR 4.06-3. Vessel operator must obtain federally approved alcohol screening device to the satisfaction of the attending MI. 46 CFR 185.210 (a)

A vessel not required to have a power driven fire pump by 46 CFR 181.300 must have at least three 2 1/2 gallon buckets with an attached lanyard satisfactory to the OCMI, placed as to be easily available during an emergency. The words "FIRE BUCKET" must be stenciled in a contrasting color on each. Vessel was missing 01 bucket, provide to the satisfaction of the attending MI. 46 CFR 181.610

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precautions against hazards that might endanger the vessel and the persons being transported. In accordance with SECJAX-WI-DOM-01 granted solo operations requires signage for contacting the company office or first responders that is simple and available to the passengers. Place signage next to existing life jacket placards to the satisfaction of the attending MI. 46 CFR 185.100

Any vessel of at least 5 net tons which engages in coastwise trade must have a Certificate of Documentation. Valid COD not on board.

The master of a vessel will have mounted at the operating station, emergency instructions. Emergency Instructions not posted.

The Certificate of Inspection must be kept on board in a weathertight container readily available for use by the crew and display to passengers and others on request. Most current COI not on board vessel.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places. Lifejacket donning instructions were not posted.
